ABSTRAK
Leukemia limfoblastik akut LLA merupakan penyakit keganasan yang banyak terjadi pada anak-anak. Walaupun demikian, LLA memiliki prognosis yang baik jika ditangani dengan dini dan sesuai protokol yang ada. Cytarabine merupakan salah satu obat yang dipakai di dalam menangani LLA risiko tinggi pada fase reinduksi. Salah satu efek samping cytarabine adalah terjadinya kerusakan jaringan hati yang akan menghambat terapi LLA. Hingga saat ini, belum ada penyesuaian dosis pemberian cytarabine kepada anak dengan usia yang lebih muda dimana kerusakan jaringan hati lebih rentan kepada anak dengan usia muda. Tujuan: Penelitian ini bertujuan untuk mengetahui perbedaan nilai enzim transaminase pada kelompok usia yang berbeda. Metode: Desain penelitian adalah potong lintang dengan consecutive sampling pada 58 pasien yang sesuai kriteria. Data didapatkan dari rekam medis RS Cipto Mangunkusumo dan Laboratorium Departemen Patologi Klinik RSCM. Hasil: Data ALT dan AST pada kelompok usia yang berbeda menunjukan persebaran yang tidak normal dengan nilai median AST 46.5?/L dan ALT 48.5?/L. Perbandingan median dua kelompok dilakukan dengan uji Mann Whitney menunjukan hasil yang tidak signifikan AST p=0.374; ALT p=0.773 . Pada kelompok usia 1-12 tahun, jumlah subjek dengan peningkatan ALT dan AST diatas dua kali batas atas normal lebih banyak dibandingkan dengan kelompok usia 13-18 tahun. Kesimpulan: Perbedaan median antara kelompok usia 1-12 tahun dan 13-18 tahun pada pengukuran ALT dan AST tidak menunjukan kebermaknaan secara statistik maupun secara klinis. ABSTRACT
Acute lymphoblastic leukemia ALL in a malignant disease which often occurs in children. Different from the other malignancy, ALL has a good prognosis if treated properly. Cytarabine is one of the medication that is recommended for treating high risk ALL in the reinduction phase. Despite the effectiveness of the drug, the adverse effect of cytarabine can cause liver damage that will disrupt the therapy protocol. Until now, there are no protocol that apply dose reduction in cytarabine treatment for young childrent despite the higher risk of developing liver disease. Objective This study aims to establish the difference of aminotransferase level in different pediatric age group. Method The design of the study is cross sectional with consecutive sampling in 58 patient that match the criteria. Data is obtained from Cipto Mangunkusumo national hospital medical record and laboratory results from Cipto Mangunkusumo department of clinical pathology. Results The AST and ALT level from both groups shows an uneven distribution with median of 46.5 and 48.5. Comparison of median from those groups is using Mann Whitney test that shows non significant results. The pediatric group of 1 12 years old has more subject with AST and ALT level rise more than two times upper normal limit. Conclusion The median between pediatric group aged 1 12 and 13 18 does no have a significant difference both stylistically and clinically.
