"Hydraulic Excavator PC4000 brand Komatsu merupakan unit excavator hidrolik terbesar yang beroperasi di wilayah pertambangan indonesia dengan kapasitas produksi sebesar (22 m3) yang masuk di indonesia melalui distributor tunggal resmi Komatsu yaitu PT. United Tractors Tbk. Terdapat 7 unit hydraulic shovel PC4000 yang beroperasi di wilayah pertambangan batubara Lati Mining Operation (LMO) – Kabupaten Berau, Kalimantan Timur dengan pemilik alat PT Bukit Makmur. Hydraulic Excavator merupakan alat produksi (loading) utama untuk memuat material tanah atau Over Burden (OB) yang mana kesiapan (availability) unit sangat berpengaruh besar terhadap pencapaian produksi tanah overburden. Salah satu masalah yang terjadi pada excavator hidrolik PC4000 adalah kerusakan pada klem pipa hidrolik yang menyebabkan tidak tercapainya kesiapan unit untuk menunjang proses produksi. Untuk mengatasi problem tersebut berdasarkan hasil analitik fishbone – cause/effect diagram dibuatlah upaya perbaikan dan modifikasi bagian klem pipa hidrolic dengan merubah jenis material yang digunakan. Dalam penerapan upaya modifikasi dan perbaikan dilakukan beberapa proses seperti penentuan tindakan perbaikan, simulasi perbaikan, persiapan dokumen kerja yang terkait perizinan, flow proses kerja serta kajian keselamatan dan kesehatan kerja (JSA & WI) dan proses evaluasi kerja
......Hydraulic Excavator PC4000 Komatsu brand is the largest hydraulic excavator unit operating in the Indonesian mining area with a production capacity of (22 m3) which enters Indonesia through Komatsu's official sole distributor, namely PT. United Tractors Tbk. There are 7 PC4000 hydraulic shovel units operating in the Lati Mining Operation (LMO) coal mining area - Berau Regency, East Kalimantan with the owner of PT Bukit Makmur. This Hydraulic Excavator is the main production (Loading) tool for loading soil material or Over Burden (OB) where the readiness (availability) of the unit greatly influences the calculation of achieving annual overburden soil production. One of the problems that occurred with the PC4000 hydraulic excavator was damage to the hydraulic pipe clamps which caused the units to not be ready to support the production process. To overcome this problem, based on the results of the analysis of the fishbone – cause/effect diagram, an attempt was made to repair and modify the hydraulic pipe clamps by changing the type of material used. In implementing modification and improvement efforts, several processes are carried out such as determining corrective actions, simulating repairs, preparing work documents related to licensing, work process flow and occupational safety and health studies (JSA & WI) and work evaluation processes"
