"Penggunaan obat di puskesmas dapat dikatakan terbatas karena jumlah pelayanan kesehatan pada puskesmas yang terbatas. Penggunaan obat mendorong pelaksanaan pengadaan obat di puskesmas yang membutuhkan proses perencanaan kebutuhan. Proses tersebut membutuhkan data laporan pemakaian dan permintaan obat (LPLPO) yang dikelola oleh Apoteker. Maka dari itu, dilakukan penelitian untuk melihat efektivitas laporan pemakaian dan lembar permintaan obat (LPLPO) di Puskesmas Purwokerto Selatan pada periode triwulan II tahun 2023. Metode pengambilan data penelitian menggunakan metode retrospektif. Pada puskesmas Purwokerto Selatan, didapatkan jumlah persediaan yang terdapat dalam data LPLPO sebanyak 282 obat, alat kesehatan, dan larutan laboratorium. Hasil penelitian yang diperoleh yaitu pemakaian obat tertinggi diduduki oleh golongan vitamin sebanyak 12600 jenis (vitamin B kompleks). Sementara itu, permintaan obat tertinggi diduduki oleh golongan vitamin sebanyak 15450. Permintaan obat ditemukan angka negative yang menunjukkan tidak dilakukannya permintaan. Berdasarkan penelitian tersebut, pembuatan laporan pemakaian dan lembar permintaan obat (LPLPO) di Puskesmas Purwokerto Selatan dibuat dengan efektif karena sudah mencatat data LPLPO yang mencakup stok awal, penerimaan, persediaan, pemakaian, sisa stok, stok optimum, permintaan, pemberian, serta nomor batch dan ED secara lengkap.

The use of drugs in community health centers (Puskesmas) can be considered limited due to the constrained healthcare services provided by this Puskesmas. The use of drugs necessitates the implementation of drug procurement at Puskesmas, which requires a planning process based on usage and demand data, managed by pharmacists through Drug Usage and Request Forms (LPLPO). Therefore, a study was conducted to assess the effectiveness of Drug Usage and Request Forms (LPLPO) at the South Purwokerto Community Health Center in the second quarter of 2023. The research data were collected using a retrospective method. At the South Purwokerto Community Health Center, the data from LPLPO indicated a total inventory of 282 drugs, medical equipment, and laboratory solutions. The research findings revealed that the highest drug usage was in the vitamin category, specifically 12,600 units (vitamin B complex). Meanwhile, the highest drug demand came from the vitamin category, totaling 15,450 units. Negative figures were observed in drug demand, indicating a lack of requests. Based on the study, the Drug Usage and Request Forms (LPLPO) at the South Purwokerto Community Health Center were deemed effective as they comprehensively documented LPLPO data, including initial stock, receipts, inventory, usage, remaining stock, optimum stock, demand, dispensing, batch numbers, and expired date."
