"Latar belakang: Fungsi kognitif adalah proses mental dalam memperoleh pengetahuan, manipulasi informasi, dan penalaran yang meliputi 6 domain kognitif. Telemedis mulai dimanfaatkan pada pasien penurunan fungsi kognitif untuk anamnesis, pemeriksaan kognitif dan bahasa serta tatalaksana lanjutan terutama saat pandemi COVID-19. Sebagian besar aplikasi telemedis di Indonesia menggunakan chat untuk konsultasi pasien dan penelitian ini dilakukan untuk mengetahui gambaran keluhan kognitif pada layanan telemedis di Indonesia.Metode penelitian: Merupakan studi deskriptif dengan metode potong lintang menggunakan data sekunder dari chat pasien dengan keluhan kognitif pada aplikasi telemedis di Indonesia secara retrospektif (Maret 2020 - Mei 2021). Aplikasi telemedis melakukan filtering kata kunci keluhan kognitif pada semua chat, kemudian chat yang mengandung kata kunci melalui proses unidentifikasi dan peneliti menelaah chat terpilih.Hasil: Proporsi konsultasi pasien dengan keluhan kognitif 0,008%. Perempuan 1,7 kali lebih banyak dari laki-laki dengan usia terbanyak ≥60 tahun dan lupa atau pikun sebagai keluhan tersering (65,6%). Sebagian besar (82,3%) keluhan kognitif disampaikan keluarga dan atau pramurukti. Sebanyak 53,1% chat keluhan kognitif dijawab dokter umum dan demensia pada usia lanjut sebagai kesimpulan terbanyak (43,7%). Tidak ada deteksi dini gangguan kognitif pasien. Sebanyak 62,5% pasien mendapatkan terapi farmakologis maupun non-farmakologis dan sebanyak 42,7% pasien dirujuk dokter spesialis saraf.Kesimpulan: Proporsi keluhan kognitif 0,008% dengan perempuan 1,7 kali lebih banyak. Keluhan kognitif terbanyak adalah lupa atau pikun, usia ≥60 tahun dan sebagian besar disampaikan keluarga dan atau pramurukti. Chat sebagian besar dijawab dokter umum dengan demensia pada usia lanjut sebagai kesimpulan terbanyak sesuai template chat. Pasien mendapat rujukan dokter spesialis saraf dan terapi farmakologis maupun non-farmakologis untuk tindak lanjut.

Background: Cognitive function is mental process in acquiring knowledge, manipulating information, and reasoning which includes 6 cognitive domains. Telemedicine has started to be used in patients with cognitive decline for history taking, cognitive examinations and further management, especially during COVID-19 pandemic. Most telemedicine applications in Indonesia use chat for patient consultation and this study was conducted in order to learn the description of cognitive complaints on telemedicine in Indonesia.

Methods: This descriptive cross-sectional study using secondary data from patient chats with cognitive complaints on telemedicine in Indonesia retrospectively (March 2020 - May 2021). Telemedicine filter keywords for cognitive complaints in all chats, then chats containing keywords went through an unidentification process and researchers examined the selected chats.

Results: The proportion of patients consultations with cognitive complaints was 0.008%. Women 1,7 times more than men with the majority age ≥60 years old and forgetfulness or senile dementia as the most common complaint (65.6%). Most (82.3%) of cognitive complaints were delivered by family and/or caregiver. A total of 53.1% of chats with cognitive complaints were answered by general practitioners and dementia in the elderly is highest number of conclusion reached (43.7%). There was no early detection of the patient's cognitive impairment. A total of 62.5% patients received pharmacological and non-pharmacological therapy and about 42.7% patients were referred to neurologist.

Conclusion: The proportion of cognitive complaints is 0.008% with 1.7 times more women. The most cognitive complaints are forgetfulness or senile dementia, age ≥60 years and most of them are delivered by family and/or caregiver. Most of chats are answered by general practitioners and dementia in the elderly is the most conclusions reached. Patients receive referral to neurologist and pharmacological and non-pharmacological therapy for follow-up."
