Teknologi bisa memiliki dampak terhadap masyarakat setara dengan pengaruh
undang-undang atau kebijakan pemerintah, karena itu keputusan teknis, menurut
Andrew Feenberg, harus masuk dalam ranah demokrasi. Yang menjadi perintang
demokratisasi teknologi adalah pandangan bahwa pengembangan teknologi
sepenuhnya mengandalkan nalar tanpa pengaruh faktor-faktor sosial, sehingga
teknologi dianggap mempengaruhi tetapi tidak dipengaruhi nilai dan budaya
masyarakat. Kajian teknologi dan pemikiran konstruktivisme sosial membuktikan
bahwa teknologi dan masyarakat memiliki hubungan saling mempengaruhi.
Feenberg berusaha memadukan esensialisme teknologis dan konstruktivisme
sosial dalam teori dua-tingkat. Namun, disertasi ini berargumen bahwa
esensialisme teknologis tidak perlu dipertahankan karena peristiwa
dekontekstualisasi yang mengambil obyek alam dari konteksnya untuk menjadi
obyek teknis tetap mempengaruhi lingkungan manusia, dan dalam reduksionisme
yang memangkas sifat-sifat yang tidak diperlukan suatu obyek teknis
menghasilkan limbah yang bisa berbahaya bagi manusia. Sebagai gantinya penulis
mengajukan teori yang menjelaskan perkembangan teknologi secara serentak
dipengaruhi oleh susbistem teknologis, ekonomi dan sosial, yang masing-masing
memiliki dualitas struktur-kepelakuan dan semua subsistem ini saling
mempengaruhi yang menyebabkan spiral perubahan teknologi, ekonomi dan
masyarakat.;

regulations or policies of a government, therefore technical decision, according to
Andrew Feenberg, should belong to the democratic sphere. The constraint to
democratization of technology is the thought that technology is fully developed
based on reason, without experiencing the influence of social factors. It implies
that technology is influencing but not being influence by society. The field of
technology studies and social constructivism prove that technology and society
are constituting each other. Feenberg has reconciled technological essensialism
and social constructivism in his two-level theory. But, the dissertaion argues that
we should not keep technological essentialism because the moment of
decontextualization, which takes object from its natural context, influences the
environment, and the moment of reductionism, which strips technically useless
qualities of technical object, disposes wastes that may be harmful to human
beings. Therefore the dissertation suggest an alternative theory that explains that
the development of technology is simultaneously influenced by technological,
economic and social subsystems each of which has the duality of structure-agent
and all subsystems are influencing each other which in turn cause the spiral of the

"Latar Belakang
Resistensi antibiotik fluoroquinolone merupakan suatu ancaman serius terutama karena peranannya dalam pengobatan TB Resisten Obat (TBRO). Ketidakrasionalan penggunaan antibiotik merupakan faktor yang dapat menyebabkan resistensi. Oleh karena itu, studi ini bertujuan untuk mengukur kerasionalan penggunaan antibiotik fluoroquinolone di Puskesmas Kota Depok.
Metode
Penelitian ini merupakan penelitian deskriptif yang mengumpulkan data penggunaan antibiotik golongan fluoroquinolone dengan metode cluster sampling dari satu puskesmas kecamatan dan satu puskesmas kelurahan di Kota Depok, dipilih salah satu dari lima yang memiliki jumlah pasien terbanyak dan dekat Universitas Indonesia. Diagnosis penyakit, fluoroquinolone yang diberikan, frekuensi pemberian, kekuatan sediaan, dan durasi pengobatan diambil dari rekam medis. Data tersebut dibandingkan dengan Panduan Praktik Klinis (PPK) layanan primer 2022 untuk menilai rasionalitas. Kriteria inklusi berupa pasien mendapat antibiotik fluoroquinolone secara oral di tahun 2023, serta kriteria eksklusi berupa data tidak terbaca atau sedang dalam pengobatan tuberkulosis.
Hasil
Dari total 96 sampel yang didapatkan, ciprofloxacin 500 mg merupakan satu-satunya antibiotik fluoroquinolone yang digunakan di puskesmas. Stoknya selalu tersedia selama 2023. Ciprofloxacin paling banyak digunakan untuk infeksi saluran pernafasan atas tidak spesifik (27,08%) dan Infeksi Saluran Kemih (ISK) (26,04%). Indikasi penggunaan yang rasional hanya ditemukan pada diagnosis ISK dan tifoid sebesar 37,5%. Kerasionalan kekuatan sediaan dan frekuensi sebesar 100%. Kerasionalan durasi pemberian sebesar 97,2%. Total kerasionalan penggunaan antibiotik ciprofloxacin hanya sebesar 36,46%.
Kesimpulan
Penggunaan antibiotik fluoroquinolone di Puskesmas Kota Depok hanya 36,46% yang rasional.

Background
Fluoroquinolone antibiotic resistance is a serious threat, especially because of its role in the treatment of Drug-Resistant TB (DRTB). Irrational use of antibiotics is a factor that can cause resistance. Therefore, this study aims to measure the rationality of the use of fluoroquinolone antibiotics in Depok City Health Centers.
Methods
This study is a descriptive study that collects data on the use of fluoroquinolone antibiotics using the cluster sampling method from one sub-district and village health center in Depok City which has one of the five largest number of patients and is close to the University of Indonesia. Data on disease diagnosis, fluoroquinolone given, frequency of administration, strength of preparation, and duration of treatment were taken from medical records. The data were compared with the 2022 Primary Care Clinical Practice Guidelines (PPK) in the assessment of rationality. Inclusion criteria were patients receiving oral fluoroquinolone antibiotics in 2023, and exclusion criteria were unreadable data or undergoing tuberculosis treatment.
Results
From a total of 96 samples obtained, ciprofloxacin 500 mg is the only fluoroquinolone antibiotic used in health centers. Its stock is always available during 2023. Ciprofloxacin is mostly used for non-specific upper respiratory tract infections (27.08%) and Urinary Tract Infections (UTI) (26.04%). Indications for rational use were only found in the diagnosis of UTI and typhoid at 37.5%. The rationality of the strength of the preparation and frequency was 100%. The rationality of the duration of administration was 97.2%. The total rationality of the use of ciprofloxacin antibiotics was only 36.46%.
Conclusion
The use of fluoroquinolone antibiotics in the Depok City Health Center is only 36.46% rational.

"FORNAS disusun dengan tujuan untuk menjadi acuan bagi fasilitas pelayanan kesehatan dalam menjamin aksesibilitas obat yang berkhasiat, bermutu, aman, dan terjangkau dalam sistem JKN. Hasil pemantauan yang dilakukan oleh Direktorat Pelayanan Kefarmasian Kementerian Kesehatan pada tahun 2015 atas kesesuaian penggunaan FORNAS di fasilitas kesehatan memberikan hasil berupa kesesuaian penggunaan FORNAS pada FKTP di Dinkes Kabupaten/Kota sebesar ±70,77%. Puskesmas Kecamatan Grogol Petamburan sebagai salah satu bagian dari FKTP juga memerlukan adanya pemantauan untuk mengetahui apakah obat-obatan yang digunakan telah memasuki standar ketetapan nasional atau sekiranya dibutuhkan perbaikan yang lebih mendalam dalam pemilihan dan perencanaan obatnya. Penelitian dilakukan dengan menggunakan data berupa daftar konsumsi obat Puskesmas Kecamatan Grogol Petamburan Tahun 2021 yang selanjutnya diklasifikasi berdasarkan obat yang ada di e-FORNAS dan obat yang diluar e-FORNAS. Persentase penggunaan obat dihitung dengan rumus: total pemakaian obat dikali kekuatan obat dalam gram dibagi ddd. Hasil perhitungan dan klasifikasi menunjukkan otal penggunaan 5 kelas terapi obat-obat diluar FORNAS Puskesmas Kecamatan Grogol Petamburan Tahun 2021 sebesar 2,94% dan persentase kesesuaian penggunaan obat di Puskesmas Kecamatan Grogol Petamburan dengan FORNAS pada tahun 2021 sebesar 97,04% tergolong dalam pengobatan yang rasional dan sangat baik karna telah melebihi target kesesuaian yang ditetapkan pemerintah dalam peraturan Direktorat Pelayanan Farmasi yaitu sebesar 70%.

FORNAS was prepared with the aim of becoming a reference for health service facilities in ensuring the accessibility of efficacy, quality, safety and affordable medicines in the JKN system. The results of monitoring carried out by the Directorate of Pharmaceutical Services of the Ministry of Health in 2015 regarding the suitability of using FORNAS in health facilities gave results in the form of suitability for using FORNAS in FKTP in Regency/City Health Offices of ±70.77%. The Grogol Petamburan District Health Center as a part of the FKTP also requires monitoring to find out whether the medicines used have met national standards or whether deeper improvements are needed in the selection and planning of medicines. The research was carried out using data in the form of a list of drug consumption at the Grogol Petamburan District Health Center for 2021, which was then classified based on drugs in e-FORNAS and drugs outside e-FORNAS. The percentage of drug use is calculated using the formula: total drug use multiplied by drug strength in grams divided by ddd. The results of calculations and classification show that the total use of 5 classes of drug therapy outside FORNAS at the Grogol Petamburan District Health Center in 2021 was 2.94% and the percentage of conformity between drug use at the Grogol Petamburan District Health Center with FORNAS in 2021 was 97.04%, which is classified as rational and very good because it has exceeded the suitability target set by the government in the regulations of the Directorate of Pharmaceutical Services which is 70%."
