Abstrak :
Skizofrenia merupakan kondisi gangguan kejiwaan serius dan kronis yang dapat menyebabkan pikiran, persepsi, emosi, gerakan, dan perilaku yang menyimpang. Penderita skizofrenia dapat mengalami peningkatan emosi seperti mudah marah, mudah tersinggung, dan mengamuk yang dapat meningkatkan risiko untuk melakukan perilaku kekerasan. Perilaku kekerasan merupakan suatu keadaan dimana hilangnya kendali perilaku seseorang yang dapat diarahkan pada diri sendiri, orang lain, maupun lingkungan. Tujuan karya ilmiah ini untuk memberikan gambaran terkait penerapan asuhan keperawatan pada pasien dengan risiko perilaku kekerasan melalui terapi musik. Penerapan terapi musik yang digunakan yaitu terapi musik aktif atau bernyanyi. Pasien bernama Tn. M, berusia 20 tahun, memiliki riwayat gangguan jiwa sejak 2019 lalu yang mengalami risiko perilaku kekerasan. Tindakan keperawatan yang diberikan untuk mengendalikan risiko perilaku kekerasan sesuai dengan standar asuhan keperawatan yang sudah ditetapkan untuk pasien yaitu dengan tindakan keperawatan generalis selama 11 hari dan terapi musik sebagai teknik relaksasi selama 6 hari. Hasil penerapan terapi musik yang dilakukan menunjukkan adanya penurunan tanda gejala risiko perilaku kekerasan dari skor 19 menjadi skor 2 dan kemampuan mengendalikan risiko perilaku kekerasan yang meningkat dari skor 1 menjadi 11. Terapi musik diharapkan dapat diterapkan sebagai tindakan keperawatan inovasi untuk mengendalikan risiko perilaku kekerasan dalam pemberian asuhan keperawatan di rumah sakit. ......Schizophrenia is a serious and chronic psychiatric disorder that can cause distorted thoughts, perceptions, emotions, movements and behavior. People with schizophrenia can experience increased emotions such as irritability, irritability, and tantrums which can increase the risk of violent behavior. Violent behavior is a situation in which a person's behavior is lost which can be directed at oneself, others, or the environment. The purpose of this scientific work is to provide an overview regarding the application of nursing care to patients at risk of violent behavior through music therapy. The application of music therapy used is active music therapy or singing. The patient named Mr. M, 20 years old, has a history of mental disorders since 2019 which is at risk of violent behavior. Nursing actions given to control the risk of violent behavior are in accordance with nursing care standards that have been set for patients, namely generalist nursing actions for 11 days and music therapy as a relaxation technique for 6 days. The results of the application of music therapy showed that there was a decrease in symptoms of the risk of violent behavior from a score of 19 to a score of 2 and the ability to control the risk of violent behavior increased from a score of 1 to 11. It is hoped that music therapy can be applied as an innovative nursing action to control the risk of violent behavior in giving nursing care in hospital.

Latar belakang: Skizofrenia merupakan sindrom pada perilaku maupun kognitif yang berhubungan dengan gangguan perkembangan otak seseorang. Halusinasi merupakan salah satu gejala positif yang paling umum pada skizofrenia yaitu dialami oleh 60% - 80% penderita skizofrenia. Kasus: Ny. P (33 tahun) diantar oleh keluarga ke rumah sakit karena merasa takut dan mendengar suara bisikan – bisikan tanpa ada wujudnya. Selain itu, menunjukkan sikap curiga, marah – marah, sulit tertidur, dan keluyuran. Diskusi: Asuhan keperawatan dimulai dari pengkajian, analisa data, perencanaan, implementasi, hingga evaluasi. Proses asuhan keperawatan dilakukan pada tanggal 4 April 2023 hingga 18 April 2023 di Ruangan Utari Rumah Sakit Jiwa Dr. Marzoeki Mahdi Bogor. Intervensi yang diberikan kepada Ny. P sesuai dengan standar asuhan keperawatan generalis dan terapi modifikasi yang digunakan adalah kombinasi mendengarkan murottal dengan teknik relaksasi napas dalam sebagai distraksi atau pengalihan pasien pada halusinasinya. Kesimpulan: Penerapan intervensi keperawatan generalis dan terapi modifikasi kombinasi mendengarkan murottal dengan teknik relaksasi napas dalam pada masalah keperawatan halusinasi dapat mengurangi tanda dan gejala halusinasi, dan meningkatkan kemampuan pasien dalam mengontrol halusinasi. ......Background: Schizophrenia is a behavioral and cognitive syndrome associated with the development of a person's brain disorder. Hallucinations are one of the most common positive symptoms in schizophrenia, which is experienced by 60% - 80% of people with schizophrenia. Case: Mrs. P (33 years) was taken by his family to the hospital because he was afraid and heard whispering voices without his form. In addition, showing a suspicious attitude, angry, difficulty falling asleep, and wandering. Discussion: Preparation care starts from assessment, data analysis, planning, implementation, to evaluation. The nursing care process was carried out from 4 to 18 April 2023 in the Utari Room of the Dr. Marzoeki Mahdi Bogor. The intervention given to Mrs. P according to general care service standards and the modified therapy used is a combination of murottal listening with deep breathing relaxation techniques as a distraction for her hallucinations. Conclusion: The application of generalist interventions and modification therapy in combination with murottal listening and breathing relaxation techniques in hallucinatory disorder problems can reduce the signs and symptoms of hallucinations, and improve the patient's ability to control hallucinations.

Isolasi Sosial merupakan salah satu gejala negatif skizofrenia. Isolasi sosial adalah kondisimenyendiri yang dialami seseorang dan perasaan segan terhadap orang lain sebagai sesuatuyang negatif atau keadaan yang mengancam. Masalah sosial seringkali merupakan sumberutama keprihatinan keluarga dan penyedia layanan kesehatan, karena efeknya lebih menonjodaripada gejala kognitif dan persepsi. Tujuan penulisan karya ilmiah ini untukmenggambarkan penerapan terapi generalis, terapi aktivitas kelompok sosialisasi, dan social skill training pada pasien isolasi sosial dengan pendekatan teori hubungan interpersonal Hildegard E. Peplau. Terapi diberikan dan dinalisa pada 35 pasien di ruang Bratasena. Hasilpenerapan terapi generalis, terapi aktivitas kelompok dan social skill training dapatmenurunkan gejala isolasi sosial dan meningkatkan kemampuan bersosialisasi pada pasien.Berdasarkan hasil diatas perlu direkomendasikan integrasi tindakan keperawatan generalisindividu dan kelompok serta terapi spesialis social skill training pada pasien isolasi sosial. ......Social isolation is one of negative symptoms of schizophrenia. Social isolation as a solitaryexperience of a person and shyness toward others as something negative or threateningcircumstances. Social problems are often a major source of concern families and health careproviders, because the effect is more pronounced than cognitive symptoms and perception. The purpose of this final scientific paper was to describe the implementation of generaltherapy, socialization activity group therapy and social skill training toward social isolationclients using interpersonal relationship theory Hildegard E. Peplau rsquo s were conducted to 35selected clients at Bratasena rsquo s ward. Results there were reductions in symptoms of socialisolation and increase in the client 39 s ability to socialize. Based on the result above, integratedtherapies as general therapy, socialization activity group therapy and social skill training onclients with social isolation is recommended.

Latar Belakang: Kekambuhan masih menjadi masalah utama pada pasien skizofrenia sehingga membutuhkan pelayanan kesehatan jiwa yang tepat serta efektif dan efisien. Pelayanan kesehatan jiwa diberikan pada pasien skizofrenia untuk mencegah kekambuhan melalui penentuan diagnosis ketidakefektifan pemeliharaan kesehatan (KPK) menggunakan terapi Acceptance and commitment therapy (ACT) dengan pendekatan Self-transcendence theory (STT). Tujuan: penulisan karya ilmiah akhir spesialis ini sebagai tindakan pencegahan kekambuhan melalui penanganan KPK menggunakan ACT dengan pendekatan STT. Metodologi: Desain penulisan karya ilmiah akhir spesialis ini adalah case series penatalaksanaan KPK menggunakan terapi spesialis ACT dengan pendekatan KPK di ruang Utari RSJ Dr. Marzoeki Mahdi Bogor. Jumlah pasien kelolaan sebanyak 20, instrumen yang digunakan adalah instrument tanda dan gejala KPK, instrument kemampuan KPK yang dikeluarkan oleh departemen keperawatan jiwa FIK UI, instrument Birchwood Insight Scale (BIS) dan instrument kepatuhan minum obat. Hasil: terjadi penurunan tanda dan gejala KPK serta peningkatan kemampuan pasien KPK, peningkatan insight dan peningkatan kepatuhan minum obat melalui setelah pelaksanaan terapi. Kesimpulan: ACT dapat digunakan dengan pendekatan self-transcendence theory untuk pencegahan kekambuhan melalui penanganan masalah KPK. ......Introduction: Relapse is still a major problem in schizophrenic patients, so it requires appropriate, effective and efficient mental health services. Mental health services are provided to schizophrenic patients to prevent relapse through determining the diagnosis of health maintenance care (KPK) using Acceptance and commitment therapy (ACT) with a Self-transcendence theory (STT) approach. Purpose: writing this specialist's final scientific paper as an action to prevent recurrence through the handling of the KPK using ACT with the STT approach. Methods: The design for writing this specialist final scientific paper is a case series on the management of the KPK using ACT specialist therapy with the KPK approach in the Utari room of Dr. RSJ. Marzoeki Mahdi Bogor. The number of patients managed was 20, the instruments used were the KPK signs and symptoms instrument, the KPK capability instrument issued by the psychiatric nursing department of FIK UI, the Birchwood Insight Scale (BIS) instrument and the medication adherence instrument. Results: there was a decrease in signs and symptoms of KPK as well as an increase in the ability of KPK patients, increased insight and increased adherence to taking medication after the implementation of therapy. Conclusion: ACT can be used with a self-transcendence theory approach to prevent recurrence through handling KPK problems.

ABSTRAK
Skizoprenia adalah gangguan psikotik yang kronik, prevalensi skizoprenia di Indonesia 70% dari klien gangguan jiwa berat atau psikotik serta sering disertai dengan gangguan perilaku yaitu perilaku agitasi, agresif, dan perilaku kekerasan. Terapi musik ini merupakan salah satu intervensi terapi lingkungan dalam dimensi kognitif yang dapat diberikan pada klien yang mengalami masalah perilaku kekerasan dengan metode mendengarkan musik, ekplorasi perasaan, diskusi dan umpan balik. Gangguan jiwa yang dirawat di Rumah Sakit Jiwa Daerah Surakarta 93 % klien yang mengalami Skizoprenia dan masalah keperawatan perilaku kekerasan sebanyak 41 % dengan alasan masuk klien dibawa ke rumah sakit karena melakukan perilaku amuk. Penelitian ini berjudul pengaruh terapi musik terhadap perubahan perilaku pada klien Skizoprenia dengan perilaku kekerasan di Rumah Sakit Jiwa Daerah Surakarta. Penelitian ini bertujuan untuk mengetahui pengaruh terapi musik terhadap perubahan perilaku pada klien Skizoprenia dengan perilaku kekerasan. Desain yang digunakan dalam penelitian ini adalah ”Quasi experimental pre post test with control group ” dengan intervensi terapi musik. Cara pengambilan sampel adalah total sampling dengan sampel sebanyak 80 klien dibagi 2 kelompok yaitu 40 klien kelompok yang mendapatkan terapi musik dan 40 klien kelompok yang tidak mendapatkan terapi musik. Pada kelompok yang mendapat terapi musik dilakukan pertemuan sebanyak 4 sesi dalam rentang waktu 4 hari. Perilaku kekerasan yang meliputi respon fisik, respon kognitif, respon perilaku dan respon sosial diukur dengan menggunakan kuesioner , observasi, dan pemeriksaan fisik serta dianalisis menggunakan dependent t-test, independent t-test, chi-square dan regresi liniear ganda. Hasil penelitian ini menunjukan penurunan perilaku kekerasan baik dalam respon fisik, respon kognitif, respon perilaku dan respon sosial secara bermakna, baik pada kelompok yang mendapat terapi musik dan kelompok yang tidak mendapat terapi musik. Penurunan perilaku kekerasan baik dalam respon fisik, respon kognitif, respon perilaku dan respon sosial pada kelompok klien yang mendapatkan terapi musik menurun lebih rendah secara bermakna dibanding dengan kelompok klien yang tidak mendapatkan terapi musik. Terapi musik direkomendasikan sebagai terapi dalam merawat klien dengan perilaku kekerasan.
ABSTRACT
Schizophrenia is a disturbance of chronicles psychotic. Prevalence of schizophrenia in Indonesia are 70% of clients with hard mental illnes or psychotic and it is often accompanied by behavior disturbances including agitate, aggressive, and violent behavior. Music therapy is one of the intervention of environment therapy on cognitive dimension that can be given for client who happened the problems of violent behavior by the methods of hearing music, exploration feelings, discussion and feedback. Client with mental illnes at Mentally Hospital in District of Surakarta are 93% of clients who have schizophrenia and almost 41% with nursing problems of violence behavior. The reasons that they deliver to the hospital because doing fury behavior. This study is given title of the effect of music therapy toward behavior changes on schizophrenia client with violence behavior at Mentally Hospital in District of Surakarta. This study purpose to determine the effect of music therapy toward behavior changes on schizophrenia client with violence behavior. Design of this study is “quasi experimental pre post test with control group “ by the intervention of music therapy. The sample was using total sampling used skizofrenis client with violent behavior with the total 80 clients who divided into 2 groups, 40 clients who received music therapy and 40 clients who did not receive music therapy. The groups who received music therapy get 4 sessions meeting during four days. Violent behavior including the responses of physical, cognitive, behavior and social which were measured by using questionnaires, observation, and physical examination and they analyzed statistically using dependent t-test, independent t-test, chi-square dan double linier regression. Results of this study indicated degradation of changes on violent behavior including the responses of physical, cognitive, behavior and social both of clients who received music therapy and did not receive music therapy.. Changes on violent behavior including the responses of physical, cognitive, behavior and social to client groups who received music therapy decreased lower as means compared to client groups who did not receive music therapy. It is recommended to form and conduct music therapy to care therapy clients with violent behavior.
