"[ABSTRAK
Latar Belakang : Perbedaan antara demam dengue ( DD ) dan demam berdarah
dengue ( DBD ) adalah terjadinya kebocoran plasma pada DBD. Kebocoran
plasma pada ruang interstitial ditandai dengan adanya efusi cairan di pleura dan
peritoneal, hemokonsentrasi, serta hipovolemia intravaskular. Keadaan ini
menyebabkan gangguan perfusi ke jaringan, sehingga menyebabkan metabolism
anaerob. yang menimbulkan peningkatan kadar laktat dalam darah.
Tujuan Penelitian: Mengetahui peran laktat sebagai prediktor prognosis dan
diagnosis kebocoran plasma pada infeksi dengue pasien dewasa.
Metode: Studi potong lintang, pada infeksi virus dengue pasien dewasa yang
dirawat di bangsal penyakit dalam RS Cipto Mangunkusumo dan RS Persahabatan
Jakarta. Jumlah subjek sebanyak 57 orang. Dilakukan pemeriksaan kadar laktat
untuk melihat perbedaan rerata kadar laktat antara DD dan DBD dengan uji t-tes
tidak berpasangan, dan nilai titik potong kadar laktat pada keadaan tanpa atau
dengan kebocoran plasma dilakukan dengan menentukan sensitifitas dan
spesifisitas terbaik dari kurva ROC yang sudah dibuat.
Hasil: Rerata kadar laktat pada DBD secara bermakna lebih tinggi daripada DD.
Nilai titik potong untuk prediktor prognostik pada hari ke-3 yang ditentukan
dengan kurva ROC mendapatkan nilai kadar laktat ≥ 2,65 mmol/ L dengan AUC
0,626 ; IK 95% 0,480-0,772. Dan nilai titik potong untuk diagnostik pada hari ke-
5 mendapatkan nilai kadar laktat ≥ 2,55 mmol/L memberikan sensitivitas 66,6%%
dan spesifisitas 54,2%.
Kesimpulan: Terdapat perbedaan bermakna kadar laktat antara DD dan DBD.
Nilai kadar laktat ≥ 2,65 mmol/L belum dapat digunakan sebagai prediktor
prognostik adanya kebocoran plasma pada fase kritis. Nilai kadar laktat ≥ 2,55
mmol/L pada saat fase kritis dipakai sebagai petanda adanya kebocoran plasma
dengan akurasi yang rendah.

ABSTRACT
Background : The difference between dengue fever (DF) and dengue
hemorrhagic fever (DHF) is plasma leakage which occurs in DHF. The leakage of
plasma into interstitial space is shown by pleura and peritoneal effusion,
hemoconcentration, and intravascular hypovolemia. Anaerob metabolism will
occur due to perfusion dysfunction which will cause increased serum lactate.
Objectives : To determine the role of lactate as a prognostic predictor and
diagnostic in plasma leakage which occurs in adult dengue-infected patients.
Methods : This is cross-sectional study which is conducted in adult dengueinfected
patients hospitalized in internal medicine ward of Cipto Mangunkusumo
Hospital and Persahabatan Hospital in Jakarta. There are 57 adult dengue-infected
patients recruited. Serum lactate is examined to determine the mean difference
between DF and DHF. The data is analyzed by t-test independent and cut-off point
is identified in presence as well as absence of plasma leakage which is to
determine the sensitivity and specificity based on ROC curve.
Results : The mean of serum lactate in DHF is significantly higher compared to
DF. The cut-off point of prognostic predictor in day three of fever which is
determined based on ROC curve shows lactate serum ≥ 2.65 mmol/L with AUC
0.626; 95% CI 0.480-0.772. Moreover, the cut-off point of diagnostic factor in
day five of fever is shown by serum lactate ≥ 2.55 mmol/L with sensitivity 66.6%
and specificity 54.2%.
Conclusion : There is difference of serum lactate in DF and DHF. Serum lactate ≥
2.65 mmol/L could not be used as a prognostic predictor of plasma leakage in
critical phase. Serum lactate ≥ 2.55 mmol/L during critical phase could be used as

Latar Belakang: Penyakit Demam Berdarah Dengue (DBD) masih endemis dan merupakan masalah yang besar dan serius di Indonesia. Peningkatan kadar laktat dalam darah merupakan petanda hipoksia jaringan pada penyakit DBD, bila hipoksia jaringan tidak terdeteksi lebih awal/dini, dan tidak diberikan cairan lebih agresif dan sesuai, maka akan meningkatkan angka komplikasi dan kematian. Tujuan: Mengetahui perbedaan rerata kadar hematokrit dan albumin serum serta beda proporsi efusi pleura dan atau asites, pasien infeksi dengue dewasa pada berbagai derajat hiperlaktatemia untuk mengetahui secara dini adanya hipoksia jaringan Metode: Penelitian ini adalah Studi uji Potong Lintang. Penelitian dilakukan di RSUPN Cipto Mangunkusumo dan RSUP Persahabatan Jakarta, pada pasien yang dirawat periode waktu April 2014 sampai dengan Mei 2015. Menilai beda rerata kadar hematokrit darah dan albumin serum menggunakan uji statistik Uji T, sedangkan beda proporsi efusi pleura dan atau asites dengan Uji Kai Kuadrat. Hasil: Sebanyak 62 pasien infeksi demam dengue, dibagi kedalam 2 kelompok masing-masing 31 pasien berdasarkan kadar laktat darah. Kelompok I dengan kadar laktat darah > 2 sampai ≤ 2,4 mmol/L dan kelompok II > 2,4 mmol/L. Rerata kadar hematokrit darah pada kelompok I dan II masing-masing 40,06 (SB 4,54) dan 41,03 (SB 4,77). Tidak ada perbedaan rerata kadar hematokrit darah pada kedua kelompok dengan nilai p = 0,42. Rerata kadar albumin serum pada kelompok I dan II masing-masing 3,94 (SB 0,29) dan 3,89 (SB 0,30). Tidak ada perbedaan rerata kadar albumin serum pada kedua kelompok dengan nilai p = 0,49. Proporsi efusi pleura dan atau asites pada kelompok I dan II masing-masing 54,8% dan 58,1%. Tidak ada perbedaan proporsi adanya efusi pleura dan atau asites pada kedua kelompok dengan p = 1. Kesimpulan: Tidak ada perbedaan rerata kadar hematokrit darah dan albumin serum, serta beda proporsi efusi pleura dan atau asites pada kelompok kadar laktat darah > 2 sampai ≤ 2,4 mmol/L dibandingkan > 2,4 mmol/L.

Increased levels of lactate in the blood is a marker for tissue hypoxia in DHF , when tissue hypoxia is not detected early and not given the more aggressive fluids and appropriate, it will increase the rate of complications and mortality. Objective: To determine differences in average levels of hematocrit and serum albumin as well as different proportions pleural effusion and/or ascites, adult patients with dengue infection in various degrees hyperlactatemia to know at an early stage for tissue hypoxia. Methods: This study is a test study Cross Sectional. The study was conducted at Cipto Mangunkusumo and at Persahabatan Hospital, Jakarta, in-patients admitted to the time period April 2014 through May 2015. Assessing the mean difference of blood hematocrit levels and serum albumin using statistical test T test, while the different proportion of pleural effusion and/or ascites with test Chi Square. "Tesis ini bertujuan untuk mengetahui perbedaan nilai prediksi VO2max dan kadar laktat pada uji latih submaksimal menggunakan sepeda Astrand saat menggunakan masker bedah dan N95 pada tenaga medis. Metode penelitian adalah desain experimental. Subjek penelitian adalah tenaga medis usia dewasa muda baik laki-laki atau perempuan dengan IMT normoweight sampai obesitas I. Tingkat aktivitas fisik subjek dinilai dengan kuesioner IPAQ sebelum dimulai uji latih. Total subjek penelitian adalah 54 orang. Subjek penelitian melakukan uji latih submaksimal sepeda Astrand dengan menggunakan masker medis dan masker N95 untuk mengetahui nilai prediksi VO2max. Interval uji latih dengan kedua masker adalah minimal 2 hari dan maksimal 1 minggu. Peningkatan kadar laktat diukur melalui selisih nilai laktat yang diperiksa sebelum uji dan sesaat setelah uji latih selesai. Hasil penelitian menunjukkan terdapat nilai prediksi VO2max yang lebih rendah secara signifikan saat uji latih memakai masker 95 dibandingkan masker bedah (p<0.001). Nilai prediksi VO2max yang lebih rendah secara signifikan juga didapatkan pada kelompok jenis kelamin perempuan, laki-laki, kelompok normoweight dan obesitas I dengan nilai p <0.001, 0.09, <0.001, 0.06 secara berurutan. Peningkatan laktat saat uji latih menggunakan masker N95 lebih tinggi secara signifikan dibandingkan dengan saat memakai masker bedah dengan nilai p=0.003. Kesimpulan penelitian terdapat perbedaan bermakna pada nilai prediksi VO2max dan peningkatan laktat saat uji latih submaksimal sepeda astrand dengan menggunakan masker bedah dan N95 pada tenaga medis
......The objection of this study is to evaluate the difference of VO2max predicted value and blood lactate level in submaximal exercise testing with astrand ergocycle using N95 and surgical mask among physician. The method of this study is experimental design. The subjects are male or female young adult physician with BMI within normo-weight to obesity I. Activity level of the subjects was assessed using IPAQ quisioner before the exercising testing. Total of the subject in this study was 54. The subject performed the submaximal exercise testing with astrand ergocycle using surgical and N95 mask to obtain the VO2max prediction value. The interval between the exercise testing was minimum 2 days and maximum 1 week. The increase of lactate level was measure by subtracting the lactate level post exercise testing with pre-exercise testing level. The result of this study shows that the predicted VO2max from submaximal exercise testing with N95 mask is significantly lower than while using surgical mask (p<0.001). According to the gender, the same result also found in female and male groups using N95 and also according to BMI, the same result also found in normo-weight and obesity I group with p value <0.001, 0.09, <0.001, 0.06 respectively. The increase of blood lactate level also higher significantly in N95 mask group with p 0.003. The conclusion of the study is there are significant difference in predicted VO2max and the increase of blood lactate level from the submaximal exercise testing with N95 and surgical mask among physician"

"Tesis ini disusun untuk mengetahui pengaruh latihan interval intensitas tinggi terhadap kadar laktat darah dan tingkat usaha pada dewasa sehat sedentary. Penelitian menggunakan desain uji pre-post. Subjek penelitian merupakan dewasa yang telah dinyatakan sehat dan tergolong sedentary secara aktifitas fisik. Semua subjek melakukan uji latih dengan basis laboratoium menggunakan alat Cardiopulmonary Exercise Testing dan mengikuti program berupa latihan interval intensitas tinggi dengan intensitas 80% berdasarkan heart rate yang diselingi dengan intensitas 40% selama 20 menit yang dilakukan tiga kali dalam seminggu selama empat minggu dengan menggunakan treadmill. Hasil keluaran penelitian ini berupa kadar laktat darah yang diukur dengan pengambilan darah kapiler serta tingkat usaha yang diukur menggunakan Rate of Percieved Exertion dari Skala Borg. Analisis statistik dilakukan untuk membandingkan kadar laktat darah dan tingkat usaha setelah melakukan latihan interval intensitas tinggi pasca latihan pertama dan pasca latihan ke dua belas. Hasil penelitian menyatakan bahwa dengan latihan interval intensitas tinggi, terdapat penurunan kadar laktat darah dan tingkat usaa pada dewasa sehat sedentary. Penurunan kada laktat darah serta tingkat usaha secara berurutan adalah sebesar 1,1 mmol/ L dan 2 pada Skala Borg dimana didapatkan berbeda signifikan dengan nilai p<0,0001. Diperlukan penelitian lebih lanjut untuk menggunakan uji latih berbasis lapangan untuk penentuan peresepan latihan serta menentukan manfaat latihan interval intensitas tinggi pada populasi dengan faktor risiko penyakit kardiorespirasi sebelum dapat digunakan pada populasi sakit

This thesis was aimed to determine the effect of high intensity interval training (HIIT) on blood lactate levels and rate of percieved exertion (RPE) in sedentary healthy adults. The design used was pre-post study. Subjects was adults that was stated healthy and sedentary on physical activity. All subjects underwent laboratory based exercise testing using a Cardiopulmonary Exercise Testing equipment and given HIIT of 80% with a interval of 40% intensity based on heart rate with a total duration of 20 minutes, three times a week for four weeks using treadmill. Capillary blood was obtained to measure blood lactate level and Borg Scale was used to report Rate of Percieved Exertion. After the first and twelveth exercise, statistical analysis was performed to compare blood lactate level and RPE. The result of the study shows a decrease of blood lactate level and RPE after given HIIT in sedentary healthy adults. The reduction of blood lactate level and RPE consecutively was 1,1 mmol/ L and 2 on Borg Scale with a significant difference of p < 0,0001. Further research is needed using a field based exercise testing to determine exercise prescription and to obtain the benefit of HIIT in population with cardiovascular risk factor before utilizing in patients."
