ABSTRACTThis study aimed to examine the dynamic change in bone metabolism by immediate loading in several sites around implants using high-resolution Na18F-PET scan. Two titanium implants ( 1.2 mm) were inserted parallel to each other in the right tibiae of Wistar rats (n = 4). The left tibia was set as control side. One day after insertion, closed coil springs of 4.0 N were attached to the expose superior portions of the implants to apply a continuous mechanical stress. The rats with fluorine-18 (18F) ion (5 mCi/rat) intravenously injected were scanned by PET scanner at 4, 7, 14, 28 days after load application. Round region of interests (ROIs) were set around the distal implant of the right tibia (loaded side) and same site (control) of the left tibia. Furthermore, four rectangular ROIs were set at the superior and inferior parts of traction side (mesial) and opposite side (distal) of the distal implant. Longitudinal dynamic changes in bone metabolism were evaluated by examination of the accumulation count of 18F ion at each ROI. The uptake values of ROIs (loaded side) initially increased until 7 days, and they gradually decreased from the peak level to the pre-loading level despite a static force being applied to the implants. In cancellous bone, the uptake values at the superior part of traction side and inferior part of opposite side showed significantly high value compared with those at other parts. In conclusion, immediate loading to the implant initially enhanced bone metabolism around it, especially at the part with compressive stress. Peri-implant bone metabolism varies according to different loading conditions."

"Pendahuluan: lmplantasi sel punca mesenkimal autologus dalam suspensi asam hyaluronat terbukti dapat mencegah perburukan kerusakan kartilago artikular pada sendi lutut. Prosedur tersebut menjadi pilihan bagi penderita osteoartritis usia muda namun berhubungan dengan serangkaian morbiditas seperti prosedur yang lebih dari satu kali maupun kegagalan dalam kultur untuk mendapatkan sel dengan potensi kondrogenik. Pada penelitian ini, kami menerapkan penggunaan injeksi intra artikular sel punca mesenkimal-jeli Wharton untuk menunda progresivitas dari osteoartritis sendi lutut domba dewasa yang diinduksi dengan menggtmakan menisektomi. Metode: Dua betas domba dengan karakteristik serupa diinduksi osteoartritis dengan menggunakan metode menisektomi meniskus lateral lutut diikuti dengan latihan . Domba tersebut dibagi menjadi 4 kelompok perlakuan dengan jumlah masing-masing 3 ekor. Kelompok perlakuan antara lain kelompok kontrol, injeksi intra artikular dengan asam hyaluronat (hylan G-F 20; Synvisc®), injeksi intra artikular dengan sel punca mesenkimal-jeli Wharton dan injeksi intra artikular dengan sel ptmca mesenkimal-jeli Wharton dalam suspensi asam hyaluronat (hylan G-F 20; Synvisc®). Dilakukan pengambilan foto polos lutut secara anteroposterior sebelum intervensi (1 bulan paska menisektomi) dan setelah intervensi (6 minggu paska penyuntikkan) tmtuk penilaian dengan menggunakan sistem Kellgren-Lawrence. Domba tersebut dikorbankan setelah 4 minggu tmtuk penilaian gross pathology dan histopatologi sesuai dengan sistem Osteoarthritis Research Society International (OARS I). Hasil: Kelompok dengan injeksi intra artikular dengan sel ptmca mesenkimal jeliWharton dalam suspensi asam hyaluronat memiliki basil penilaian gross pathology dan histopatolgi yang lebih bila dibandingkan kelompok kontrol (p<0,05). Tidak ada perbedaan bermakna dari penilaian foto polos dari keempat kelompok. Pembahasan: Penggtmaan injeksi sel punca mesenkimal-jeli Wharton dalam suspensi asam hyaluronat intra artikular merupakan pilihan yang dapat dipertimbangkan dalam tatalaksana osteoartritis. Penilaian efektivitas injeksi untuk regenerasi kartilago artikular dapat dieksplorasi lebih lanjut pada uji klinis.
Introductio11: Autologus mesenchymal stem cell implantation suspended in hyaluronic acid has been proven to delay worsening of articular cartilage degeneration in the knee joint. This has become the treatment of choice for young patient with osteoarthritic knee but associated with morbidities such as multiple procedure, donor site morbidity and failure in achieveing cultured cells with chondrogenic properties. Our study investigates the possibility of direct intra articular injection of Wharton 's jelly-derived mesenchymal stem cells for menisectomy induced osteoarthritis of an adult sheep. Metllods: Twelve sheeps with the same characteristics was osteoarthritical/y induced with lateral meniscectomy and a series of exercise routines. Those sheeps were divided into four groups with 3 sheeps each. The groups are the control group, intra articular hyaluronic acid (hylan G-F 20; Synvisc®) injection group. intra articular Wharton's jelly-derived mesenchymal stem cells group and intraarticular Wharton 's jelly-derived mesenchymal stem cells su!>pended in hyaluronic acid (hylan G-F 20; Synvisc®) as su!>pension. Plain anteroposterior radiography of the knee joint was taken before intervention (1 month after meniscectomy) and after intervention (6 weeks after injection). The plain radiograph was analysed using the Kel/gren-Lawrence system. The sheeps were sacrificed at 4 weeks for gross pathology and histopathological graded according to the Osteoarthritis Research Society International (OARS/) system. Results: The intra articular Wharton 's jelly-derived mesenchymal stem cells suspended in hyaluronic acid group showed better gross pathology and histopathology grade compared with control groups (p<0,05). There was no difference in plain radiography of all groups. Discussions: The use of intra articular injection of Wharton's jelly-derived mesenchymal stem cells suspended in hyaluronic acid is an option in treating knee osteoarthritis. Its efficacy in articular cartilage regeneration would be further explained in clinical trials."
