Abstrak :
Timah hitam (Pb) adalah logam yang berbahaya, akan tetapi juga berguna sebagai bahan tambahan untuk bahan bakar kendaraan bermotor. Bahaya yang ditimbulkan adalah keracunan darah, penurunan tingkat kepandaian anak (IQ) dan gangguan alat reproduksi. Jakarta menduduki peringkat ke-3 negara yang memiliki atmosfer terkotor di dunia, dan penyebab utamanya adalah emisi kendaraan bermotor. Kadar Pb rata-rata diatas standar WHO yaitu melebihi 1,8 ug/m3, sehingga dapat dibayangkan bahwa penduduk Jakarta telah terkontaminasi oleh Pb, . Mengingat wanita hamil adalah segment terpenting dalam suatu peristiwa kelahiran generasi baru. Dampak Pb terhadap kesehatan dan reproduksi patut menjadi peringatan bagi sektor kesehatan masyarakat dan lingkungan. Penelitian ini bersifat Studi Kohort Historis Prospektif Pengaruh Timah Hitam (Pb) terhadap Kesehatan Ibu Hamil 8 bulan di dua lokasi yang berbeda, kemudian ditelusuri sampai kelahiran bayinya dan dipantau berat badan dari bayi yang dilahirkannya. Penelitian ini memfokuskan kepada faktor-faktor dan lingkungan dan individu yang ditelusuri ke masa lampau atau bersifat historis untuk dapat menerangkan lebih lanjut mengenai riwayat keterpaparan Pb ke dalam darah ibu hamil. Selanjutnya pengaruh Pb yang ada di dalam darah ibu hamil dilihat dampak yang muncul melalui jalur perantara yaitu kejadian anemia dan pengaruh langsung terhadap kelahiran BBLR. Data-data dalam penelitian ini adalah primer dan original, yang dikumpulkan dengan mengunakan analisa darah di laboratorium dan kuesioner. Pengambilan data dilaksanakan ketika kehamilan berusia 8 bulan, di dua lokasi yang berbeda dengan asumsi awal ibu hamil yang berasal dari lokasi yang diperkirakan memiliki kualitas udara yang bersih dan ibu hamil yang berasal dari lokasi yang memiliki kualitas udara yang kotor. Sedangkan pemantauan berat badan bayi yang lahir dilakukan secara susul menyusul menurut perkiraan part-as dari setiap ibu hamil. Hasil dari penelitian ini menunjukkan bahwa terdapat keterkaitan riwayat status pekerjaan, lama kerja serta jenis alat transportasi yang biasa digunakan ibu hamil dengan keberadaan Pb didalam darah ibu hamil. Di temukan hubungan antara anemia dengan kelahiran bayi BBLR, sedangkan hubungan antara kadar Pb darah ibu hamil dengan kejadian BBLR itu sendiri ternyata tidak dapat dibuktikan secara statistik.

Lead is a harmful chemical and also useful metal for gasoline addition in transportation sector. The harmful things as air pollution is able to cause systemic toxicity in blood, IQ descent and reproduction disorder. Jakarta is the 3rd grade culmination of unclean atmosphere in the world. One of the causes of air pollution is lead emission. The average lead pollution in Jakarta atmosphere is over 1,8 .µg/m3 whereas the standard of World Health Organization, and so people in Jakarta was already contaminated by lead in their blood. Remember of pregnant women is the crucial segment of birth of future have to give special warning in Public Health and Environmental sector specially to the government that making generation on all the long centuries. Impact of lead in their health and pregnancy adjustment of gasoline regulation in Indonesia. There are a lot of studies of lead and impact of it that have been done in many countries in the worldwide. This research is a new study of lead impact on reproduction particularly in women reproduction that causes the low birth weight infant. In this research, we've done a community base approach to kinetic agent or the way of some factors that contributed in contaminating process of lead as pollutant through human reproduction with a longitudinal historical-prospective cohort study. 71 pregnant women who had a blood analysis, we searching their historical background related with Pb contact and we follow trough to the future until they delivery their babies. We have founded in this research correlation between Pb in blood with occupational status, working period; kinds of daily transportation and a association between anemia and Low Birth Weight (LBW) infant, but we can not found association between Pb level in blood with anemia nor LBW.

Abstrak :
Low birth weight (LBW) and preterm birth are the most cause of infant death. After going home there is a problem of preterm infant care due to inadequate and inability mother to anticipate the emergency condition that threat the baby. The purpose of this study is to identify the influence of 'RINDU' health education package to mother readiness for nurturing preterm baby at home. The design of the study is quasi-experimental with pretest-posttest control group. The samples of this study was 50 mothers of preterm infant treated in Aura Syifa, Melinda, Muhammadiyah, and Gambiran district hospital at Kediri. The samples was devided in two groups, 25 participants of intervention group and 25 participants of control group. Descriptive statistic were gotten the data that almost 56% mother were 25th years, 88% had the under junior high school, 52% participants had income more than Rp 450.000,00 and 76% had no experience to preterm infant care. There are significant differences in the readiness of the mother in intervention and control group for caring preterm infant at home (p=0,000; α=0,05). Readiness of mother take care of preterm infant is not influenced by age, education, incomes and experience ( p>0,05). 'RINDU' health education package effectively used to improve knowledge, attitude, and skill of mother to care the preterm babies at home. Nursing services in hospital should be using 'RINDU' a health education package as an independent nursing intervention programe for preterm infant’s mother.
