"Developmental Defects of Enamel (DDE) pose a significant dental problem for children and adolescents. Affected teeth are discolored, often sensitive and prone to fracture or wear easily, increasing the risk of dental caries. Furthermore, they are often very difficult to restore because conventional dental materials do not bond effectively to the defective enamel. Clinically, affected individuals have heightened levels of dental anxiety making management challenging and stressful for the child, parent and clinician alike. This book discusses the epidemiology and etiology of DDE, including defects of both primary and permanent teeth, associated syndromes, Molar Incisor Hypomineralization and more generalized genetic defects. The structure and mechanical properties of affected enamel are discussed particularly in relation to clinical behavior. The signs and symptoms that allow accurate diagnosis are outlined and management protocols described. The clinical chapters are well illustrated, providing clear, evidence based guidelines where appropriate. Importantly the impact of living with DDE is explored from the young person?s perspective. These insights challenge the dental profession to seek improved understanding of the condition and to develop new management strategies. Avenues for future research are identified that may help the profession rise to this challenge and to better meet the needs and expectations of future generations of children and adolescents."

"Peran Persepsi Ketidakpastian, Identitas Ego, Persepsi Kendali Perilaku Pasien dalam Menyikapi Operasi Medis. Operasi medis kadang merupakan pilihan terbaik dalam rangka kesejahteraan hidup pasien, namun tidak semua pasien mau menjalaninya. Kegagalan terapeutik seringkali disebabkan oleh tidak kooperatifnya pasien yang berpangkal dari sikap negatif pasien terhadap operasi medis. Penelitian ini bertujuan untuk menemukan model teoretis yang menjelaskan faktor-faktor psikologis dalam diri pasien yang berperan dalam membentuk sikap tersebut. Penelitian korelasional prediktif ini dilakukan terhadap 99 pasien berpenyakit jantung dan kontinum kanker yang membutuhkan operasi di DKI Jakarta. Penelitian ini menunjukkan bahwa aspek komitmen dari identitas ego mampu meramalkan sikap terhadap operasi medis secara tidak langsung melalui mediasi persepsi ketidakpastian. Persepsi kendali perilaku meramalkan sikap tersebut secara langsung dalam arah negatif. Penelitian ini menyimpulkan bahwa komitmen terhadap identitas berperan signifikan dalam diri pasien ketika menyikapi operasi medis.

This predictive correlational research was conducted on 99 patients suffering heart disease and cancer continuum who require medical surgery in DKI Jakarta, Indonesia. Research results showed that a commitment aspect of ego identity is able to indirectly predict attitude toward medical surgery through mediation of perceived uncertainty. Perceived behavioral control directly predicts the attitude in a negative direction. This research concludes that patients? commitment towards their identity plays a significant role as they deal with medical surgery."

"Lembaga pendidikan keperawatan Poltekes Jurusan Keperawatan Palembang bertujuan menghasilkan perawat profesiona/pemula melalui berbagai kegiatan belajar mengajar yang dilaksanakan di kelas, laboratorium keperawatan dan lahan praktek sesuai dengan tuntutan kompetensi yang diharapkan.
Salah satu kegiatan belajar mengajar yang dilakukan untuk mencapai tujuan tersebut adalah kegiatan belajar mengajar keperawatan medikal bedah (217A). Namun kegiatan belajar mengajar medikal bedah yang dilakukan belum memperoleh basil yang sesuai dengan tuntutan kompetensinya. Kondisi tersebut ditandai oleh rendahnya tingkat penguasaan ilmu pengetahuan dan keterampilan keperawatan yang dimiliki oleh mahasiswa.
Penelitian ini bertujuan untuk menggali berbagai informasi mengenai kegiatan pembelajaran keperawatan medikal bedah (217A) di Poltekes Jurusan Keperawatan Palembang berdasarkan pendekatan sistem. Rancangan penelitian menggunakan metode kualitatif, sedangkan pengumpulan informasi dilakukan dengan teknik wawancara mendalam, diskusi kelompok terarah dan dokumentasi. Kemudian analisis dilanjutkan melalui triangulasi metode dan sumber, sedangkan hasilnya diteliti melalui analisis isi.
Hasil penelitian menunjukkan bahwa kegiatan belajar mengajar masih berkualitas rendah. Hal ini disebabkan oleh rendahnya kualitas dosen, terbatasnya fasilitas dan sumber belajar yang tersedia. Selain itu, perencanaan, pengorganisasian, pelaksanaan serta pengawasan kegiatan belajar mengajar yang dilaksanakan belum optimal.

Analysis on Educational System of Medical Surgery Nursing at Palembang Polytechnics of Health Majoring in Nursing Department 2002Palembang Polytechnics of Health in Nursing Department aims at educating beginning professional nurse through lecturing and educational activities conducted in classes, nursing laboratories, and practical places based on hoped competence demand.
One of the lecturing and educational activities to achieve that goal is medical surgery lecture (217A). On the other hand that lecture activity has not fulfilled the competence demands. That condition is marked by students' low masteries of nursing knowledge and skills.
This research objective is to obtain some information concerning lecturing activities of medical surgery (217A) in Palembang Polytechnics of Health based on systemic approach. This research design uses qualitative method, while its information is collected through in-depth interview, focus group discussion and documentation. The analysis is continued through method and source triangulation, while the result is studied based on content analysis.
The research result shows that lecturing quality is low due to lowly qualified lecturer, limited facility and lecturing resources. Moreover, lecture planning, organizing and controlling have not been optimal yet.

"Bedah jantung merupakan tindakan yang berisiko tinggi menyebabkan kecacatan atau bahkan mortalitas jika dibandingkan dengan disiplin bedah lainnya, terlebih pada kasus jantung bawaan yang memiliki kompleksitas tinggi. Untuk itu, informed consent berperan penting sebagai pondasi tindakan medis yang memberikan pelindungan hukum bagi pasien anak dan dokter, khususnya dokter jantung anak dan dokter bedah jantung anak. Dengan metode penelitian doktrinal menggunakan pendekatan analitis, penelitian ini menganalisis mengenai kedudukan hukum informed consent serta pelindungan hukum yang diberikan pada tindakan bedah jantung anak dengan metode Open Heart Surgery. Dapat disimpulkan bahwa pada pasien anak, pemberian informed consent wajib didampingi oleh orang tuanya dan pengambilan keputusan pada suatu tindakan medis harus diputuskan secara bersama antara anak dengan orang tua melalui komunikasi yang efektif. Hal ini sebagai wujud penghargaan evolving capacities of the child dan hak anak untuk berpartisipasi. Selain itu, informed consent dapat menjadi alasan penghapus pidana dan alat bukti sah di pengadilan, sehingga dokter tidak dapat dituntut ataupun digugat selama terbukti tidak ada kelalaian dan menjalankan tindakan sesuai informed consent. Oleh sebab itu, Pemerintah Indonesia sebaiknya menyempurnakan Permenkes 290/2008 dengan menambahkan ketentuan yang spesifik mengatur consenting age bagi anak. Informed consent sebagai kerangka acuan tindakan medis harus dipatuhi oleh dokter maupun pasien. Adapun, penyebab pasti PJB harus diteliti lebih lanjut guna menerapkan upaya preventif, serta diperlukan pengadaan rumah sakit khusus jantung secara merata di Indonesia.

Heart surgery is a high-risk procedure that can result in disability or even mortality compared to other surgical disciplines, specifically for congenital heart disease that have such high complexity. Therefore, informed consent plays a crucial role as the foundation of medical action, providing legal protection for both pediatric patienst and doctors, namely pediatric cardiologist and pediatric cardiac surgeons. Using doctrinal research methods with an analytical approach, this study analyzes the legal status of informed consent and the legal protection provided for pediatric heart surgery using the Open Heart Surgery method. It can be concluded that for pediatric patients, the granting of informed consent must be accompanied by the parents, and decisions regarding medical procedures must be made jointly between the child and the parents through effective communication. This reflects respect for the evolving capacities of the child and the children’s rights of participation. Additionally, informed consent can serve as a justification for exoneration from criminal liability and as valid evidence in court, meaning that doctors cannot be prosecuted or sued as long as there is no negligence and the procedure is carried out in accordance with the informed consent. Therefore, the Government of Indonesia should refine Regulation of the Minister of Health No. 290/2008 by incorporating specific provisions regarding the consenting age for children. Informed consent, as a reference framework for medical procedures, must be adhered to by both doctors and patients. Furthermore, the exact causes of congenital heart defects (CHD) should be further investigated to implement preventive measures, and the establishment of specialized cardiac hospitals should be evenly distributed across Indonesia."
