This book highlights the rapidly developing field of advanced optical methods for structural and functional brain imaging. As is known, the brain is the most poorly understood organ of a living body. It is indeed the most complex structure in the known universe and, thus, mapping of the brain has become one of the most exciting frontlines of contemporary research. Malaria merupakan salah satu masalah kesehatan masyarakat yang paling parah di dunia, berdasarkan data WHO tahun 2020 malaria menyebakan 627.000 kematian. Ditingkat regional Indonesia menjadi negara peringkat kedua dengan kasus malaria terbanyak di Asia tenggara (regional WHO). Berdasarkan data kementerian kesehatan total kasus malaria di Indonesia mencapai 428.517 kasus, dimana kasus terbanyak berada di wilayah Indonesia timur. Indonesia menargetkan eliminasi malaria pada tahun 2030. Salah satu upaya yang dilakukan untuk mencapai target tersebut adalah dengan menggalakan diagnosis malaria dengan menggunakan metode microscopy dan metode RDT sesuai rekomendasi WHO. Diagnosis dini dan pengobatan malaria yang tepat sasaran akan berpotensi mengurangi penyakit, mencegah kematian dan mengurangi penularan penyakit. Jika dilihat dari sisi efektifitasnya metode diagnosis malaria dengan microscopy dan RDT masih terdapat perbedaan beberapa penelitian menyatakan RDT lebih efektif dan pada beberapa penelitian juga ada yang menyatakan Microscopy lebih efektif. Namun demikian terdapat penelitain yang menyatakan akibat buruk atas kesalahan diagnosis dini malaria diantaranya adalah diagnosis yang tidak sesuai dapat mengakibatkan penggunaan obat anti malaria yang berlebihan, sehingga akan berakibat pada potensi penyebaran malaria yang resisten terhadap obat dan selain itu juga akan menimbulkan kerugian ekonomi. Tujuan penelitian ini adalah untuk menganalisis efektivitas biaya diagnostic parasit malaria dengan Metode Diagnostic Rapid Diagnostic Test (RDT) dan Microscopy pada sampel malaria di BBTKLPP Jakarta Tahun 2022. Perspektif penelitian ini menggunakan perspektif penyedia yakni BBTKLPP Jakarta. Analisis biaya dengan manggunakan metode Activity Based Costing (ABC) dengan kompoenen biaya yang dihitung meliputi biaya langsung dan tidak langsung, outcome yang dianalisis adalah intermediate outcome, yaitu sensitivity dan specificity dengan gold standard pemeriksaan menggunakan PCR. Efektivitas biaya diperoleh dari perbandingan antara biaya pengujian dan perolehan outcome metode diagnostic RDT dengan Microscopy. Model keputusan yang digunakan pada penelitian ini adalah decision tree. Jumlah sampel pada setiap metode diagnosis adalah 110 pasien suspek malaria. Hasil penelitian mendapati biaya yang dibutuhkan untuk pengujian sampel malaria menggunakan RDT lebih besar dibandingkan dengan menggunakan metode microscopy yaitu RDT sebesar Rp.466.452,00, dan Microscopy sebesar Rp.413.666,00. Perolehan outcome didapati RDT lebih rendah dibandingkan microscopy, dimana RDT memperoleh nilai Sensitivity sebesar 62,50%, Specificity sebesar 97,67%, dan akurasi pengujian sebesar 90,00%. Sedangkan microscopy memperoleh nilai Sensitivity sebesar 100,00%, Specificity sebesar 97,70%, dan akurasi pengujian sebesar 98,18%. Sehingga dapat disimpulkan bahwa Microscopy mendominasi metode RDT dalam mendeteksi parasite malaria di BBTKLPP Jakarta Tahun 2022. ......Malaria is one of the most serious public health problems in the world, based on WHO data in 2020 malaria caused 627,000 deaths. At the regional level, Indonesia is the second-ranked country with the most malaria cases in Southeast Asia (WHO regional). Based on ministry data health total cases of malaria in Indonesia reached 428,517 cases, where the most cases are in the eastern of Indonesia. Indonesia is targeting malaria elimination by 2030. One of the efforts made to achieve this target is with promoting the diagnosis of malaria with using microscopy method and RDT method according to WHO recommendations. Early diagnosis and appropriate treatment of malaria will potentially reduce disease, prevent death and reduce transmission disease. Viewed from the effectiveness of the method of diagnosing malaria with microscopy and RDT, there are still differences, some studies say that RDT is more effective and in some studies say that microscopy is more effective. However, there are studies which state the negative consequences of an early misdiagnosis of malaria, include that an inappropriate diagnosis can result the excessive using of anti-malarial drugs, which will result the spreading potential of drug-resistant malaria and besides that it will also cause economic losses. The purpose of this study was to analyze the cost-effectiveness of diagnosing malaria parasites using the Diagnostic Rapid Diagnostic Test (RDT) Method and Microscopy on malaria samples at BBTKLPP Jakarta in 2022. The perspective of this study uses the perspective of a provider, namely BBTKLPP Jakarta. Cost analysis uses the Activity Based Costing (ABC) method with calculated cost components including direct and indirect costs, the outcome being analyzed is the intermediate outcome, namely sensitivity and specificity with gold standard inspection using PCR. Cost effectiveness is obtained from comparison between the cost of the test and the outcome of the RDT diagnostic method with microscopy. The decision model used in this study is the decision tree. The number of samples for each method of diagnosis was 110 patients with suspected malaria. The results of the study found that the costs required for testing malaria samples using RDT were greater than using the microscopy method, RDT Rp.466.452.00, and Microscopy Rp.413.666.00. The outcome obtained was that RDT was lower than microscopy, where RDT obtained a Sensitivity value of 62.50%, Specificity of 97.67%, and test accuracy of 90.00%. While microscopy obtained a Sensitivity value of 100.00%, Specificity of 97.70%, and testing accuracy of 98.18%. In conclusion that microscopy dominates the RDT method in detecting malaria parasites at BBTKLPP Jakarta in 2022.
