Abstrak :
Kualitas asuhan keperawatan yang diberikan perawat sangat dipengaruhi seberapa besar pemberian layanan yang diterima pasien. Upaya yang dilakukan untuk meningkatkan kualitas asuhan keperawatan dengan menerapkan berbagai peran perawat spesialis menerapkan Evidence Based Nursing EBN serta peran sebagai inovator. Peran pemberi asuhan keperawatan dilakukan pada pasien dengan stroke iskemik dan 30 pasien dengan gangguan sistem persarafan menggunakan Model adaptasi Roy MAR . Penerapan EBN yang dilakukan pada orang pasien stroke dan menunjukkan bahwa latihan Active Assisstive range of Motion mampu mengatasi masalah mobilitas fisik. Program inovasi penambahan format pengkajian neurologi mampu meningkatkan pengetahuan perawat mengenai format pengkajian selain dari format pengkajian yang telah ada di ruangan. Diharapkan mobilisasi dini dengan latihan active assisstive range of motion tetap diajarkan pasien stroke yang mengalami masalah mobilitas fisik dikarenakan perlu waktu yang cukup lama untuk mengatasi masalah mobilitas. ...... Advanced clinical practice in the neurological system is intended to be able to provide nursing care, apply Evidence Based Nursing EBN as well as the role of an innovator. Nursing care roles were performed in patients with Stroke Ischemic and 30 patients with impaired neural system using the Roy adaptation model RAM . The behavioral mode of physiological adaptation most often experiences maladaptive behavior. The emerging nursing diagnosis is the risk of perfusion of cerebral tissue perfusion. The nursing management intervention of cerebral edema is intended to improve patient adaptation in enhancing cerebral tissue perfusion. Application of EBN Active Assisstive Range of Motion of Mobility performed on 3 stroke patients and showed that. The innovation program for the addition of the neurological assessment format was able to increase the nurse 39 s knowledge of the assessment format apart from the existing assessment formats in the room. Assessment of behaviors and stimuli in RAM need to be applied to patient assessment.

Abstrak :
Diabetes melitus tipe 2 merupakan penyakit kronis yang dapat menimbulkan berbagai komplikasi. Komplikasi diabetes melitus yang berdampak terhadap kondisi fisiologis maupun psikologis memerlukan proses adaptasi. Tujuan laporan ini yaitu untuk menganalisis penerapan teori adaptasi Roy dalam proses asuhan keperawatan pada pasien dengan diabetes melitus tipe 2 disertai dengan berbagai komplikasi melalui metode studi kasus dan studi literatur. Hasil menunjukkan masalah utama adalah kerusakan integritas kulit dan resiko ketidakstabilan kadar glukosa darah serta kurangnya pengetahuan terkait manajemen diabetes melitus, dimana intervensi utama dalam laporan ini yaitu untuk meningkatkan proses adaptasi klien dengan diabetes melitus tipe 2 yaitu meningkat pemahaman terkait manajemen diabetes melitus secara mandiri. Kesimpulan: konsep adaptasi Roy dapat diterapkan dalam asuhan keperawatan pada pasien diabetes melitus tipe 2 dengan komplikasi ulkus diabetik dan gagal ginjal kronik. ......Diabetes mellitus type 2 is a chronic disease that can cause various complications. Complications of diabetes mellitus that affect the physiological and psychological conditions require adaptation process. The purpose of this report is to analyze the application of Roy 39;s adaptation theory in the process of nursing care in patients with type 2 diabetes mellitus with various complications through case study method and literature study. The results indicate the major problem are the deterioration of skin integrity, the risk of instability of blood glucose levels and the lack of knowledge related to diabetes mellitus management, where the main intervention in this report is to improve the adaptation process of clients with diabetes mellitus type 2, by increasing understanding of diabetes mellitus management independently. Penurunan level oksigen didalam darah hipoksemia telah dilaporkan berkontribusi terhadap tingginya angka kematian pada anak yang disebabkan oleh infeksi saluran pernapasan akut. Tujuan dalam penulisan karya ilmiah ini adalah memberikan gambaran mengenai pemenuhan kebutuhan oksigenasi anak melalui penerapan pengkajian dini tingkat keparahan gangguan sistem pernapasan pada anak dengan pendekatan Model Adaptasi Roy. Terdapat lima kasus infeksi saluran pernapasan akut pada anak dengan usia 0 ndash; 2 tahun yang diberikan asuhan keperawatan menggunakan Model Adaptasi Roy. Aplikasi Model Adaptasi Roy dapat membantu memberikan asuhan keperawatan secara komprehensif pada anak dan membantu dalam memahami bahwa setiap anak itu unik dan memiliki respon perilaku yang berbeda terhadap sebuah stimulus berdasarkan level adaptasinya. Instrumen Modified Tal rsquo;s Score memiliki validitas dan reliabilitas, serta sensitivitas, dan spesifisitas yang baik dalam menilai keparahan gangguan sistem pernapasan pada anak. Instrumen ini dapat digunakan dalam pengkajian perilaku pada mode adaptasi fisiologis oksigenasi sehingga dapat memudahkan perawat dalam memahami kondisi anak.

ABSTRACT
Low levels of oxygen in the blood hypoxemia has been reported being responsible for high mortality in children caused by acute respiratory infections. The purpose of this study is to describe the fulfillment of the oxygenation needs in children through the application of early assessment tools of the respiratory status severity using the Roy Adaptation Model. There are five cases of acute respiratory infections in children aged 0 - 2 years who are given nursing care using the Roy Adaptation Model. The results of this study describe that application of Roy Adaptation Model can provide a comprehensive nursing care in children and helps in understanding that each child is unique and has different behavioral responses to a stimulus based on their adaptation level. Modified Tal 39;s Score instruments have a great validity and reliability, as well as sensitivity, and specificity in assessing the severity of respiratory status in children. This instrument can be used to assess the behavioral assessment especially in physiologic-physical mode: oxygenation and can facilitate the nurse understanding the condition of the child.

Anak dengan kanker disebabkan oleh proses penyakit kanker itu sendiri dan efek samping dari pemberian terapi. Keluhan seperti mual muntah, diare, demam, dan mielosupresi berkontribusi terhadap kejadian dehidrasi pada anak kanker. Hidrasi adalah pemenuhan air sesuai kebutuhan. Karya ilmiah akhir ini bertujuan untuk memberikan gambaran aplikasi Model Adaptasi Roy (MAR) dalam memberikan asuhan keperawatan pada anak kanker yang berisiko mengalami ketidakseimbangan cairan. Metode karya ilmiah ini adalah studi kasus yang terdiri atas lima kasus anak kanker yang mendapatkan kemoterapi yang diberikan asuhan keperawatan dengan pendekatan MAR. Intervensi keperawatan berdasarkan evidence based practice adalah memberikan edukasi tentang hidrasi pada pengasuh anak kanker. Edukasi kepada pengasuh menggunakan multimedia video dan booklet terbukti efektif meningkatkan pengetahuan dan sikap positif dalam mendampingi anak kanker selama menjalani kemoterapi. ......Children with cancer can experience fluid imbalance disorders caused by cancer itself and the side effects of therapy. Complaints such as nausea, vomiting, diarrhea, fever, and myelosuppression contribute to the incidence of dehydration in children with cancer. Hydration is the fulfillment of water as needed. This final scientific paper aims to provide an overview of the Roy Adaptation Model (MAR) application in providing nursing care to cancer children who are at risk of fluid imbalance. The method of this scientific work is a case study consisting of five cases of cancer children who received chemotherapy and given nursing care with the MAR approach. Nursing intervention based-evidenced practice is to provide education about hydration to cancer caregivers. Education for caregivers using multimedia videos and booklets has proven to be effective in increasing knowledge and positive attitudes in assisting children with cancer during chemotherapy.

Praktik Spesialis Keperawatan Anak bertujuan untuk melakukan praktik dengan mengaplikasikan peran perawat melalui pendekatan Model Adaptasi Roy. Peran sebagai pemberi asuhan keperawatan diterapkan pada 5 orang klien anak dengan masalah perkemihan yang mengalami ketidakseimbangan cairan, dimana satu orang klien sebagai kelolaan utama yaitu klien sindroma nefrotik relaps dan hipoalbuminemia yang mengalami edema anasarka. Peran sebagai inovator melalui penyusunan program pemberian edukasi manajemen cairan pada keluarga dan perawat yang bertujuan untuk mengurangi kelebihan volume cairan tubuh, mengurangi udema, meningkatkan diuresis, meningkatkan kenyamanan klien anak dengan masalah perkemihan, dan meningkatkan kualitas asuhan keperawatan dengan menerapkan tindakan keperawatan yang berbasis pembuktian ilmiah (evidence based nursing). Hasil praktik ini menunjukkan bahwa Model Adaptasi Roy efektif digunakan pada klien anak dengan masalah perkemihan yang mengalami ketidakseimbangan cairan, dan edukasi efektif untuk mengurangi risiko masalah yang dapat timbul akibat kelebihan volume cairan tubuh.

ABSTRACT
Practice of Pediatric Nursing Specialist aims to practice the application of the role of nurses through the Roy Adaptation Model approach. Role as nurse provider applied to 5 children clients with urinary problems that undergo fluid imbalance, and one major client is a client of relapse nephritic syndrome and hypoalbuminemia who experienced edema within whole body. Role as an innovator by designing fluid management program of education providing for families and nurses which aims to reduce excess body fluid volume, reduce edema, increase diuresis, improve the comfort of children client with urinary problems, and improve the quality of nursing care by implementing nursing actions based on scientific evidence (evidence based nursing). Roy Adaptation Model is proving that education to families and nurses is one of fluid management techniques in children with urinary problems. The result of this practice indicates that this model is effective to be used on the children client with urinary problems with imbalance of fluid and effective education to reduce the risk of problems due to excess body fluid volume.;

Soiling dan inkontinensia feses adalah masalah yang sering terjadi pada anak pascaoperasi Pullthrough yang menyebabkan terjadinya perineal dermatitis dan gangguan integritas kulit. Karya ilmiah akhir ini bertujuan untuk menggambarkan efektifitas penggunaan VCO untuk mengoptimalkan integritas kulit pada anak post pullthrough. Sebanyak 5 anak pascaoperasi Pullthrough yang mengalami risiko kerusakan integritas kulit diberikan intervensi berupa perawatan perineal dan pemberian VCO. Empat dari lima pasien menunjukkan respon adaptif. Rekomendasi hasil karya ilmiah ini adalah mengaplikasikan model adaptasi Roy dan penggunaan VCO dalam proses keperawatan pada anak dengan masalah integritas kulit. ...... Soiling and fecal incontinence is a common problem among children with postoperative Pullthrough that cause perineal dermatitis and impaired skin integrity. This final scientific research aimed to describe the effectivity of VCO to optimize skin integrity among children with post pullthrough. Five children with postoperative Pullthrough were managed as Clients were at risk for impaired skin integrity and were cared with VCO application. Four out of five child showed an adaptive response. This research recomends the application of Roy adaptation model and VCO application into nursing process in caring children with skin integrity problem.

Program spesialis keperawatan medikal bedah khususnya neurologi dimaksudkan untuk menjadikan seorang perawat spesialis neurosains yang berperan dalam praktik keperawatan berupa pemberi asuhan keperawatan lanjut, melakukan pembuktian ilmiah dan agen pembaharu. Asuhan keperawatan dilakukan pada kasus pasien dengan cedera kepala dan 31 pasien dengan gangguan neurologis dengan pendekatan Model adaptasi Roy (RAM). Perilaku maladaptif paling banyak terganggu pada mode fisologis dengan diagnosis penurunan kapasitas adaptif intrakranial. Evidence Based Nursing dilakukan dengan menerapkan sistematik oral care pada 12 pasien gangguan neurologis dengan hasil yang signifikan (p value 0,004). Program inovasi menerapkan pemberdayaan caregiver pasien stroke dengan hasil perbedaan pengetahuan dan keterampilan caregiver melalui edukasi dan demonstrasi (p value <0.005). Pengkajian perilaku dan stimulus pada RAM perlu diterapkan pada pengkajian pada pasien gangguan neurologis. Intervensi keperawatan diharapkan berbasis pada evidence based nursing. Pemberdayaan caregiver pada pasien gangguan neurologis khususnya stroke perlu dilakukan sejak dini untuk menyiapkan caregiver dalam merawat pasien gangguan neurologis.

ABSTRACT
Nurse specialist program of medical surgical nursing especially neurological nurses proposed neuroscience nurse specialist play role in nursing practice as advanced nursing care providers, conduct scientific evidence and innovator. Roy adaptation model approach was used in the nursing care of the head injury patients and 31 patients of neurological disorders cases. Decreased intracranial adaptive capacity was the most often of nursing diagnosis enforced which was caused maladaptive behavior in physiological mode. Evidence based nursing was implemented by practice systematic oral care in 12 neurological disorders patients that showed oral assessment score differed significantly (p value 0,004). Role as an innovator was implemented by engage caregiver empowerment through education and demonstration in stroke patients that showed there was differences in the knowledge and skills of the caregivers (p value <0.005). Roy adaptation model can be one effective approach for the assessment of the neurological disorders patients. Nursing interventions are expected to always be based on evidence based nursing. Early caregiver empowerment in neurological disorders patients especially stroke is required to set up the caregivers in caring of the patients.

Keperawatan menurut Mc Mahon merupakan terapi yang melibatkan tiga 3 komponen yaitu partnership, intimate dan reciprocity disetiap tindakan keperawatan. Begitu juga yang harus dilakukan oleh perawat ners spesialis atau perawat lanjutan. Pelaksanaan praktik residensi keperawatan khususnya neurologi dilakukan sebagai bagian dari proses untuk mencapai pendidikan perawat ners spesialis. Perawat ners spesialis dalam memberikan asuhan keperawatan harus memiliki tujuh kompetensi yaitu memberikan pelayanan langsung, konsultasi, kepemimpinan, kolaborasi, pendidikan, penelitian dan pengambil keputusan yang mengedepankan etik. Rangkaian pendidikan spesialis keperawatan ini adalah mengelola kasus utama, menyusun kasus resume yang dilaporkan sebanyak 30 pasien dengan gangguan neurologi, melakukan Eviden based Nursing EBN latihan Menelan dengan madu pada pasien disfagia serta melakukan inovasi membuat video edukasi. Kasus terbanyak selama praktik adalah stroke dan diagnosis keperawatan terbanyak yaitu resiko ketidakefektifan perfusi jaringan serebral, gangguan mobilisasi dan gangguan pemenuhan kebutuhan personal hygine.. Nursing in Mc Mahon say that nursing is a therapy that involves three 3 components of partnership, intimate and reciprocity in every action of nursing. So also must be done by a specialist nurse nurse or advanced nurse. Implementation of nursing residency practices, especially neurology is done as part of the process to achieve specialist nurse education. The role of nurse specialist in providing nursing care should have seven competencies that is to provide direct services, consultation, leadership, collaboration, education, research and decision makers that put ethics forward. This series of nursing specialist education is managing the main case, compiling 30 reported cases of patients with neurological disorders, performing an Evidence based Nursing EBN and innovating with patients with neurological disorders. The most cases during the practice were stroke and the most common nursing diagnoses were the risk of perfusion of cerebral tissue, impaired mobilization and impaired fulfillment of personal hygine needs. EBN is a swallowing exercise with honey and the implementation of group innovation to make educational videos to increase self awareness and family against the risk of recurrent stroke. Spesialis keperawatan medikal bedah peminatan onkologi memiliki tanggung jawab sebagai pemberi asuhan keperawatan, peneliti, pendidik, advokat, dan inovator juga menjadi tuntutan penting untuk dijalankan. Peran pemberi asuhan keperawatan dilakukan pada pasien Kanker Nasofaring dengan faktor penyulit TB Paru dan Infeksi dengan menggunakan Model Adaptasi Roy. Disamping itu terdapat 30 pasien lainnya yang juga mendapat penerapan dari model keperawatan ini. Penerapan EBN yang dilakukan adalah akupresur P6 dan edukasi perawat untuk menagtasi mual muntah pasien kemoterapi. Nursing professions to be able to provide quality nursing services, nurses are required capable of integrating science and art in performing nursing services and perform the role of professional nurses. Specialized medical nursing specialist oncology specialists have responsibilities as nursing caregivers, researchers, educators, advocates, and innovators are also important demands to run. The role of nurse care caregivers is done in patients with Nasopharyngeal Cancer factors of Pulmonary TB and Infection by using the Roy Adaptation Model. In addition there are 30 other patients who also received the application of this nursing model. Implementation of EBN conducted is acupressure P6 and nurse education to mensing the nausea vomiting chemotherapy patients.
