Penelitian ini membahas tentang desain Failure Mode And Effect Analysis FMEA sebagai upaya pencegahan pasien jatuh di Instalasi Rawat Inap RSAB Harapan Kita Tahun 2017. Tujuan dari penelitian ini adalah diperolehnya desain FMEA sebagai upaya pencegahan pasien jatuh di Instalasi Rawat Inap pasien RSAB Harapan Kita pada tahun 2017. Jenis penelitian ini adalah kualitatif dengan menggunakan operational research dengan metode observasi, wawancara mendalam, telaah data sekunder dan telaah dokumen. Hasil penelitian ini adalah faktor rumah sakit ketersediaan sarana prasarana dan faktor pasien/ keluarga pengetahuan pasien/ keluarga ikut berkontribusi sebagai penyebab pasien jatuh di RSAB Harapan Kita. Desain FMEA yang evidence based dan sesuai dengan kebutuhan RSAB Harapan Kita diperoleh melalui proses Focus Group Discussion FGD Tim FMEA sehingga diperoleh prioritas perbaikan dan menghasilkan upaya-upaya pencegahan pasien jatuh di Instalasi Rawat Inap RSAB Harapan Kita.

ABSTRACT
This study discusses the design of Failure Mode And Effect Analysis FMEA as prevention of inpatient falls at Harapan Kita Women and Children Hospital in 2017. The purpose of this research is to obtain the design of FMEA as prevention efforts of inpatient falls at Harapan Kita Women and Children Hospital in 2017. This type of research is qualitative by using operational research with the method of observation, in depth interview, secondary data study and document review. The results of this study are hospital factors availability of infrastructure and patient family factors patient family knowledge contributed as the cause of the patient falls at Harapan Kita Women and Children Hospital. The evidence based design of FMEA and in accordance with the needs of Harapan Kita Women and Children Hospital is obtained through the Focus Group Discussion FGD process of FMEA Team to obtain the priority of improvement and resulted prevention efforts of the inpatient falls at Harapan Kita Women and Children Hospital.

Pasien jatuh merupakan jatuhnya pasien secara tidak sengaja ke lantai/tanah/tempat yang lebih rendah dari sebelumnya dengan atau tanpa cedera pada pasien. Pasien jatuh termasuk ke dalam tiga besar insiden keselamatan pasien di rumah sakit yang menempati posisi kedua setelah medical error. Kejadian pasien jatuh di rumah sakit tidak hanya terjadi di instalasi rawat inap namun juga rawat jalan. Rumah sakit harus memiliki prosedur pencegahan pasien jatuh di rawat inap maupun rawat jalan. Salah satu cara pencegahan yaitu dengan mengenali risiko apa saja yang mungkin terjadi menggunakan desain Failure Mode and Effect Analysis (FMEA). Penelitian ini membahas proses pembuatan desain Failure Mode and Effect Analysis (FMEA) untuk mencegah pasien jatuh di instalasi rawat jalan (Poli Fisioterapi) di RS Hermina Tangerang pada tahun 2019. Tujuan penelitian ini yaitu diperolehnya desain FMEA sebagai upaya pencegahan pasien jatuh di Instalasi Rawat Jalan (Poli Fisioterapi). Penelitian ini menggunakan jenis penelitian kualitatif dengan menggunakan operational research. Metode penelitian yang digunakan yaitu dengan wawancara mendalam, observasi, telaah dokumen dan telaah data sekunder. Hasil penelitian ini yaitu ditemukannya faktor risiko yang dapat menyebabkan kejadian jatuh dari faktor rumah sakit dan faktor pasien/keluarga. Desain FMEA yang dibuat berdasarkan hasil penelitian yang kemudian dilakukan skoring dengan pihak terkait. untuk mengetahui nilai Severity, Occurrence, Detection dan RPN. Diperolehnya prioritas perbaikan dan rekomendasi perbaikan yang dapat dilakukan di Instalasi Rawat Jalan (Poli Fisioterapi). ......Falling patients are patients who accidentally go to the floor/ground/place that is lower than before with or without injury to the patient. Fall patients included in the top three reported patient safety in hospitals that had second place after medical errors. The incidence of patients falling in hospitals does not only occur in inpatient care but also in outpatient care. Hospitals must have procedures to wait for mature patients to be hospitalized as well as outpatients. One way to overcome this is to issue any problems that might occur using the design of the Failure Mode and Effect Analysis (FMEA). This study discusses the process of making a design of the Failure Mode and Effect Analysis (FMEA) design to prevent patients from falling on an outpatient care (Physiotherapy Units) at Hermina Tangerang Hospital in 2019. The purpose of this study is to obtain the FMEA design as an effort to prevent patients from falling in the Outpatient Installation (Physiotherapy Units). This study uses a type of qualitative research using operational research. The research method used is in-depth interviews, observations, document review and review of secondary data. The results of this study are the discovery of risk factors that can cause the incidence of hospital factors and patient/family factors. The FMEA design is made based on the results of research which is then scored with the relevant parties to find out the value of Severity, Occurrence, Detection and RPN. The priority of repairs and repairs that can be done at the Outpatient Installation (Poly Physiotherapy).

Jatuh merupakan kondisi yang tidak diharapkan. Jatuh dapat memberikan dampak yang tidak diinginkan bagi pasien maupun rumah sakit. Kepatuhan perawat menjadi penting dalam melaksanakan Standar Prosedur Operasional (SPO) pencegahan pasien jatuh. Penulisan ini bertujuan agar teridentifikasi tingkat kepatuhan perawat dalam melaksanakan pencegahan pasien jatuh di ruang rawat inap. Jumlah responden dalam penulisan ini adalah 28 perawat. Pengambilan responden dengan teknik purposive sampling. Analisis dilakukan dengan kuesioner dari penelitian sebelumnya dan disesuaikan kembali dengan SPO instansi terkait. Data dikumpulkan dalam kuesioner tersebut yang terdiri dari 27 pernyataan. Hasil penulisan menunjukkan bahwa kepatuhan perawat dalam pencegahan pasien jatuh di ruang rawat inap sebagian besar dikategorikan patuh dalam rentang 3,21–3,68. Hasil penulisan ini merekomendasikan perawat selalu dipertahankan kepatuhannya terhadap SPO pencegahan pasien jatuh dan selalu memberikan edukasi kepada pasien dan penunggu pasien sehingga dapat mengaplikasikan peranannya dalam upaya pencegahan pasien jatuh. Dalam melakukan pencegahan pasien jatuh diperlukan fungsi pengawasan dan kontroling secara ketat oleh kepala ruangan sebagai langkah awal mendisiplinkan perawat untuk melaksanakan pencegahan pasien jatuh. Selain itu juga diharapkan adanya program resosialisasi, pelatihan, dan motivasi secara berkala oleh kepala ruangan. Dengan demikian dapat meningkatkan kepercayaan masyarakat terhadap pelayanan kesehatan yang diberikan. ......Falling conditions are not expected. Falling can have an unwanted impact on patients and hospitals. Nurse compliance is important in implementing Standard Operating Procedures (SOP) for preventing patient falls. This writing aims to identify the level of compliance of nurses in implementing patient fall prevention in the inpatient room. The number of respondents in this writing is 28 nurses. Taking respondents with purposive sampling technique. The analysis was carried out using a questionnaire from previous studies and readjusted to the SPO of the relevant agency. Data was collected in the questionnaire which consisted of 27 statements. The results showed that the adherence of nurses in preventing patient falls in the inpatient room was mostly adherent in the range of 3.21–3.68. The results of this paper recommend that nurses always maintain adherence to the SPO for preventing patient falls and always provide education to patients and patient caretakers so that they can implement their functions in preventing patient falls. In preventing patient falls, strict supervision and control functions are needed by the head of the room as the first step in disciplining nurses to carry out patient fall prevention. In addition, it is also hoped that there will be periodic socialization, training and motivation programs by the head of the room.
