"Pelayanan kefarmasian di puskesmas bertujuan untuk meningkatkan mutu kehidupan pasien dengan tolok ukur standar yang telah ditetapkan. Standar pelayanan kefarmasian meliputi pelayanan farmasi klinik dan pengelolaan sediaan farmasi dan bahan medis habis pakai. Petunjuk Teknis Standar Pelayanan Kefarmasian di Puskesmas membantu tenaga kefarmasian dalam melaksanakan pelayanan yang sesuai standar dan mendukung fungsi pokok puskesmas. Penelitian ini bertujuan untuk menyusun rancangan panduan pelayanan farmasi klinis di Puskesmas Kecamatan Pasar Rebo, dengan fokus pada standar pelayanan kefarmasian yang berlaku. Metode penelitian yang digunakan adalah deskriptif-kualitatif dengan pengumpulan data melalui wawancara dan observasi langsung terhadap kegiatan farmasi klinis di Puskesmas Kecamatan Pasar Rebo. Hasil penelitian ini adalah penyusunan panduan pelayanan farmasi klinis yang sesuai dengan Petunjuk Teknis Standar Pelayanan Kefarmasian di Puskesmas yang diterbitkan oleh Kemenkes, serta peraturan yang relevan. Perlu dilakukan penerapan standar pelayanan farmasi klinis secara konsisten guna meminimalkan risiko kesalahan dalam pelayanan farmasi klinis dan meningkatkan mutu pelayanan kefarmasian secara keseluruhan.

Pharmaceutical services in community health centers also called as Puskesmas aim to enhance patient quality of life by adhering to established standards. These standards encompass clinical pharmacy services and the management of pharmaceutical preparations and disposable medical supplies. The Technical Guidelines for Pharmaceutical Service Standards in Puskesmas guide pharmacists in delivering standardized services and supporting the core functions of puskesmas. This study aimed to develop a clinical pharmacy service guideline for Pasar Rebo District Puskesmas, focusing on applicable pharmaceutical service standards. A descriptive-qualitative research method was employed, involving data collection through interviews and direct observation of clinical pharmacy activities at Pasar Rebo District Puskesmas. The research outcome was the creation of a clinical pharmacy service guideline aligned with the Technical Guidelines for Pharmaceutical Service Standards in Puskesmas issued by the Ministry of Health and relevant regulations. Consistent implementation of clinical pharmacy service standards is necessary to minimize medication error risks and improve overall pharmaceutical service quality.

ABSTRAK
Dalam memberikan pelayanan kefarmasian, apoteker dituntut untuk memiliki kemampuan manajerial dan memberikan pelayanan farmasi klinik. Kegiatan Praktek Kerja Profesi Apoteker (PKPA) di apotek bertujuan untuk memberikan pemahaman mengenai peran, tugas, fungsi dan tanggung jawab apoteker baik secara teknis maupun non teknis. Pelaksanaan PKPA dilakukan di apotek atrika selama empat minggu dengan pembagian tugas pelayanan kefarmasian dan manajerial persediaan farmasi. Pelayanan kefarmasian yang dilakukan meliputi pelayanan obat resep dan non-resep, pemberian informasi obat, konseling dan pelayanan cek kesehatan. Sedangkan tugas manajerial yang dilakukan meliputi pengadaan obat melalui pemesanan ke distributor resmi, penyimpanan obat dengan sistem FEFO (First Expired First Out), pemusnahan resep narkotik, psikotropik dan non-narkotik, pencatatan stok obat dengan pencatatan faktur dan kartu stok, pelaporan narkotika dan psikotropika, serta penanganan pengembalian obat yang mendekati kadaluarsa kepada distributor. Kegiatan PKPA di apotek atrika memberikan kesempatan kepada mahasiswa untuk belajar mengelola persediaan obat di apotek, melakukan pelayanan farmasi klinis, melakukan dokumentasi obat yang baik dan belajar untuk melayani pasien.

The internship activities in apotek atrika give students the opportunity to learn managing drug supply in pharmacy, conduct clinical pharmacy service, perform good medicine documentation and learn to serve patients.

"Standar pelayanan kefarmasian adalah tolak ukur yang dipergunakan sebagai pedoman bagi tenaga kefarmasian dalam menyelenggarakan pelayanan kefarmasian. Standar pelayanan kefarmasian di puskesmas meliputi manajemen farmasi dan pelayanan farmasi klinik. Apoteker di puskesmas mempunyai tugas dan tanggung jawab untuk menjamin terlaksananya pengelolaan sediaan farmasi dan Bahan Medis Habis Pakai (BMHP) dan memberikan pelayanan farmasi klinik kepada pasien dengan efektif dan efisien, tepat sasaran, dan memprioritaskan pasien. Pelayanan resep merupakan salah satu kegiatan pelayanan farmasi klinik berupa rangkaian kegiatan mulai dari peracikan obat, penyerahan obat hingga pemberian informasi obat kepada pasien. Pelayanan resep di Puskesmas Kecamatan Ciracas telah menerapkan sistem resep terintegrasi dengan komputer sejak tahun 2022. Penerapan sistem tersebut masih tergolong baru, maka dari itu perlu dilakukan sosialisasi kepada pasien atau pengujung mengenai alur pelayanan resep elektronik di Puskesmas Kecamatan Ciracas. Penyusunan laporan khusus ini bertujuan untuk memberikan panduan berupa video mengenai alur pelayanan resep elektronik di Puskesmas Kecamatan Ciracas sebagai salah satu upaya untuk meningkatkan efektivitas dan efisiensi pelayanan resep. Panduan berupa video mengenai alur pelayanan resep elektronik di Puskesmas Kecamatan Ciracas ditayangkan menggunakan media elektronik dekat ruang farmasi di Puskesmas Kecamatan Ciracas. Video tersebut diharapkan dapat membantu bagi pasien atau pengujung Puskesmas Kecamatan Ciracas untuk menjadi lebih tertib dan memahami mengenai langkah-langkah pelayanan resep elektronik.

Apothecary at puskesmas have the duties and responsibilities of ensuring the management of pharmaceutical preparations and consumable medical materials and providing clinical pharmacy services to patients in an effective and efficient manner, on target, and prioritizing patients. Prescription service is one of the clinical pharmacy service activities in the form of a series of activities starting from dispensing drugs, dispensing drugs to providing drug information to patients. Prescription services at the Ciracas District Health Center have implemented a prescription system integrated with computers since 2022. The application of this system is still relatively new, therefore it is necessary to socialize to patients or visitors regarding the flow of electronic prescription services at the Ciracas District Health Center. The purpose of preparing this special report is to provide guidance in the form of a video regarding the flow of electronic prescription services at the Ciracas District Health Center as an effort to increase the effectiveness and efficiency of prescription services. A guide in the form of a video regarding the flow of electronic prescription services at the Ciracas District Health Center is displayed using electronic media near the pharmacy room at the Ciracas District Health Center. It is hoped that this video will help patients or visitors to the Ciracas District Health Center to become more orderly and understand the steps for electronic prescription services."

"Konseling pasien pulang yang dijalankan dengan fungsi pelayanan apoteker yang dijalankan di RSCM dapat menyebabkan perbedaan pengetahuan obat dan kepuasan pasien. Tujuan penelitian ini adalah untuk membandingkan pengetahuan obat dan kepuasan pasien setelah diberikan konseling pulang oleh apoteker dengan fungsi pelayanan yang berbeda. Metode penelitian yang dilakukan studi potong lintang (crosssectional study) menggunakan kuesioner yang telah diuji validitas dan reliabilitasnya. Subyek penelitian adalah pasien pulang rawat inap gedung A bagian penyakit dalam RSCM periode Februari – Maret 2020 sebanyak 60 responden. Data yang didapatkan dianalisis secara univariat dan bivariat menggunakan SPSS. Hasil penelitian menunjukkan tidak ada perbedaan pengetahuan obat dan kepuasan pasien secara signifikan pada statistik (p= 0,605, OR = 1,495; p= 1,000, OR = 2,071). Kesimpulan penelitian ini adalah tidak ada perbedaan pengetahuan obat dan kepuasan pasien setelah diberikan konseling oleh apoteker pelayanan yang berbeda.

Discharge counseling carried out by different functions of the pharmacist services performed at RSCM could impact patient’s knowledge and satisfaction. This research aims are to compare patient’s knowledge and satisfaction after being given discharge counseling by both pharmacists with different service functions. The research method used was a cross-sectional study using a questionnaire that had been tested for validity and reliability. The research subjects were inpatient hospitalized that would be discharged in building A in the internal medicine section of the RSCM from February to March 2020 with total 60 respondents. The data obtained were analyzed by univariate and bivariate using SPSS. The results showed the differences of patient’s knowledge and pastient’s satisfaction is not statistically significant (p = 0,605 OR = 1,495; p = 1,000 OR = 2,071). The conclusion of this research is there was no differences in patient’s knowledge and satisfaction after being given discharge counseling by different pharmacist services."

"Pelayanan kefarmasian merupakan proses pelayanan dengan tanggung jawab langsung kepada pasien yang berkaitan dengan sediaan farmasi dengan tujuan meningkatkan mutu kehidupan pasien. Pelayanan kefarmasian di Indonesia terbagi menjadi tiga ranah, yakni pelayanan kefarmasian di rumah sakit, apotek, dan puskesmas. Salah satu cakupan pelayanan farmasi klinik di apotek merupakan pengkajian resep. Kesalahan penulisan resep dapat menimbulkan masalah pengobatan (medication error) pada pasien, sehingga perlu dilakukan pengkajian resep. Tugas khusus ini disusun menggunakan metode kualitatif dengan menggunakan studi literatur, observasi, dan pengkajian secara langsung terhadap resep dari Tn. H dan Ny. J di Apotek Kimia Farma 005 Cikini. Studi literatur dilakukan untuk melihat aspek telaah administrasi, farmasetika, dan klinis yang disesuaikan dengan Permenkes No. 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Berdasarkan pengkajian resep yang telah dilakukan, pengkajian resep terhadap resep pasien Tn. H dan Ny. J belum memenuhi aspek administratif tetapi memenuhi aspek farmasetika dan aspek klinis. Terdapat potensi interaksi dari penggunaan obat pasien Tn. H sehingga diperlukan pemantauan lebih lanjut.

Pharmaceutical service is a service process with direct responsibility to patients related to pharmaceutical preparations with the aim of improving the quality of life of patients. Pharmaceutical services in Indonesia are divided into three domains, namely pharmaceutical services in hospitals, pharmacies and health centers. One of the scope of clinical pharmacy services in pharmacies is prescription review. Error prescription writing can cause medication errors in patients, so it is necessary to review the prescription. This assignment was prepared using a qualitative method using literature studies, observation, and direct assessment of the recipe from Mr. H and Mrs. J at Apotek Kimia Farma 005 Cikini. A literature study was conducted to look at aspects of administration, pharmaceuticals, and clinical studies that were adjusted to Permenkes No. 73 of 2016 concerning Pharmaceutical Service Standards at Pharmacies. Based on the prescription review that has been carried out, the prescription review of the patient's prescription, Mr. H and Mrs. J has not fulfilled the administrative aspect but fulfills the pharmaceutical and clinical aspects. There is a potential for interactions with the patient's drug use. H so that further monitoring is needed."

Analisa risk assessment dilakukan sebagai tugas khusus untuk dapat menganalisa kejadian atau hal-hal berbahaya apa saja yang dapat atau telah terjadi di dalam pelayanan kefarmasian sehingga dapat dicarikan solusi baik secara preventif maupun koreksi agar dapat meningkatkan kualitas pelayanan terhadap pasien.

"Peran apoteker dalam menjalankan pelayanan farmasi klinik di apotek salah satunya adalah pelayanan dan pengkajian resep. Salah satu terapi penyakit yang yang dilayani resep obatnya di Apotek Kimia Farma 202 adalah terapi penyakit hiperplasia prostat jinak. Pengkajian resep yang dilakukan meliputi kajian administratif, kesesuaian farmasetika, dan pertimbangan klinis. Adapun obat yang digunakan untuk terapi hiperplasia prostat jinak yang ditemukan adalah kombinasi dutasteride dan tamsulosin, monoterapi tamsulosin, dan monoterapi silodosin. Berdasarkan empat resep yang dikaji, seluruh resep tidak memenuhi kajian administratif sehingga dilakukan konfirmasi kembali ke pasien dan dokter penulis resep. Seluruh obat dari resep 1 sampai 4 memenuhi kesesuaian farmasetika dan pada pertimbangan klinis dilakukan pemberian informasi obat mengenai efek samping dan penggunaan pada saat penyerahan obat.

One of the roles of pharmacists in clinical pharmacy at pharmacy is drug prescription service and review. One of the disease therapies served by drug prescriptions at Apotek Kimia Farma 202 is the therapy of benign prostatic hyperplasia. The prescription review carried out includes administrative studies, pharmaceutical suitability, and clinical considerations. The drugs used for the therapy of benign prostatic hyperplasia found are a combination of dutasteride and tamsulosin, tamsulosin monotherapy, and silodosin monotherapy. Based on the four prescriptions reviewed, all prescriptions did not meet the administrative review so that confirmation was made back to the patient and the prescribing doctor. All drugs from prescriptions 1 to 4 have met pharmaceutical suitability and for clinical considerations have provided drug information about side effects and usage of the drugs when pharmacist delivered the drugs to the patients."

"Apotek merupakan suatu sarana pelayanan kefarmasian tempat dilakukannya praktek kefarmasian oleh Apoteker. Pelayanan kefarmasian di Apotek terbagi kedalam dua jenis yaitu pengelolaan Sediaan Farmasi, Alat Kesehatan, dan Bahan Medis Habis Pakai serta pelayanan farmasi klinis. Mengingat banyaknya ketidaksesuaian antara resep yang diberikan oleh dokter kepada pasien, maka tugas khusus ini dilakukan untuk melakukan observasi terkait pelayanan farmasi klinis di Apotek Biak khususnya terkait ketepatan pelayanan resep serta kesesuaian data dan kelengkapan resep yang diterima di Apotek Roxy sesuai dengan ketentuan Standar Pelayanan Kefarmasian yang berlaku. Hal ini diharapkan dapat mencegah terjadinya kesalahan pemberian obat (medication error). Pembuatan laporan tugas khusus Praktik Kerja Profesi Apoteker (PKPA) di Apotek Roxy Biak diawali dengan mengumpulkan beberapa data berupa sampel resep bulan Februari 2023 dan dilakukan analisis resep berdasarkan telaah aspek administratif, farmasetik, dan klinis. Berdasarkan hasil kegiatan yang telah dilakukan, pelayanan farmasi klinis yang dilakukan di Apotek Roxy termasuk pelayanan dan pengkajian resep serta dispensing obat yang dilakukan sudah tepat dan sesuai dengan Standar Pelayanan Kefarmasian di Apotek, serta Resep obat yang diterima dan dilayani di Apotek Roxy biak sudah sesuai dengan kelengkapan data yang tepat berdasarkan pengkajian administrative, farmasetika, dan klinis.
