"Apotek sebagai tempat melakukan praktek kefarmasian yang dilakukan oleh tenaga kefarmaian. Pelayanan kefarmasian yang dilakuakan di apotek salah satunya adalah pelayanan farmasi klinik yang meliputi pengkajian resep. Salah satu kajian resep yang dilakukan adalah resep untuk obat penyakit lambung. Menurut data dari Departemen Kesehatan RI angka presentase dari kejadian gastritis di Indonesia adalah 40,8% salah satu faktornya adalah tingkat kesadaran masyarakat tentang kesahatan lambung masih sangat rendah. Namun, penyakit lambung tidak dapat dianggap ringan sehingga perlu dilakukan Pengkajian Resep dan Pembuatan Media Edukasi Terkait Penyakit Lambung Untuk Pasien di Apotek Roxy Condet. Kegiatan pengkajian resep dilakukan dengan skrining resep dan membandingkan hasil skrining resep dengan dengan ketentuan obat pada lieteratur serta membuat media edukasi terkait penyakit lambung. Resep yang dilayani sebagian besar telah sesuai kajian administrasi dan farmasetik namun masih terdapat beberapa kajian administrasi, farmasetik, dan pertimbangan klinis yang belum lengkap sehingga konfirmasi kepada dokter pada resep yang kurang lengkap secara administrasi dan farmasetik dianggap perlu untuk meningkatkan jaminan terhadap kesehatan pasien. Pembuatan informasi (leaflet) mengenai penyakit dan obat-obatan serta terapi penunjang dapat dilakukan secara rutin untuk meningkatkan edukasi pasien terhadap kesehatan.

A pharmacy is where pharmaceutical staff carry out pharmaceutical practice. One of the pharmaceutical services provided in pharmacies is clinical pharmacy services, which include reviewing prescriptions. One of the prescription studies carried out was a prescription for medication for gastric diseases. According to data from the Indonesian Ministry of Health, the percentage of gastritis incidents in Indonesia is 40.8%, one of the factors being that the level of public awareness about stomach health is still very low. However, gastric disease cannot be considered mild, so it is necessary to review recipes and create educational media related to gastric disease for patients at the Roxy Condet Pharmacy. However, several administrative, pharmaceutical, and clinical considerations still need to be completed, so confirmation of incomplete prescriptions to the doctor is necessary to increase patient health guarantees. Information (leaflets) regarding diseases and medicines and supporting therapy can be created regularly to increase patient education regarding health."

The conceptualization of relevance is important because it lies at the basis of efforts to make the social sciences more relevant to conditions in the Third World. Nevertheless, the calls for greater relevance have generally been made in vague terms owing to the less than systematic manner in which "irrelevance" was discussed. "edication error (interaksi obat dan duplikasi terapi), ketidakpatuhan dan efek samping obat sehingga diperlukan pengkajian resep, baik adminitrasi, farmasetik, dan klinis untuk meminimalkan risiko medication error akibat pemberian obat polifarmasi. Laporan ini bertujuan untuk melakukan pengkajian resep yang meliputi kajian administrasi, kajian farmasetik, dan kajian klinis terhadap resep polifarmasi yang masuk ke Apotek Roxy Depok. Metode yang digunakan adalah studi literatur. Hasil yang diperoleh menunjukkan, bahwa aspek administrasi berupa data berat badan dan nomor telepon dokter perlu dilengkapi pada kedua resep dan dari aspek farmasetik perlu ditanyakan kembali kepada dokter yang meresepkan terkait bentuk sediaan (resep 1) dan data stabilitas obat (resep 1 dan 2). Dari aspek klinis pada resep 1 perlu dilakukan penambahan aturan pakai medixon dan alopurinol sementara resep 2 terdapat potensi interaksi obat.

The use of drugs in large quantities can lead to an increased risk of medication errors (drug interactions and duplication of therapy), non-compliance and side effects of drugs, so it is necessary to study prescriptions, both administrative, pharmaceutical, and clinical to minimize the risk of medication errors due to the administration of polypharmacy drugs. This report aims to conduct a prescription review which includes administrative review, pharmaceutical review, and clinical review of polypharmacy prescriptions that enter the Roxy Depok Pharmacy. The method used is literature study. The results obtained show that the administrative aspects in the form of weight data and the doctor's telephone number need to be completed on both prescriptions and from the pharmaceutical aspect it is necessary to ask the prescribing doctor regarding the dosage form (prescription 1) and drug stability data (recipes 1 and 2). From a clinical aspect, prescription 1 needs to be added to the rules for using medixon and allopurinol, while prescription 2 has the potential for drug interactions."

"Pengkajian resep adalah bagian dari pelayanan farmasi klinik yang dilakukan oleh Apoteker untuk menganalisa adanya masalah terkait obat dan menghindari terjadinya medication error terutama pada tahap peresepan (presribing error.  Penelitian ini bertujuan untuk mengkaji resep pasien dengan diagnosis hiperlipidemia di Apotek Kimia Farma 049 dan mengetahui adanya medication error. Hiperlipidemia merupakan suatu keadaan dimana terjadi peningkatan kadar kolesterol dengan atau tanpa peningkatan kadar trigliserida dalam darah. Penelitian ini menggunakan pendekatan deskriptif dengan metode analisis data resep pasien dengan diagnosis hiperlipidemia yang diterima di apotek selama periode penelitian. Fokus utama dari kajian ini adalah untuk menilai kepatuhan resep dan jenis obat yang diresepkan. Hasil pengkajian resep yang telah dilakukan secara administratif masih terdapat beberapa aspek yang kurang lengkap, seperti; Surat Izin Praktik (SIP) dokter, alamat dokter, nomor telepon dokter, paraf dokter, umur pasien, jenis kelamin pasien, dan berat badan pasien. Berdasarkan pertimbangan klinis secara umum dapat dikatakan tidak ada permasalahan yang terlalu signifikan tetapi hanya perlu monitoring mengenai efek samping pada masing-masing obat, serta konfirmasi kepada dokter mengenai interaksi beberapa obat yang bersifat moderate.

Prescription review is part of clinical pharmacy services carried out by pharmacists to analyze drug-related problems and avoid medication errors, especially at the prescribing stage (prescribing errors).  This study aims to examine prescriptions for patients diagnosed with hyperlipidemia at Kimia Farma 049 Pharmacy and determine whether there are medication errors. Hyperlipidemia is a condition where there is an increase in cholesterol levels with or without an increase in triglyceride levels in the blood. This study used a descriptive approach with a data analysis method for prescriptions from patients diagnosed with hyperlipidemia received at the pharmacy during the research period. The main focus of this study is to assess the fulfillment of prescriptions and the type of medication promised. The results of the review of prescriptions that have been carried out administratively still contain several aspects that are incomplete, such as; Doctor's Practice License (SIP), doctor's address, doctor's telephone number, doctor's initials, patient's age, patient's gender, and patient's weight. Based on clinical considerations in general, it can be said that there are no problems that are too significant, but it is only necessary to monitor the side effects of each drug, as well as confirm with the doctor regarding moderate drug interactions."

This thesis is conducted with the background that there is still low result of the dismantling of narcotics and narcotic prescursors syndicate network, in which the competence and 1he training of interdiction personnel are part of the problems. This research is a study using a quantitative approach to measure actual competency gaps and ideal investigator to identity the needs fur improving and narrow the gap throught training effort. By mapping the competencies, we can identify the necessity of improvement of the competency gaps through training,and for doing the right training to the competency gaps, we need Training Need Analysis.

The analysis of actual and ideal competency gaps in1he study,used ibe T test analysis 1n0ls, and to identify training needs, researchers used a model of training needs analysis Mc Cann.
