"Praktik Ners spesialis keperawatan medikal bedah dengan kekhususan pada sistem endokrin dilaksanakan selama kurang lebih satu tahun yang bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan keperawatan, pendidik, peneliti dan inovator. Peran sebagai pemberi asuhan keperawatan dilakukan dengan mengelola 30 pasien dengan gangguan pada sistem endokrin dan 1 pasien kelolaan utama dengan diabetes melitus Tipe 2 yang memiliki komplikasi selulitis, sepsis, AKI, hipoalbumin, hiperkoagulitas, anemia, hipertensi dan dislipidemia menggunakan pendekatan teori model adaptasi Calista Roy. Diabetes melitus (DM) merupakan penyakit kronik yang membutuhkan perawatan seumur hidup, sehingga keterlibatan pasien secara aktif dalam manajemen DM sangat diperlukan. Keyakinan (self efikasi) pasien terhadap manajemen DM mempengaruhi outcome (HbA1c) pasien. Peran sebagai peneliti dan pendidik dilakukan dengan menerapkan asuhan keperawatan berbasis bukti (evidence based practice) screening self efikasi dan edukasi pasien DM dengan Thai Diabetes Management Self Efficacy Scale (TDMSES). Peran sebagai inovator dilakukan dengan mengembangkan proyek inovasi Hypoglycemia Nursing Early Warning Score System) (Hy-newss) untuk screening resiko kejadian hipoglikemia berat pada pasien DM tipe 2.

The practice of medical surgical nurses specializing in the endocrine system was carried out for approximately one year with the aim of implementing the role of nurses as care givers, educators, and innovators. The role as a care giver is given by managing 30

patients with disorders in endocrine system and 1 main patient with type 2 diabetes mellitus with complications of cellulitis, sepsis, AKI, hypoalbumin, hypercoagulability, anemia, hypertension and dyslipidemia using the Calista Roy adaptation theory approach. Diabetes mellitus (DM) is a chronic disease that requires a lifetime management, so active involvement of patient in management of DM is very important. Confidence (self- efficacy) of patients on DM management would affects patient outcomes (HbA1c). Roles as researchers and educators are carried out by implementing evidence-based practice in nursing by screening self-efficacy and educating patients with the Thai Diabetes Management Self Efficacy Scale (TDMSES). The role as innovator is carried out by developing the Hypoglycemia Nursing Early Warning Score System (Hy-news) as innovation project to screen the risk of severe hypoglycemia in type 2 DM patients."
