"Pasien sepsis yang mendapatkan terapi obat mempunyai risiko mengalami masalah terkait obat. Kompleksitas penyakit dan penggunaan obat, serta respons pasien yang sangat individual meningkatkan munculnya masalah terkait obat. Hal tersebut menyebabkan perlunya dilakukan Pemantauan Terapi Obat (PTO) dalam praktik profesi untuk mengoptimalkan efek terapi dan meminimalkan efek yang tidak dikehendaki. Penelitian ini bertujuan untuk mengetahui Pemantauan Terapi Obat (PTO) pada pasien dengan penyakit sepsis di ruang rawat inap teratai RSUP Fatmawati dan mengidentifikasi masalah terkait obat menggunakan PCNE V9.0. Metode yang digunakan berdasarkan temuan/kasus permasalahan terkait obat diklasifikasikan berdasarkan PCNE V9.0 Kemudian direkomendasikan penyelesaian permasalahan berdasarkan literatur. Hasil menunjukan penatalaksanaan sepsis sudah dilakukan sesuai dengan panduan tatalaksana sepsis (Kemenkes) dan identifikasi Drug Related Problem (DRP) menggunakan PCNE V9.0 didapatkan tiga masalah yaitu C1. Pemilihan obat; C2. Bentuk obat; C3 Pemilihan dosis.
Sepsis patients who receive drug therapy are at risk of experiencing drug-related problems. The complexity of the disease and the use of drugs, as well as the highly individualized patient responses increase the emergence of drug-related problems. This causes the need for Drug Therapy Monitoring (PTO) in professional practice to optimize therapeutic effects and minimize unwanted effects. This study aims to determine Drug Therapy Monitoring (PTO) in patients with sepsis in the lotus inpatient ward of Fatmawati Hospital and identify drug-related problems using PCNE V9.0. The method used is based on the findings/cases of drug-related problems classified according to PCNE V9.0 Then it is recommended to solve the problem based on the literature. The results showed that the management of sepsis had been carried out in accordance with the sepsis management guidelines (Ministry of Health) and the identification of Drug Related Problems (DRP) using PCNE V9.0, three problems were found, namely C1. drug selection; C2. medicinal form; C3 Dosage selection."

"Pemantauan Terapi Obat pada Neonatus Kurang Bulan, Bayi Berat Lahir Rendah, dan Sepsis di Ruang Neonatus Intensive Care Unit (NICU) RSUP Fatmawati Bulan September 2020.
Neonates, especially preterm infants have weak physical defenses and immature immune function, making them susceptible to bacterial invasion. One of the efforts to achieve the Sustainable Development Goals (SDGs), which is to reduce the incidence of neonatal mortality to 12 per 1,000 live births, it is necessary to collaborate between health workers in treating patients. Pharmacists in hospitals need to carry out their roles, one of which is by monitoring the therapy given. Data were collected directly by looking at the cardex and patient's medical records with the following criteria: less-month neonatal patients, receiving polypharmacy, patients with antibiotic therapy, and length of stay in the hospital ≥5 days. The data obtained were analyzed using the SOAP method and identified problems related to drugs. From the Monitoring of Drug Therapy (PTO), it was found that drug-related problems that occurred were dose mismatches and frequency mismatches, and analysis of the evaluation of antibiotic administration (EPA) with the Gyssens method found that the use of antibiotics was found in category IIIa, namely giving meropenem too long and category IIa, which was not the right dosage."
