Latar Belakang: Sel endotel kornea (SEK) paling mudah mengalami kerusakan pasca fakoemulsifikasi (fako). Pengaturan parameter fako menjadi salah satu cara untuk mengurangi kerusakan SEK. Tekanan intaokular (TIO) selama fako berlangsung mempengaruhi kenyamanan pasien. TIO dipengaruhi oleh pengaturan parameter fako. Tujuan: Membandingkan pengaturan fako dengan parameter high (H) dan low (L) di Rumah Sakit Cipto Mangunkusumo (RSCM) terhadap SEK dan persepsi nyeri pasien selama fako. Desain: Uji klinis randomisasi tersamar ganda. Hasil: 48 sampel untuk kedua kelompok fako parameter high dan low terkumpul selama periode November 2013-April 2014. Penilaian objektif SEK meliputi endothel cell density (ECD) dan central corneal thickness (CCT). Persepsi nyeri untuk menilai persepsi nyeri pasien digunakan kartu visual analog scale (VAS) yang telah menjadi standar JCI di RSCM. Terjadi peningkatan CCT dan penurunan ECD kedua kelompok parameter pasca fako 1 bulan, masing-masing 0.23VS2.23 dan 8.53VS6.99 (p>0.05). Tidak ada perbedaan signifikan pada VAS kedua parameter. Efikasi fako berdasarkan penilaian cumulative dissipated energy (CDE) kelompok H lebih baik daripada L (15.80VS21.29). Kesimpulan: Tidak ada perbedaan keamanan dan kenyamanan pasien fako parameter H dan L. ......Background: Corneal endothelial cell (CEC) prone to damage after phacoemulsification (phaco). Phaco parameter setting is an effort to reduce damage to the CEC. Patient?s comfort during phaco is influenced by IOP during phaco, in which are influenced by parameter settings. Purpose: To compare phaco setting parameters from high (H) and low (L) parameters in Cipto Mangunkusumo (CM) hospitals impacted on CEC and patient?s pain perception (PP) during phaco procedure. Study design: randomized control trial double blind. Results: 48 outpatients were elegibly selected by RCT at CM hospital in periods of November 2013 ? April 2014. Impact of setting parameter difference were observed by objective measurement of endothel cell density (ECD), central corneal thickness (CCT). For PP a JCI approved standard using visual analog scale (VAS) were adapted. A built in software for phaco US energy count which is cumulative dissipated energy (CDE) used to objectively timed the phaco time, duration of operation (DO) were timed, and standard visual acuity (VA) was also noted. Analisis data using general linear model (GLM) repeated measures. Increase of CCT and decrease of ECD after 1 month in high and low phaco parameter are not significantly difference, respectively 0.23VS2.23 and 8.53VS6.99 (p>0.05).Significant difference were found in CDE between H and L; 15.80VS21.29 (p0.015).No statisticaly significant difference of VAS nor DO and VA. Conslusion: No different in safety and patient's comfort using high and low parameter phaco.

Tujuan: membandingkan efektivitas yang dinilai berdasarkan Cumulative Dissipated Energy (CDE), Phaco Time, Best Corrected Visual Acuity (BCVA), dan Keamanan yang diukur berdasarkan Endothelial Cell Density (ECD), Central Corneal Thickness (CCT), dan Balanced Salt Solution. (BSS) volume yang digunakan, dari empat dan enam segmen nukleofraktis dalam teknik fakoemulsifikasi stop and chop untuk katarak sedang-keras. Metode: Uji klinis prospektif yang melibatkan 42 dengan densitas nuklear derajat NO/NC 3-5 berdasarkan Lens Opacities Classification System III (LOCS III), dirandomisasi menjadi dua kelompok nukleofraksis, empat segmen (21 subjek) atau enam segmen (21 subjek). Pengukuran objektif dilakukan pre operatif, 1 hari, 1 minggu, dan 1 bulan pasca operasi yang meliputi ECD, CCT, dan TPDK. Intra- operatif dinilai CDE, phaco time, dan volume BSS yang terpakai. Hasil: terjadi penurunan ECD (5.76 ± 29.08 μm VS 2.33 ± 13.73 μm) dan peningkatan CCT (346.42 ± 154.45 sel/mm2 VS 247.05 ± 160.40 sel/mm2) pada kedua kelompok pada satu bulan pasca operasi. Tidak ada perbedaan yang bermakna pada TPDK satu bulan pasca operasi kedua kelompok (logMAR 0.05 VS 0.04). Parameter intra-operatif dalam kelompok empat segmen (CDE 20.73 ± 6.46, phaco time 78.49 ± 23.63 detik, BSS 59.38 ± 12.04 ml) sebanding dengan kelompok enam segmen (CDE 20.46 ± 5.47, phaco time 78.62 ± 13.80 detik, BSS 58.86 ± 13.32 ml), dan tidak ada perbedaan yang bermakna secara statistik. Simpulan: tidak terdapat perbedaan yang bermakna secara statistik parameter efektivitas dan keamanan antara kelompok nukleofraksis empat segmen dan enam segmen. Namun, pasca operasi nukleofraksis empat segmen mempunyai kecenderungan menimbulkan efek kerusakan endotel lebih banyak dibandingkan nukleofraksis enam segmen. ......Objective: to compare effectiveness, assessed by cumulative dissipated energy (CDE), phaco time, best corrected visual acuity (BCVA), and safety which were observed by endothelial cell density (ECD), central corneal thickness (CCT), and balanced salt solution (BSS) volume used, of four and six segments nucleofractis in stop and chop phacoemulsification technique for moderate-hard cataract. Methods: This prospective study comprised forty-two subjects with NO/NC 3-5 nuclear density according to the Lens Opacities Classification System III (LOCS III) system. Patients were equally randomized into four segments or six segments nucleofractis group. Stop-and-chop technique were applied in all subjects. The objective measurements of ECD, CCT, and BCVA were performed pre-operative, 1 day, 1 week, and 1 month post-operative. Phaco time, CDE and BSS volume were measured intraoperatively. Results: The mean ECD were reduced (5.76 ± 29.08 μm VS 2.33 ± 13.73 μm) and CCT increased (346.42 ± 154.45 cells/mm2 VS 247.05 ± 160.40 cells/mm2) in both groups after 1 month follow-up. No statistically difference was found between mean BCVA at 1 month follow-up in both group (logMAR 0.05 VS 0.04). All intraoperative parameters of four segments group (CDE 20.73 ± 6.46, phaco time 78.49 ± 23.63 second, BSS 59.38 ± 12.04 ml) were comparable with six segments group (CDE 20.46 ± 5.47, phaco time 78.62 ± 13.80 second, BSS 58.86 ± 13.32 ml). Conclusions: No effectiveness and safety difference between four and six segments nucleofractic in stop and chop phacoemulsification technique for moderate - hard cataract. However, our study demonstrates the tendency of higher endothelial cell loss in four segments nucleofractis.

Tujuan: mengetahui tingkat efisiensi dan keamanan antara teknologi Ellips FX-Peristaltik dengan Ellips FX-Venturi pada fakoemulsifikasi katarak derajat sedang-keras. Desain: Penelitian ini merupakan uji klinis randomisasi. Metode: sebanyak 48 pasien dilakukan randomisasi untuk dilakukan fakoemulsifikasi. Setiap subjek diukur densitas sel endotel dan ketebalan kornea sentral. Efisiensi dinilai dari effective phaco time EFX . Mesin fakoemulsifikasi yang digunakan adalah Signature Ellips FX. Hasil: Tidak terdapat perbedaan yang signifikan untuk median phaco time, penurunan densitas sel endotel dan peningkatan ketebalan kornea sentral. Tidak terdapat komplikasi pada penelitian ini. Kesimpulan: tidak ada perbedaan efisiensi dan keamanan antara sistem Ellips FX-Peristaltik dengan Ellips FX-Venturi. ......Purpose: To know the level of efficiency and safety of the technology Ellips FX peristaltic with Ellips FX Ventury in phacoemulsification of moderate to hard grade cataract. Methods: 48 outpatients were eligible selected by RCT at CM hospital in periods of January 2016 ndash June 2016. Impacts of pumps system setting difference were observed by objective measurement of endothelial cell density ECD and central corneal thickness CCT. Efficiency was recorded as effective phaco time EFX. Signature Ellips FX were used as phacoemulsification machine. Result: The total sample that has been analyzed was 46 patients. There is no significant difference in median phaco time, a decrease in endothelial cell density and an increase in central corneal thickness. There were no complications in this study.
