"Praktik kerja profesi di PT Taisho Pharmaceutical Indonesia Periode Bulan Februari - Maret 2020 bertujuan untuk memahami peranan, tugas, dan tanggung jawab apoteker di industri farmasi, memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan pekerjaan kefarmasian di industri farmasi, memahami penerapan Cara Pembuatan Obat yang Baik (CPOB) di Industi Farmasi, dan memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di industri farmasi. Sedangkan, praktek kerja profesi di Apotek Roxy Depok Periode Mei 2020 bertujuan agar calon apoteker memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktek pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika yang berlaku, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktik kefarmasian di apotek, memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktik kefarmasian.

Internship at PT Taisho Pharmaceutical Indonesia Period February - March 2020 aims to understand the duties and responsibilities of pharmacists in the pharmaceutical industry, have insight, knowledge, skills and practical experience to undertake pharmaceutical work in the pharmaceutical industry, and understanding of the application of good manufacturing practice (GMP) in the pharmaceutical industry. While, internship at Apotek Roxy Depok on May 2020 is intended to allow the apothecary profession students to directly understand the duties and responsibilities of pharmacists in pharmacy management, do the pharmaceutical practice accordance to laws and ethics, and fulfill the knowledge, skills, practical experience and also have the insight of pharmaceutical practice issues in the real conditition."

"ABSTRAKSeorang apoteker memegang peranan penting, salah satunya dalam bidang industri farmasi dan apotek. Apoteker harus memenuhi standar kompetensinya sebagai persyaratan untuk memasuki dunia kerja dan menjalani praktik profesi. Standar kompetensi apoteker Indonesia terdiri dari sepuluh (10) standar kompetensi sebagai kemampuan yang diharapkan oleh apoteker saat lulus dan masuk ke dalam dunia kerja. Sebagai bekal dan pengalaman calon apoteker untuk memahami perannya dan meningkatkan kemampuan kompetensinya, maka pada kesempatan kali ini, penulis bereksempatan untuk mengikuti kegiatan Praktik Kerja Profesi Apoteker (PKPA) di PT. Taisho Pharmaceutical Indonesia dan Apotek Roxy Pitara selama periode bulan Februari hingga Juni 2020. Selama PKPA ini, diharapkan calon apoteker dapat memperluas wawasan, pemahaman, dan pengalaman untuk melakukan pekerjaan kefarmasian di tempat praktik kerja profesi nantinya.

ABSTRACTA pharmacist plays an important role, one of which are in the pharmaceutical industry and pharmacies. Pharmacists are required to meet competency standards as a requirement in professional field and carry out their professional practice properly. Pharmacist competency standards in Indonesia consist of ten (10) competency standards as the abilities expected by pharmacists when they graduate and enter the professional field. As a provision and experience of prospective pharmacists to understand their role and improve their competence, on this occasion, the authors had the opportunity to participate in the Pharmacist Professional Work Practices (PKPA) at PT. Taisho Pharmaceutical Indonesia and Apotek Roxy Pitara during the period from February to June 2020. During this PKPA, it is hoped that prospective pharmacists can broaden their horizons, understanding, and experience to do pharmaceutical work in the professional practice later."

Sebagai bekal dan pengalaman calon apoteker untuk dapat memamahami peran apoteker dan meningkatkan kompetensi, maka dilaksanakan Praktik Kerja Profesi Apoteker (PKPA) di PT Taisho Pharmaceutical Indonesia Tbk dan Apotek Safa Bukit Duri periode bulan Februari – Mei tahun 2020. Selama PKPA, calon apoteker diharapkan dapat menambah ilmu, wawasan dan pengalaman untuk melakukan pekerjaan kefarmasian di tempat praktik kerja profesi.

Pharmacists play an important role in the pharmaceutical industries and pharmacy. Prospective pharmacists must meet competency standards as the requirement for entering the workforce and undergoing professional practice. Indonesian Pharmacist Competency Standards consist of ten competency standards that expected to be owned by pharmacists when graduating and entering professional work practices. As a preparation and to get the experience to be able to understand the role and competences of pharmacists, prospective pharmacists must undergo Pharmacist Professional Work Practices/ Praktik Kerja Profesi Apoteker (PKPA) at PT Taisho Pharmaceutical Indonesia Tbk and Apotek Safa Bukit Duri period February - May 2020. During PKPA, prospective pharmacists are expected to get knowledge, insight and experience to do pharmaceutical work at professional work practices."

"Praktik kerja profesi apoteker di PT. Taisho Pharmaceutical bertujuan untuk mengetahui peran apoteker dalam industri farmasi. Di Industri farmasi apoteker memegang peran penting dalam proses produksi, pengendalian mutu dan pemastian mutu dari produk farmasi yang dihasilkan. Di dalam proses produksi, salah satu peran apoteker adalah menjamin proses produksi dijalankan sesuai dengan kaidah Cara Pembuatan Obat yang Baik. Oleh karena itu, proses produksi selalu didokumentasikan dalam batch record yang terdiri dari dokumen pengelolaan induk dan dokumen pengemasan induk. Dokumen pengemasan induk berisi informasi terkait proses pengemasan tiap produk spesifik yang disusun secara jelas untuk menghasilkan produk akhir yang aman, bermutu dan berkhasiat serta ekonomis. Dalam mencapai tujuan tersebut, PT. Taisho Pharmaceutical melakukan simplifikasi dokumen pengemasan induk yang mengacu pada konsep lean manufacturing. Selama program praktik kerja ini, telah berhasil dilakukan simplifikasi dokumen pengemasan induk dari produk theragran tablet, engran tablet, kenacort tablet, myco-z cream, kenalog in oral base, stugeron tablet, Imodium tablet, daktarin cream 5 gram, daktarin cream 10 gram, dan nizoral ss 1%.

Pharmacist internship in PT. Taisho Pharmaceutical aims to determine the role of pharmacists in the pharmaceutical industry. Pharmacist in pharmaceutical industry plays an important role in the production process, quality control and quality assurance of pharmaceutical products produced. In the production process, one of the pharmacist's role is to ensure that the production process is executed in accordance with the principles of Good Manufacturing Practice. Therefore, the production process is always documented in the batch record consisting of manufacturing instruction and packaging instruction. Packaging instruction contains information about packaging process for each specific product are clearly arranged to produce a final product that is safe, quality, efficacious and economical. In achieving these objectives, PT. Taisho Pharmaceutical did simplification for the packaging instruction with the concept of lean manufacturing. During internship, successfully did simplification of packaging instruction of theragran tablets, engran tablets, kenacort tablets, Myco-z cream, kenalog in oral base, stugeron tablets, Imodium tablets, daktarin cream 5 grams, 10 grams of daktarin cream, and Nizoral SS 1%."

Taisho Pharmaceutical Indonesia Tbk., serta mengetahui dan memahami peran dan tanggung jawab Apoteker dalam industri farmasi yang diharapkan dapat menjadi bekal untuk menghadapi dunia kerja yang sesungguhnya. Tugas khusus yang diberikan berjudul Simplifikasi Dokumen Manufacturing Instruction (MI), Packaging Instruction (PI), dan Laboratory Report. Tugas khusus ini bertujuan untuk menyederhanakan ketiga dokumen tersebut sehingga menghindari pengisian dokumen dengan informasi yang sama secara berulang-ulang.

Taisho Pharmaceutical Indonesia Tbk., and to know and understand the roles and responsibilities of pharmacists in the pharmaceutical industry. It is expected to help pharmacist student to face the real working world. The special task is about simplification of Manufacturing Instruction (MI), Packaging Instruction (PI), and the Laboratory Report. This special task aims to simplify the those documents thus avoiding the charging document with the same information repeatedly."

"Latar Belakang : Karsinoma Sel Hati (KSH) merupakan kanker dengan prognosis yang buruk dan Indonesia termasuk negara dengan prevalensi hepatitis B yang tinggi. Performa alfa fetoprotein (AFP) sebagai penanda tumor pada surveilans KSH terutama dipengaruhi oleh etiologi penyakit hati yang mendasari. Titik potong AFP untuk surveilans KSH di Indonesia tidak berdasarkan etiologi penyakit hati yang mendasari.Tujuan : Mengetahui titik potong terbaik pemeriksaan biomarker AFP untuk surveilans KSH dengan etiologi hepatitis B kronik.Metode : Penelitian mengambil data rekam medis Divisi Hepatobilier RSUPN Dr. Cipto Mangunkusumo periode tahun 2017-2023. Sebanyak 506 subjek hepatitis B kronik semua spekturm (hepatitis B tanpa sirosis, sirosis hati, dan KSH stadium awal BCLC 0 dan A) diambil secara total sampling dalam kurun waktu 26 Juli 2023 hingga 31 Agustus 2023. Penentuan nilai titik potong AFP dilakukan dengan metode receiver operating characteristics (ROC).Hasil : Untuk surveilans KSH dengan etiologi hepatitis B, analisis kurva ROC memberikan area under the curve (AUC) didapatkan 0.792 (IK 95%, 0.719-0.866), dan titik potong dengan index Youden tertinggi adalah 8.7 ng/ml, dengan nilai sensitivitas 58%, spesifisitas 94%, nilai duga positif (NDP) 56.14%, nilai duga negatif (NDN) 94.43%. Analisis kurva ROC dilanjutkan berdasarkan status sirosis pasien. Untuk menentukan titik potong yang membedakan KSH dengan sirosis hati dari sirosis hati saja, menghasilkan AUC 0.803 (IK 95%, 0.722-0.884) dengan titik potong yang didapatkan adalah 8.6 ng/ml, dengan sensitivitas, spesifisitas, NDP, NDN, dan RK + adalah 60.5%, 92.4%, 45,10%, 95,24%, dan 7,09 masing-masing. Untuk menentukan titik potong yang membedakan KSH tanpa sirosis hati dari hepatitis B kronik tanpa sirosis hati, menghasilkan AUC 0.777 (IK 95%, 0.631-0.923) dengan titik potong yang didapatkan adalah 6.6 ng/ml yang memberikan hasil sensitivitas, spesifisitas, NDP, NDN, dan RK positif adalah 63.16%, 98.35%, 85.71%, 94.44%, dan 38.21 masing-masing.Kesimpulan : Titik potong surveilans KSH dengan etiologi spesifik hepatitis B lebih rendah dibandingkan dengan nilai titik potong AFP surveilans KSH sebelumnya yang tidak spesifik etiologi. Nilai titik potong 8.7 ng/ml menghasilkan sensitivitas dan spesifisitas terbaik untuk titik potong surveilans KSH dengan etiologi hepatitis B. Pada pasien KSH non sirosis, titik potong surveilans AFP lebih rendah yakni 6.6 ng/ml. Hal ini perlu menjadi perhatian klinisi dalam surveilans kelompok pasien hepatitis B kronik tanpa sirosis.

Background: Hepatocellular carcinoma (HCC) is a cancer with a poor prognosis. Indonesia is a country with a high prevalence of chronic hepatitis B infection. The performance of alpha fetoprotein (AFP) as a tumor marker in HCC surveillance is primarily influenced by the etiology of the underlying liver disease. The AFP cutoff value for HCC surveillance in Indonesia is not based on the etiology of the underlying liver disease.

Objective: To determine the best cut-off value of AFP biomarker examination for HCC surveillance in chronic hepatitis B patients.

Methods: The study took medical record data from the Hepatobiliary Division of RSUPN Dr. Cipto Mangunkusumo for the 2017-2023 period. A total of 506 chronic hepatitis B subjects of all spectrums (hepatitis B without cirrhosis, liver cirrhosis, and early stage HCC, BCLC 0 and A) were taken by total sampling in the period 26 July 2023 to 31 August 2023. Determination of the AFP cut-off value was carried out using the receiver operating characteristics (ROC) method.

Results: For HCC surveillance caused by hepatitis B virus, ROC curve analysis gave an area under the curve (AUC) of 0.792 (95% CI, 0.719-0.866), and the cut-off value with the highest Youden index was 8.7 ng/ml, with a sensitivity value of 58%, specificity 94%, positive predictive value (PPV) 56.14%, negative predictive value (NPV) 94.43%. ROC curve analysis was then performed based on the patient's cirrhosis status. ROC curve analysis to determine the cut-off point that distinguishes HCC with liver cirrhosis from liver cirrhosis alone, resulted in an AUC of 0.803 (95% IK, 0.722-0.884) with a cut-off point 8.6 ng/ml, with sensitivity, specificity, PPV, NPV, and LR+ of 60.5%, 92.4%, 45.10%, 95.24%, and 7.09 respectively. ROC curve analysis to determine the cut-off point that distinguishes HCC without liver cirrhosis from chronic hepatitis B without liver cirrhosis resulted in an AUC of 0.777 (95% CI, 0.631-0.923) with a cut-off point 6.6 ng/ml which gave sensitivity, specificity, PPV, NPV, and LR positive results of 63.16%, 98.35%, 85.71%, 94.44%, and 38.21 respectively.

Conclusion: The cut-off value of AFP in HCC surveillance on hepatitis B specific etiology is lower than the cut-off value of AFP in previous HCC surveillance which was not etiology specific. The cut-off value of 8.7 ng/ml produces the best sensitivity and specificity for the cut-off value for HCC surveillance with hepatitis B etiology. In non- cirrhotic HCC patients, the AFP surveillance cut-off point is lower than cirrhotic HCC patients (6.6 ng/ml). This needs to be of concern to clinicians in surveillance of groups of chronic hepatitis B patients without cirrhosis."
