"Menurut data dari Tuberculosis TB national prevalence survey dalam TB Global Report 2015, angka prevalensi TB di Indonesia sekitar 650 jiwa dengan angka insidensi 403 jiwa per 100.000 penduduk. Pada pasien TB paru sering dijumpai temuan lesi paru yang khas yaitu kavitas. Jumlah pasien TB dengan kavitas pada orang dewasa bervariasi sekitar 30-50 persen. Di dalam kavitas diperkirakan terdapat sekitar 107 sampai dengan 109 bakteri. Hal ini menunjukan adanya bahwa kavitas merupakan faktor risiko penting dari kegagalan pengobatan dan relaps serta kemungkinan berhubungan dengan resistansi obat.
Penelitian bertujuan untuk mengetahui proporsi pasien TB paru, kavitas pada TB paru serta hubungan antara tingkat bacterial load dengan kavitas pada pasien TB paru. Penelitian ini merupakan studi cross sectional yang diambil dari pasien pada rentang tahun 2013 sampai tahun 2015 sebanyak 145 subjek.
Hasil studi menunjukan bahwa tingkat kepositivan BTA pada BTA 3 ,2 dan 1 ,1-9 batang memiliki hubungan bermakna pada kejadian lesi kavitas p=0.018. Ditemukan pula pada BTA positif dan negatif terdapat hubungan bermakna dengan kejadian lesi kavitas p=0.05 . Perlu dilakukan studi lebih lanjut dengan desain penelitian yang berbeda dengan desain kohort prospektif serta subjek diajarkan cara pengambilan sputum yang benar.

According to data from Tuberculosis TB national prevalence survey in Global TB Report 2015, the prevalence of TB in Indonesia is around 650 people with the incidence of 403 people per 100,000 population. In patients with pulmonary TB, cavity is a typical common finding in lung lesion. Number of TB patients with cavities in adults varies between 30 50 percent. Inside the cavity, it is estimated that there are about 107 up to 109 bacteria. This shows that the cavity is an important risk factor of treatment failure and relapse as well as possibly related to drug resistance.
The aim of this study is to determine the proportion of patients with pulmonary TB, cavity in pulmonary TB, and the association between the level of bacterial load and the occurrence of cavity in patients with pulmonary TB. The study was a cross sectional study which was taken from 145 patients from years 2013 to 2015.
The results showed that the level of positivity in AFB 3 , 2 , and 1 , 1 9 rods has a significant relationship to the occurrence of cavity lesions p 0.018 . It was also found on the positive and negative AFB that there was a significant relationship with the occurrence of the cavity lesion p 0.05 . Further study is needed to be carried out with different design, which is prospective cohort study and the subject is taught the correct way to collect sputum."

"ABSTRAK
Latar belakang : Prevalens diabetes melitus (DM) terus meningkat di negara
berkembang yang merupakan negara endemis tuberkulosis (TB). Diabetes melitus
meningkatkan risiko infeksi, hambatan konversi sputum dan kegagalan
pengobatan TB. Penelitian ini bertujuan menganalisis faktor-faktor yang
mempengaruhi konversi sputum Basil Tahan Asam (BTA) mikroskopik pada
akhir bulan kedua pengobatan TB pada pasien TB paru kasus baru dengan DM.
Metode : Penelitian kohort retrospektif ini dilaksanakan di RSUP Perahabatan
terhadap pasien-pasien TB paru BTA positif kasus baru dengan DM yang berobat
pada periode Juli 2012 sampai Juni 2015. Hubungan faktor risiko dengan konversi
sputum BTA mikroskopik dianalisis dengan analisis bivariat dan multivariat.
Hasil penelitian: Proporsi kegagalan konversi sputum BTA mikroskopik pada
pasien TB paru kasus baru dengan DM adalah 43,04%, sedangkan pada pasien
tanpa DM 22,75% (p<0,001, KI95% 0,11-0,30) dan risiko relatif (RR) kegagalan
konversi 1,89 kali lebih tinggi pada kelompok DM. Faktor risiko yang meningkat
bermakna pada pasien gagal konversi adalah kadar kepositifan sputum BTA
sebelum terapi (p=0,021), HbA1c (p=0,014), GDP (p=0,047), GD 2jam PP
(p=0,030) dan kavitas pada foto toraks (p=0,033) sedangkan albumin serum lebih
rendah bermakna (p=0,013). Analisis multivariat mendapatkan faktor risiko
terkuat untuk kegagalan konversi sputum adalah kadar albumin serum yang
rendah (p=0,046, aOR 0,464, KI95% 0,218-0,986), tingkat kepositifan sputum
BTA mikroskopik sebelum terapi yang tinggi (p=0,009, aOR 2,313, KI95%
1,230-4,349) dan kadar HbA1c yang tinggi (p=0,018, aOR 1,298, KI 95% 1,047-
1,610).
Kesimpulan: Tingkat kepositifan sputum BTA sebelum terapi yang tinggi, status
kontrol DM yang tidak baik, kavitas pada foto toraks dan kadar albumin serum
yang rendah meningkatkan risiko kegagalan konversi sputum BTA pada pasien
TB dengan DM.ABSTRACT

treatment failure. This study would like to evaluate factors associated with
delayed sputum smear conversion in the end of two months of TB treatment in
new cases TB with DM patients in Persahabatan Hospital.
Methods: This retrospective cohort study was conducted in Persahabatan
Hospital, included all new cases TB with DM patients in the period from July
2012 - June 2015. All the risk factors performed bivariate and multivariate
analysis in association with sputum smear conversion in the end of two months of
TB treatment.
Results: The proportion of sputum smear conversion failure is higher in TB-DM

"Di Indonesia, tingkat keberhasilan pengobatan tuberkulosis hanya mencapai 84% dan kemungkinan terjadinya kekambuhan berada pada 2%. Namun demikian, masih sangat sedikit penelitian yang memelajari hubungan antara ketidak teraturan obat anti tuberkulosis dengan hasil uji sputum BTA pada pasien TB kambuh. Pengambilan data dilakukan di Rumah Sakit Umum Persahabatan dengan menggunakan metode cross sectional. Target populasi dari penelitian ini adalah pasien yang terdiagnosis TB kambuh pada tahun 2018.
Dari 40 subjek penelitian, didapati subjek laki-laki berjumlah 19 (47,5%) dan perempuan berjumlah 21 (52,5%). Berdasarkan usia, jumlah kasus kambuh terbanyak dapat ditemui di rentang usia 36-55 dan 46-55 dengan jumlah 10 (25%). Didapati 24 (60%) subjek memiliki riwayat pengobatan yang tidak teratur dan hasil BTA tertinggi adalah negatif dengan jumlah subjek 13 (35%).
Dari hasil analisis chi square, didapatkan p=0,00883 dengan OR 6,43 (IK95% 1,495-27,646) dan dari hasil analisis Mann Whitney, didapatkan p=0,014 dengan rerata peringkat 15,06 dan 24,13 untuk riwayat pengobatan yang teratur dan tidak teratur. Ada hubungan antara riwayat pengobatan tuberkulosis dengan hasil jumlah BTA dengan nilai OR 6,43 dengan IK95% 1,495-27,646, dan tren hasil jumlah BTA yang cenderung naik lebih tinggi pada riwayat pengobatan yang tidak teratur.

In Indonesia, the success rate of tuberculosis treatment is only at 84% while the probability of a relapse case to occur is 2%. However, studies regarding the relation of previous tuberculosis regiments with AFB sputum smear are very limited. Datas are collected from RSUP Persahabatan by using cross-sectional method. Subjects of this experiment are patient that has been diagnosed with relapse tuberculosis in the year 2018. From 40 subjects, the ratio between male and female is 47,5% and 52,5% respectively.
Most subjects are on the age range of 36-45 and 46-55 (10 subjects each). Among those 40 subjects, 24 (60%) has been found to have irregular precious TB regiments while 13 has negative results of AFB sputum smear. Upon bivariate analysis with chi square, it is found that patients with irregular previous TB regiments are 6,43 times more likely (p=0,00883 OR 6,43 CI95% 1,495-27,646) to have a positive AFB sputum smear than those with regular previous TB regiments.
Upon using Mann Whitney analysis, it is found that average rank of irregular treatment and regular treatment is 24,13 and 15,06 respectively. There is a relation of previous TB regiments with results of AFB sputum smear with OR 6,43 CI95% 1,495-27,646 and a positive trend of AFB sputum smear on patients with irregular previous TB treatment.

"Perbedaan antara tes untuk infeksi tuberkulosis (TB) yang resistan terhadap obat menjadi lebih umum karena alat diagnostik menjadi lebih bervariasi. Hal tersebut membingungkan dokter karena belum ada tes TB diagnostik cepat dengan sensitivitas dan spesifisitas yang baik. Kasus suspek-TB di RSUPP, pusat primer dan tersier untuk kasus TB paru Indonesia, disaring dengan GeneXpert MTB / RIF dan dikonfirmasikan dengan uji kepekaan obat anti-tuberkulosis.
Discrepancies between tests for drug-resistant tuberculosis (TB) infections are becoming more common as diagnostic tools become more varied. These discrepancies confuse clinicians because there is not yet a rapid diagnostic TB test with good sensitivity and specificity. Suspected-TB cases at Rumah Sakit Umum Pusat Perhasabatan (RSUPP), a primary and tertiary center for Indonesia’s pulmonary TB cases, are screened with GeneXpert MTB/RIF and confirmed with conventional drug- susceptibility testing (DST)."

"ABSTRAK
Indonesia merupakan negara dengan beban ganda pada penyakit menular dan tidak menular. Tuberkulosis merupakan salah satu penyakit infeksi yang memiliki prevalensi cukup tinggi dengan Indonesia berperingkat dua dari seluruh negara dalam hal jumlah pasien tuberculosis, diabetes mellitus merupakan salah satu penyakit tidak menular yang memiliki prevalensi cukup tinggi juga. Konvergensi keduanya menimbulkan implikasi serius terhadap morbiditas dan mortalitas dari masing-masing penyakit tersebut, pada tuberkulosis dengan adanya komorbiditas ini meningkatkan resiko munculnya kavitas; kavitas inilah yang memiliki implikasi untuk memperlambat konversi. Dengan menggunakan metode cross-sectional serta analisisi chi-square pada pasien tuberkulosis dengan komorbiditas diabetes mellitus diteliti. Hasilnya tidak ditemukan adanya hubungan yang jelas pada subjek yang memiliki lesi kavitas dan tidak terutama pada konversi pasca 2 bulan pengobatan. Sehingga terdapat perbedaan antara pasien dengan komorbid diabetes mellitus dan tuberkulosis pada umumnya. Perlu dilakukan studi lebih lanjut dengan desain kohort prospektif dengan menghitung kontrol gula darah pada diabetesnya

ABSTRACT
Indonesia is a country with a double burden of both transmissible and intransmissible disease. Tuberculosis is one of the transmissible disease that is quite prevalent in Indonesia and currently Indonesia is second in number of tuberculosis patient, meanwhile one of the leading chronic disease in number in Indonesia is diabetes Mellitus. The convergence of both disease could lead to serious implication in both the morbidity and mortality of each, from tuberculosis standpoint it could lead to delays in elongated duration in treatment; also cavitary lesion is shown to be more common in diabetes mellitus patient, and in general tuberculosis patient it can result in delay of conversion. So, using chi-square analysis the relationship between cavitary lesion and conversion in patient with tuberculosis comorbidity diabetes mellitus is studied. The result is inconclusive (p=0.906) with the realiton between cavitary lesion and conversion in tuberculosis patient with diabetes mellitus after 2 month intensive treatment. Thus this study should be reassessed by using prospective study design and with the control of glucose level to be respected

Pendahuluan: Pneumonia komunitas CAP salah satu penyebab kematian tertinggi. Tujuan mengetahui respons pengobatan selama perawatan pasien CAP secara empiris serta faktor yang berkaitan dengan pola kuman, respons pengobatan, gejala klinis, laboratorium, foto toraks, lama rawat dan faktor komorbid di RS persahabatan.
Metode: Kohort prospektif pasien pneumonia komunitas rawat inap di RS Persahabatan selama 15 bulan terkumpul 47 pasien. Gejala klinis, hasil laboratorium, foto toraks dan hasil mikrobiologi. Sampel mirkobiologi dikumpulkan sebelum dan sesudah pemberian antibiotik.
Hasil: Terkumpul 47 pasien. laki-laki 74,5% dan perempuan 25,5%. Rerata umur 61 tahun. Gejala klinis awal paling banyak sesak napas 51% berkurang 27,7% dan batuk 32% berkurang 23,4%. Nilai awal leukosit rerata 15,27. sel/mm3 berkurang 12,0. sel/mm3. Foto toraks awal infiltrat 89,3% menurun 38,3%. Patogen pada sputum sebelum penggobatan Klebseiella pneumonia 34,0%. Hasil sputum pasca terapi empiris eradikasi 91.5%. Pengobatan antibiotik tersering seftriakson. Faktor komorbid tersering keganasan rongga toraks. Lama rawat minimal 4 hari dengan terapi sulih minimal 3 hari.
Kesimpulan: Pasien CAP paling dominan menunjukan gejala klinis sesak napas dan batuk, gambaran infiltrat pada foto toraks dan gram-negatif Klebsiella pneumonia pada sputum. Terjadi penurunan leukosit setelah pemberian antibiotik. Terapi empiris dengan antibiotik tunggal masih sensitif.

ABSTRACT
Introduction : Pneumonia is the first leading disease with the highest mortality in hospitalized patients. The purpose of this study is to determine treatment response for the empirical treatment of CAP patients and factors associated with patterns of bacteria, treatment response, clinical symptoms, laboratory and chest X-ray, length of stay and comorbidities in Persahabatan Hospital, Jakarta.
Methods : Prospective cohort study in hospitalized community acquired pneumonia patients at Persahabatan Hospital while 15 month. Clinical symptoms, laboratory findings, chest x-ray and microbiologic. Microbiologic sample is before and after antibiotic administration.
Results : There were 47 patients. Male accounted 74,5% and female 25,5%. The average age was 61 years old. Clinical symptoms before treatment were dyspnea 51% decreased to 27,7% and cough 32% decreased to 23,4%. Leukocytes count was 15,27 cell/mm3 decreased to 12,0 cell/mm3. Chest x-ray infiltrates 89,3% decreased to 38,3%. Before-treatment microbiological patterns were K. pneumoniae 34,0%. Result after empirical treatment was eradication 91,5%. The most frequent innitial antibiotic administration was ceftriaxone.The most frequent comorbidity was thoracic malignancy. The patients were hospitalized at least for 4 days with replacement therapy at least for 3 days.
Conclusion: Patients with CAP predominantly showed symptoms of dypnea and cough, infiltrates on chest x-ray and gram-negative Klebsiella pneumonia in sputum samples. There were resolution of leucocyte counts after antibiotic administration. Empirical antibiotic treatments with single drug were still sensitive."

"Latar belakang: Hubungan antara TB dan DM sudah lama diketahui. Sistem imun rendah pada DM sehingga risiko berkembangnya TB laten menjadi TB aktif lebih tinggi. Penderita DM memiliki 2-3 kali risiko untuk menderita TB dibanding tanpa DM. Sekitar 10% kasus TB secara global berhubungan dengan DM. Prevalensi TB DM berdasarkan penelitian potong lintang oleh Wijayanto di Poli Endokrin RSUP Persahabatan pada pasien DM tipe 2 dari Oktober sampai Nopember 2013 adalah 28,2%. Pablos-Mendez dkk. menemukan bahwa hanya DM yang kurang terkontrol atau tidak terkontrol dikaitkan dengan peningkatan risiko TB.
Tujuan: Tujuan utama dari penelitian ini adalah untuk menentukan sensitivitas dan spesifisitas mikroskop sputum BTA dan Xpert MTB / RIF untuk diagnosis TB pada pasien dengan DM.
Metode: Penelitian ini adalah studi analitik deskriptif potong lintang pada pasien kelompok DM yang masih menjalani pengobatan di RS Persahabatan dari bulan Mei 2014 sampai jumlah sampel tercapai. Penelitian akan dilakukan di poli Paru dan Penyakit Dalam divisi Endokrin dan Metabolik RS Persahabatan. Pasien dengan keluhan tuberklosis baik yang belum pernah minum OAT atau dengan riwayat pengobatan sbelumnya dan didukung dari foto toraks yang menunjukan gambaran kecurigaan TB akan dilakukan pemeriksaan diagnostik mikrobiologis sputum BTA dan Xpert MTB/RIF.
Hasil: Tujuh puluh subjek diskrining dan hanya 55 subjek memenuhi kriteria inklusi. Terdapat 32 laki-laki (58,2%) dan 23 perempuan (41,8%). Umur antara 25-72 tahun. Indeks massa tubuh kurang 11 (20%), normal 31(56,4%) dan lebih 13(23,6%). Gula darah antara 110-481 mg/dl. Lama menderita DM <5 tahun adalah 40 (72,7%), 5-10 tahun adalah 7(12,7 %), 10-15 tahun adalah 6(10,9%) dan >15 tahun adalah 2(3,6%). HbA1c terkontrol 11(20%) dan tidak terkontrol 44(80 %). Riwayat tidak pernah TB 41 (74,5%), TB sembuh 9(16,4%) dan gagal atau putus obat 5 (9,1%). Sputum BTA(+) 29 (52,7%) dan Xpert MTB/RIF(+) 36(65,5%). Tingkat HbA1c tidak terkontrol BTA(+)/Xpert MTB/RIF(+) 30 (54,5%). Uji diagnostik kedua alat memiliki sensitivitas 77,8 % dan spesifisitas 94,7%.
Kesimpulan: Xpert MTB/RIF memiliki nilai kepositifan yang lebih tinggi daripada sputum BTA dalam mendeteksi M.Tb. Tidak ada perbedaan sensitivitas dan spesifisitas pemeriksaan mikroskopik sputum BTA dan Xpert MTB/RIF dalam upaya penegakan diagnosis TB pada pasien DM.

Background: The relationship between Tuberculosis (TB) and Diabetes Mellitus (DM) has long been known. People with DM have 2-3 times the risk of developing TB. The prevalence of TB-DM based on cross sectional study by Wijayanto in Endocrine clinic at Persahabatan Hospital in DM type 2 patients in 2013 was 28.2%. Pablos-Mendez et al. found that only DM poorly controlled or uncontrolled associated with the increased risk of TB.
Purpose: The main purpose of this study is to determine the sensitivity and specificity of Acid Fast Bacilli (AFB) smear and Xpert MTB/RIF for diagnosis of TB in DM.
Methods: This study is a cross-sectional descriptive analytic study in patients with DM group who are still undergoing treatment at Persahabatan Hospital from May 2014. The study is conducted in Pulmonary clinic and Internal Medicine clinic. Patients with DM who have TB complaints that have not been taken either TB drug or with a history of treatment and supported from chest X-ray shows the suspicion of TB will require the microbiological diagnosis of AFB smear and Xpert MTB/RIF.
Result: Seventy subjects are screening and only 55 subjects meet the inclusion criteria. There are 32 males (58.2%) and 23 females (41.8%). There are ages between 25-72 years old. Their body mass index are less than 11(20%), normal 31(56.4%) and over 13(23.6%). The blood sugar is between 110-481 g/dl. The length of suffer DM <5 years are 40(72.7%), 5-10 years were 7(12.7%), 10-15 years were 6(10.9%) and >15 years are 2(3.6%). The controlled HbA1c is 11(20%) and uncontrolled is 44(80%). By history of TB which has never been suffered are 41(74.5%), treatment cured are 9(16.4%) and failure or withdrawal are 5(9.1%). Smear of AFB(+)ve are 29(52,7%) and Xpert MTB/RIF(+) are 36(65,5%). Uncontrolled HbA1c levels pulmonary TB with AFB smear/Xpert (+)ve are 30(54.5%). Sensitivity and specificity by using both are 77.8% and 94.7%.
Conclusion: The Xpert MTB/RIF has a higher value of positivity that detect M.Tb than AFB smear.There is no difference in the sensitivity and specificity of AFB smear and Xpert MTB/RIF for TB diagnosis in patients with DM.

"Latar belakang : Diantara pasien TB paru resisten OAT terdapat cukup banyak pasien yang memiliki riwayat pengobatan sebelumnya.
Tujuan : Tujuan utama penelitian ini adalah untuk mengidentifikasi hubungan antara riwayat pengobatan TB paru sebelumnya dengan pola resistensi OAT di RS Persahabatan. Tujuan lain penelitian ini adalah untuk mendapatkan data mengenai riwayat pengobatan sebelumnya pasien TB paru resisten OAT, menilai peranan pasien dalam menyebabkan terjadinya TB paru resisten OAT dan menilai secara tidak langsung peranan tenaga kesehatan dalam menyebabkan terjadinya TB paru resisten OAT.
Hasil : Hasil penelitan ini menunjukkan terdapat 60 subjek yang terdiri dari 38 subjek laki - laki dan 22 subjek perempuan, sebagian besar subjek berasal dari kelompok umur 30 - 39 tahun (33,3%) dengan rata - rata umur 36,8 tahun. Sebanyak 81,7% mempunyai lebih dari 1 kali riwayat pengobatan TB paru sebelumnya, 28 subjek (46,7%) mempunyai 2 kali riwayat pengobatan TB paru sebelumnya dan terdapat 2 subjek (3,3%) dengan 5 kali riwayat pengobatan TB paru sebelumnya. Pada pengobatan pertama sampai pengobatan ketiga, lalai merupakan hasil pengobatan yang terbanyak (31,7%, 26,5% dan 33,3%) dan gagal konversi kategori II merupakan kriteria suspek TB paru resisten OAT yang terbanyak (46,7%). Sebanyak 71,7% subjek pernah mendapatkan pengobatan TB paru di fasilitas pelayanan kesehatan pemerintah. Dokter umum praktek swasta merupakan fasilitas kesehatan yang paling banyak tidak melakukan pemeriksaan BTA baik untuk diagnosis maupun untuk evaluasi pengobatan TB paru (66,7%, 60% dan 100%). Terdapat persentase pemberian rejimen pengobatan tidak sesuai yang lebih besar pada riwayat pengobatan kedua 32,7% apabila dibandingkan dengan pengobatan pertama 10%. Pola resistensi terhadap H - R - S - E adalah pola resistensi terbanyak (51,7%). Tidak terdapat hubungan antara jumlah riwayat pengobatan TB paru dengan pola resistensi, odds ratio pola resistensi H-R-S-E dengan jumlah riwayat pengobatan TB 1- 2 kali terhadap jumlah riwayat pengobatan TB 3 - 5 kali adalah 0,731 (CI 95% 0,245 - 2,074; p = 0,533).
Kesimpulan : Pola resistensi H - R - S - E adalah pola resistensi terbanyak. Tidak terdapat hubungan antara jumlah riwayat pengobatan TB paru dengan pola resistensi di RSUP Persahabatan.

Background : Most of anti tuberculosis drug resistant patients had previous history of pulmonary TB treatment.
Purpose : This study is primarily aimed to see the relationship between anti TB drug resistant patients previous history of TB treatment and patients resistance patterns at Persahabatan hospital. The other aims are to collect patient’s previous history of TB treament, see patient’s role in TB drug resistance and see the health worker’s role in TB drug resistance.
Method : This study uses cross sectional. Data collected by interview with questionnaire and from medical record.ving TB treatment and evaluation (66,7%, 44% and 100%).
Result : In this study there are 60 subjects that consist of 38 males and 22 females whose ages are around 30 to 39 (33,3%) and their mean age is 36,8. Around 81,7% of those subjects had more than 1 previous history of pulmonary TB treatment, 28 (46,7%) had twice and 2 (3,3%) had five times. Default from treatement mostly happened to the 1st up to 3rd of previous TB treatment (31,7%, 26,5% and 33,3%). Failure category 2 mostly happened to the drug resistant suspected criteria (46,7%). Around 71,7% subjects have been treated at the goverment healthcare facilities. Private clinics are the healthcare facilities that often don’t perform sputum smear before giving TB treatment. There are higher mistakes ATD regiment in 2nd previous previous TB treatment 32,7% if compared to 1st previous TB treatment 10%. Resistance to Isoniazid, rifampicin, ethambutol dan streptomycin is the the most frequent resistance patterns (51,7%). There is no correlation between the numbers of previous history of TB treatment and resistance patterns at Persahabatan hospital, the odds ratio resistance patterns H - R - S - E of 1- 2 times previous history of TB treatments to 3 - 5 times is 0,731 (CI 95% 0,245 - 2,074; p = 0,533).
Conclution : Resistance to H - R - S - E is the the most frequent resistance patterns. There is no correlation between the numbers of previous history of TB treatment and resistance patterns at Persahabatan hospital."

Latar Belakang: dahak BTA mikroskopik adalah teknik diagnostik cepat yang paling
banyak digunakan untuk mendiagnosis TB paru. Di RSUP Persahabatan, pemeriksaan
dahak BTA mikroskopik membutuhkan tiga pemeriksaan sampel dahak lebih dari dua
hari, (sewaktu pertama, pagi, sewaktu kedua). Pengambilan dua sampel dahak akan
mengurangi waktu, uang dan beban kerja laboratorium mikrobiologi
Tujuan: Penelitian ini bertujuan untuk mendapatkan sensitivitas dan spesifisitas
pemeriksaan dahak BTA mikroskopik dari tiga bahan pemeriksaan dahak dan dua
bahan pemeriksaan dahak.
Metode: Kami secara acak mengumpulkan data demografi dan hasil laboratorium (hasil
dahak BTA dan hasil biakan M.tuberculosis) dari pasien terduga TB kasus baru di RSUP
Persahabatan dari data bulan Januari 2012 hingga Desember 2013. Subjek harus berusia
minimal 15 tahun dan bukan pasien TB MDR dan bukan pasien HIV
Hasil: Dari 360 pasien terduga TB paru, sebanyak 210/360 (58,3%) memiliki hasil
biakan M. tuberculosis positif. Sensitivitas 100% dan spesifisitas 69.2% pada hasil
pemeriksaan BTA mikroskopik tiga kali (S1, P, S2) dan hasil pemeriksaan BTA
mikroskopik dua kali sensitivitasnya 89.8% dan spesifisitas 94.1%.
Kesimpulan: Hasil pemeriksaan dahak BTA mikroskopik 2 kali pemeriksaan
memberikan spesifisitas yang lebih tinggi bila dibandingkan dengan hasil pemeriksaan
3 kali dahak BTA mikroskopik dan memiliki sensitivitas yang masih cukup tinggi.

"Insidensi TB di Indonesia menempati urutan kedua terbesar di dunia tahun 2016. Buruknya, sebagian besar provinsi di Indonesia masih belum bisa mencapai target keberhasilan pengobatan tahun 2016, salah satunya DKI Jakarta. Hal ini dapat disebabkan karena banyaknya jumlah putus pengobatan (default treatment). Jika dibiarkan, kondisi ini dapat menimbulkan dampak yang lebih buruk yaitu peningkatan kasus TB MDR. Oleh karena itu faktor yang berhubungan dengan kejadian putus pengobatan perlu identifikasi, namun faktornya bervariasi di berbagai tempat.
Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kejadian putus pengobatan pada pasien TB MDR di RSUP Persahabatan tahun 2016-2018. Penelitian dilakukan dengan desain potong lintang menggunakan rekam medis dan wawancara 60 subjek.
Hasil penelitian menunjukkan prevalensi TB MDR dengan riwayat putus pengobatan di RSUP Persahabatan tahun 2016-2018 adalah 16,9%. Berdasarkan analisis bivariat, faktor yang memiliki hubungan signifikan dengan putus pengobatan adalah perilaku merokok (p=0,008). Melalui analisis multivariat diketahui bahwa hanya perokok sedang yang berhubungan dengan putus pengobatan (p = 0,035, OR = 4,364; KI95% = 1,112-17,128). Dari hasil tersebut disimpulkan bahwa perokok sedang merupakan faktor yang berhubungan dengan kejadian putus pengobatan TB pada pasien TB MDR.

TB incidence of Indonesia ranks the second largest in the world in 2016. Poorly, most provinces in Indonesia still cannot reach the target of successful TB treatment in 2016, one of which is DKI Jakarta. This can be caused by the large number of dropouts (treatment default). This finding is worrisome, individuals who default from tuberculosis (TB) treatment experience an increased risk of multi drugs resistance tuberculosis cases. Therefore it is important to identify local risk factors for default and for further research to demonstrate the best programme models for reducing default.
This study aims to determine the factors associated with default from TB treatment in multi drugs resistance tuberculosis patients in RSUP Persahabatan in 2016-2018. The study was conducted with a cross-sectional design using medical records and 60 tuberculosis patients were interviewed.
The results showed the prevalence of multi drugs resistance tuberculosis with a history of default tb treatment at RSUP Persahabatan in 2016-2018 is 16.9%. Based on bivariate analysis, significant factor associated with default for drug-sensitive TB programmes is smoking behavior (p = 0.008). In multivariate logistic regression, it was found that only moderate smokers were associated with default from treatment (p = 0.035, OR = 4.364; CI95% = 1.112-17.128). From these results it was concluded that moderate smokers were a factor identified to be associated with treatment default in multi drugs resistance tuberculosis patients.
