"Pengobatan talasemia berupa transfusi darah menyebabkan penumpukan besi di organ dan kerusakan sel. Pemberian deferoksamin sebagai kelator besi banyak menimbulkan efek samping dan mahal. Oleh karena itu, diperlukan pengobatan dengan bahan yang lebih aman dan terjangkau dengan memanfaatkan bahan alami yang memiliki efek kelasi besi. Ekstrak air daun Mangifera foetida memiliki efek kelasi terhadap feritin serum penderita talasemia, namun belum diteliti apakah ekstrak etanol daun M.foetida juga menunjukkan efek kelasi terhadap feritin. Studi eksperimental ini dilakukan pada serum pasien talasemia yang dibagi ke dalam tujuh perlakuan yaitu: serum, mangiferin, mangiferin ditambah serum, ekstrak etanol 0,5 mg dan 0,75 mg, ekstrak etanol 0,5 mg dan 0,75 mg ditambah serum, namun yang akan dianalisis hanya serum, mangiferin ditambah serum, ekstrak etanol 0,5 dan 0,75 mg ditambah serum. Nilai absorbansi diukur menggunakan spektrofotometer, λ = 280 nm. Uji one way anova menunjukkan ekstrak etanol M.foetida dosis 0,5 mg dan 0,75 mg memiliki efek kelasi dibandingkan kontrol negatif (p<0,001). Uji Post hoc menunjukkan ekstrak etanol M.foetida dosis 0,5 mg memiliki efek kelasi yang sama dengan dosis 0,75 mg (p=0,133). Ekstrak etanol daun M.foetida dosis 0,5 mg memiliki efek kelasi yang sama dengan mangiferin murni (p=0,52), sedangkan dosis 0,75 mg memiliki efek kelasi berbeda (p=0,001) yang mungkin disebabkan perbedaan dosis ekstrak etanol.

Treatment of thalassemia with blood transfusion causing iron accumulation in the organs and damaging cells. Chelating agent, deferoxamine causes side effects and expensive. Therefore, it?s needed a safer and cheaper treatment by utilizing natural ingredients which have chelating effect. Water extract of Mangifera foetida leaf has chelating effect on serum thalassemia patients, but there was no research the effects in the ethanol extract. The purpose of this study was to prove the effects of ethanol extract as a chelating agent. This study used an experimental study using seven serums of patients with thalassemia by ex vivo and devided into seven treatments: serum, mangiferin, mangiferin plus serum, etanol extract 0.5 mg and 0.75 mg, etanol extract 0.5 mg and 0.75 plus serum, however only four treatments will be analized: serum, mangiferin plus serum, etanol extract 0.5 mg and 0.75 mg plus serum. They were measured in a spectrophotometer with (λ)=280 nm. The result by One Way Anova statistical test showed that the ethanol extract of M. foetida leaf 0.5 mg and 0.75 mg has the chelating effect when it compared to negative control (p <0.001). Post hoc test showed that the ethanol extract 0.5 mg has the same chelating effect with ethanol extract 0,75 mg (p = 0.133). Ethanol extract 0.5 mg has the same effect of iron chelation with the mangiferin (p=0.52), while ethanol extract 0.75 mg has different effect (p=0.001). The differences of chelating effect maybe caused by the differences of extract dose."

"Ruang lingkup dan metodologi: Glukokortikoid telah lama digunakan sebagai antiiflamasi dan imunosuppresan. Penggunaan yang panjang dengan dosis tinggi menyebabkan efek samping yang cukup serius. Dewasa ini telah dikembangkan berbagai pembawa obat yang dapat membawa obat langsung ke target obat atau ke reseptornya. Dengan menginkorporasikan obat ke pembawa obat, contohnya liposom, efek samping sistemik dapat ditekan. Purwaningsih dkk telah berhasil membuat sediaan baru yakni Liposom-metilprednisolon palmitat (L-MPLP). Penelitian ini bertujuan untuk mempelajari efek farmakologi dari L-MPLP, terutama efek antiinflamasinya. Parameter yang dilihat adalah penurunan produksi interferon gamma pada kultur limfosit T setelah distimulasi dengan concanavalin A secara in vitro maupun in vivo. Hasil dan Kesimpulan : Terjadi penurunan kadar interferon gamma setelah pemberian L-MPLP secara in vivo pada dosis 2, 8 dan 16 mg/kgBB secara signifikan dibandingkan kontrol tanpa perlakuan sedangkan pemberian MPL tidak terjadi penurunan kadar interferon gamma. Pada kultur in vitro, pemberian L-MPLP maupun MPL pada kadar 5.10 -1, 5.10-2 dan 5.10-3 keduanya mampu menekan produksi interferon gamma, dimana penekanan oleh L-MPLP lebih baik dibanding MPL secara signifikan.

The Improving of Methylprednisolone Palmitate Potency After Incorporated With Liposome. An Antiinflammation Study In Culture Of Mice?s Splenocytes. Glucocorticoid has been used as an antiinflammatory and immuno-suppresive drug. Longterm utilisation at high dose of glucocorticoid is associated with serious side effects. In recent years, many attempts have been performed in searching the appropiate vehicles to deliver the drug directly into the target organ or the receptor. By incorporating the drug into its vehicle such as liposome, the systemic side effect can be minimized. Purwaningsih et al has successfully synthetized a novel preparation of liposome methylprednisolone palmitate (L-MPLP). The aim of the study was to learn the pharmacological effect of L-MPLP, especially on antiinflammatory effect of this novel preparation, compared with the standard methylprednisolone (MPL). The parameter was the potency of L-MPLP in reducing gamma-interferon production in T-lymphocyte culture after stimulation with concanavalin A in vitro as well as in vivo. Gamma-interferon was assayed by ELISA method. The reduction of gamma interferon, in vivo, after the administration of L-MPLP at the dose of 2,8 and 16 mg/kgBW respectively, showed significant difference than a control group, while MPL did not. The addition of both L-MPLP and MPL in in vitro culture at the concentration of 5.10-3, 5.10-2 and 5.10-1 mM have proved to suppress the gamma-interferon production, where the suppression of L-MPLP was more effective than MPL, significantly."

"scleroderma is an autoimmune disease characterized by organ fibrosis, resistant to standard treatment. It is suspected the addition of Physalis angulata Linn. (Ciplukan) extract as adjuvant therapy can improve the scleroderma skin fibrosis. The aim at this study is to evaluate the effect of ciplukan extract as adjuvant on scleroderma skin fibrosis in standard therapy, based on modified Rodnan skin scale (MRSS), inflammatory biomarkers, immunology and serum fibrosis. Methods: double-blind, randomized clinical trial was performed in scleroderma patients with stable disease at Cipto Mangunkusumo hospital and Hasan Sadikin hospital during November 2015−March 2017 who met the selection criteria and continued to receive standard therapy. The subjects were randomly allocated into two groups: the study group received the ciplukan extract 3 x 250 mg / day for 12 weeks and the placebo group. Examination of MRSS, ESR, P1NP, BAFF and sCD40L was performed every 4 weeks until the end of the study. Results: fifty-nine subjects completed the study. They consisted of 29 subjects of the treatment group and 30 of the placebo group, with an average age of 41 (SD 9) years, the proportion of women: male = 9 : 1. There was a significant improvement of skin fibrosis in the study group with a highly significant decrease in MRSS (35.9% VS 6.3%, p <0.001) and a relative decrease in P1NP levels (17.8% VS 0.7%, p = 0.002). No decrease in ESR, BAFF and sCD40L levels in both groups. There was a weak but significant positive correlation between MRSS with P1NP levels (r = 0.236, p = 0.036). Conclusion: Ciplukan extract with dose 3 x 250 mg for 12 weeks as adjuvant on scleroderma standard therapy alleviates skin fibrosis significantly based on MRSS and P1NP levels."

"Tujuan penelitian ini adalah untuk menilai proses penyembuhan luka dengan menggunakan ekstrak metanol daun Jatropha multifida L. berdasarkan mekanisme penurunan jumlah leukosit PMN dan peningkatan jumlah sel fibroblas.
Metode: bahan yang digunakan dalam penelitian ini adalah ekstrak metanol dari daun Jatropha multifida Subyek penelitian terdiri dari 36 ekor tikus putih jantan galur Spraque Dawlay umur 2 bulan dengan berat badan sekitar 150-200 g. Hewan coba dibagi menjadi 4 kelompok. Kelompok I (negatif kontrol merupakan kelompok hewan coba yang dilukai tanpa diobati; kelompok II (kontrol positif) merupakan kelompok hewan coba yang diobati dengan Bethasone-N; Kelompok III (kontrol pelarut) merupakan kelompok yang diobati dengan alkohol 70% sedangkan kelompok IV (kelompok perlakuan) merupakan kelompok yang diobati dengan meneteskan 10 mg ekstrak metanol daun Jatropha multifida. Setiap kelompok terdiri dari 3 ekor tikus yang masing-masing dibagi lagi menjadi kelompok waktu dekapitasi pada hari ke 3, 6, dan 13. Pada jaringan luka dibuat sediaan histologi dengan pewarnaan HE dan dilanjutkan dengan menghitung jumlah leukosit PMN dan fibroblas.
Pada penelitian ini memperlihatkan bahwa penurunan jumlah leukosit PMN pada kelompok perlakuan dengan ekstrak metanol daun Jatropha multifida relatif lebih baik dibandingkan dengan kontrol negatif, kontrol positif dan kontrol pelarut. Peningkatan jumlah fibroblas terjadi pada hari ke 6 dan 13 setelah perlakuan. Simpulan: ekstrak metanol daun Jatropha multifida dapat mengobati luka sayat lebih baik dibandingkan dengan kontrol negatif, kontrol positif dan kontrol pelarut.

Objective: The aim of this study was to evaluate the effects of methanol extract of Jatropha multifida leaves on the wound healing process and to investigate the wound healing activity based on reduced numbers of PMN (polymorpho nuclear) leukocytes and increased numbers of fibroblasts.
Method: methanol extract of dried leaves of Jatropha multifida was used in the wound healing activity studies. The study subjects were 36 white male Sprague Dawlay rats aged 2 months with 150-200 gram body weight. The subjects were divided into 4 groups and experimentally injured: Group I (negative control) underwent injury without subsequent treatment; group II (positive control) received topical treatment with Bethasone-N after injury; group III (solvent control) was treated with 70% methanol; group IV (treatment group) was treated with 10 mg methanol extract of Jatropha multifida Each group consisted of 3 rats, which were decapitated on days 3, 6, and 13 after the start of treatment. Histological preparation was stained with hematoxyline-eosin (HE) and was continuously examined by counting the numbers of PMN leukocytes and fibroblasts as indicators of wound healing on days 3, 6, and 13 of treatment.
The study showed lower numbers of PMN leukocytes in subjects treated with the extract of Jatropha multifidaas compared to the other groups. The numbers of fibroblasts were significantly higher on days 6 and 13 of treatment. In conclusion, the treatment of injuries with methanol extract of leaves from Jatropha multifida provided better results compared to the other groups in our study."
