Pelaksanaan Praktik Kerja Profesi di RSPAD Gatot Soebroto bertujuan untuk mendapatkan pengetahuan dan gambaran secara nyata terhadap tugas dan tanggung jawab yang dilakukan oleh apoteker di rumah sakit. Melalui praktik kerja profesi ini, calon apoteker dapat menerapkan ilmu yang telah dipelajari dengan melakukan kegiatan pendistribusian, penerimaan, penyimpanan, pengkajian dan pelayanan resep, pelayanan informasi obat, visite, serta pemantauan terapi obat. Praktik kerja profesi ini dilaksanakan selama enam minggu dengan tugas khusus yaitu ldquo;Pemantauan Terapi Obat pada Pasien Anak dengan LLA Leukemia Limfoblastik Akut di Ruang Perawatan Anak IKA 2 RSPAD Gatot Soebroto rdquo;. Tujuan dari tugas khusus ini adalah memonitoring, mengidentifikasi, mnganalisa, dan memberikan rekomendasi usulan terkait dengan penyelesaian masalah pemberian obat pada pasien serta mempelajari cara berkomunikasi dengan pasien dan tenaga kesehatan lain seperti dokter dan perawat dalam penyelesaian masalah terkait pemberian pengobatan dengan kondisi pasien.

ABSTRACT
Internship at Gatot Soebroto Central Army Hospital Period January ndash; February 2018 aims to get knowledges and real illustrations about apothecary rsquo;s duties and responsilities at hospital. Through this internship, pharmacists are able to apply the knowledges which have been learnt. The activities that phamacists can do including distribution, acceptance, storage, prescription rsquo;s assessment and service, giving drugs information, visite, also drugs treatment monitoring. This internship was conducted for six weeks with special assignment ldquo;Drugs Treatment Monitoring to ALL Acute Lymphoblastic Leukemia Patient at Gatot Soebroto Central Army Hospital rdquo;. The purpose of this special assignment are to monitor, identification, analyze, and give recommendations related to patient rsquo;s medication also to learn how to communicate with the patient and others healthcare personnel like doctors and nurses.

Abstrak :
Pemantauan Terapi Obat (PTO) adalah kegiatan pelayanan farmasi klinis yang dilakukan untuk memastikan terapi obat yang aman, efektif, dan rasional bagi pasien dengan meningkatkan efektifvitas terapi, meminimalkan risiko ROTD, dan meminimalkan biaya pengobatan. Tujuan dari tugas khusus ini adalah melakukan kegiatan PTO dengan metode SOAP dan menganalisis DRP’s dengan PCNE kepada pasien di Dokmil RSPAD Gatot Soebroto. Pasien yang mendapatkan pemantauan terapi obat adalah pasien khusus dengan penyakit komplikasi stroke iskemik, hipertensi, ITP, hiperlipidemia, dan hiperurisemia. Hasil yang didapatkan selama melakukan pemantauan terapi obat adalah ditemukan masalah terkait obat (DRP’s), yaitu interaksi obat yang bersifat major, moderate, dan kontraindikasi. Interaksi yang ditemukan berpotensi untuk menyebabkan kejadian obat yang merugikan. Rekomendasi dari permasalahan tersebut adalah dengan memberikan jeda pemberian obat untuk obat yang berinteraksi (sandimmune, amlodipine, clopidogrel) dan mengganti obat yang kontraindikasi (omeprazole) dengan obat golongan lain yang memiliki indikasi pengobatan yang sama (golongan H2). Kata ......Drug Therapy Monitoring (PTO) is a clinical pharmacy service activity carried out to ensure safety, effectivity, and rationality of drug therapy for patients by increasing the effectiveness of therapy, minimizing the risk of ROTD, and minimizing medical cost. The purpose of this study is to carry out PTO activities using the SOAP method and analyze DRP’s with PCNE method for patients in Dokmil station at Gatot Soebroto Hospital. Patient who received the drug therapy monitoring was the specified patient with complexity of diseases such as stroke ischemic, hypertension, ITP, hyperlipidemia, and hyperuricemia. The results obtained while monitoring were DRP’s where the drugs given are contraindicated or interact each other with major or moderate severity risk. Interactions found have the potential to cause adverse drug reactions. The recommendation for this problem is giving the interval for administration of interacting drugs (sandimmune, amlodipine, clopidogrel) and replace drugs that are contraindicated (omeprazole) with other category of drugs that have the same indicatioins for treatment (group H2).

Pemantauan Terapi Obat (PTO) merupakan kegiatan yang bertujuan untuk meningkatkan efektivitas terapi dan meminimalkan risiko Reaksi Obat yang Tidak Dikehendaki (ROTD). Indentifikasi DRP sangat penting untuk meningkatkan efektivitas terapi obat pada pasien melalui pengkajian lebih lanjut oleh apoteker mengenai profil pengobatan. Penelitian ini bertujuan untuk Pemantauan Terapi Obat (PTO) pada pasien di RSPAD Gatot Soebroto, mengkaji permasalahan terkait obat menggunakan metode klasifikasi Pharmaceutical Care Network Europe (PCNE), dan memberi rekomendasi dan rencana terkait PTO pada pasien. Pelaksanaan kegiatan PTO dilakukan secara prospektif melalui beberapa langkah yaitu pengumpulan data pasien melalui rekam medik, profil pengobatan dan catatam penggunaan obat, serta wawancara dengan tenaga kesehatan lain. Hasil analisis menunjukkan bahwa pasien An.R dengan riwayat elektrolit imbalance (sudah stabil), demam thypoid membaik, infark hepar (perbaikan), ditemukan adanya masalah terkait obat yaitu interaksi antara phenobarbital dan omeprazole dengan efek samping peningkatan kejadian kejang. Rekomendasi dan intervensi yang diberikan ialah penghentian atau penundaan terhadap penggunaan phenobarbital, dan tetap meneruskan penggunaan omeprazole. ......Therapeutic Drug Monitoring (TDM) is an activity that aims to increase the effectiveness of therapy and minimize the risk of Adverse Drug Reactions (ADR). Identification of DRP is very important to increase the effectiveness of drug therapy in patients through further assessment by pharmacists regarding treatment profiles. This study aims to Therapeutic Drug Monitoring (TDM) in patients at Gatot Soebroto Army Hospital, examine drug-related problems using the Pharmaceutical Care Network Europe (PCNE) classification method, and provide recommendations and plans regarding PTO in patients. The implementation of TDM activities is carried out prospectively through several steps, namely collecting patient data through medical records, treatment profiles and drug use records, as well as interviews with other health workers. The results of the analysis showed that patient An.R with a history of electrolyte imbalance (stable), typhoid fever improved, hepatic infarction (improved). A drug-related problem was found, namely the interaction between phenobarbital and omeprazole with the side effect of increasing the incidence of seizures. The recommendations and interventions given are stopping or delaying the use of phenobarbital, and continuing to use omeprazole.
