"PT XYZ adalah perusahaan yang bergerak dibidang pertambangan, dimana memiliki lokasi kerja yang tersebar di Indonesia dengan karakteristik yang bervariasi. Karakteristik tersebut yang dapat memunculkan risiko terjadinya kecelakaan kerja. Penelitian ini bertujuan untuk mendapatkan gambaran tentang sistem pertahanan dalam mencegah kecelakaan sesuai dengan kerangka pikir Swiss Cheese Model. Peneliti mengkaji kegagalan sistem pertahanan dengan menggunakan metode Human Factor Analysis and Classification System in Mining Industry (HFACS-MI). Hasil penelitian ini menyebutkan bahwa kasus cedera tangan dan jari masih terjadi dan kejadiannya cenderung berulang. Berdasarkan analisis yang dilakukan terhadap 33 kasus kecelakaan, permasalahan yang banyak ditemukan diantaranya adalah decisions errors, adverse mental states, coordination and communication, planned inappropriate operations, dan organization process. Dapat disimpulkan bahwa sistem pertahanan yang ada untuk mencegah kecelakaan masih belum optimal. Oleh karena itu, perlu dilakukan perbaikan sistem pertahanan, baik yang ditargetkan kepada individu maupun organisasi, agar risiko kecelakaan dapat dikendalikan.

XYZ is a company engaged in mining, which has several work sites in Indonesia with varied characteristics. This research aims to get an overview of the defense system in preventing accidents in accordance to the Swiss Cheese Model framework. This research examined the failure of the defense system by using the Human Factor Analysis and Classification System in Mining Industry (HFACS-MI) method. The result of this study mention that the case of hands and fingers injury still occurs. Based on the analysis of 33 cases of accident, the causes are mostly the decisions of errors, adverse mental states, coordination and communication, planned inappropriate operations, and organization process. It can be concluded that the defense system in XYZ company is still not optimal to prevent occupational accidents. Therefore, the defense system will need improvement, targeted to both individuals and organizations, so that the risk of accidents can be controlled."

"Penulis ingin membahas mengenai evaluasi pelaksanaan audit internal sistem manajemen keselamatan dan kesehatan kerja yang telah berlangsung di PT X selama tahun 2012-2014. Pelaksanaan audit internal ditinjau ulang menggunakan standar internasional, yaitu ISO 19011. Penelitian yang dilakukan ini bersifat deskriptif dengan metode penelitian kualitatif. Dalam penelitian ini melibatkan informan kunci dan informan sebagai sumber data primer melalui wawancara mendalam dan telaah dokumen pelaksanaan audit pada tahun 2012-2014.Hasil penelitian menunjukkan bahwa pada pelaksanaan audit internal sistem manajemen keselamatan dan kesehatan kerja di PT belum berjalan dengan baik dan maksimal. Poin utama dari lemahnya pelaksanaan audit internal tersebut adalah implementasi, pemantauan, tinjauan ulang dan perbaikan pada sistem manajemen audit yang dilaksanakan. Perbaikan pada tahap perencanaan dan implementasi menunjukkan poin utama yang harus segera diperbaiki guna meningkatkan kualitas sistem manajemen audit dan bisa mengevaluasi sistem manajemen yang berlangsung di perusahaan dengan lebih baik.

This research studied about Evaluation of Internal Audit Implementation of Occupational Health And Safety Management System on 2012-2014 at PT X. Internal audit implementation has been monitor by using international standard, ISO 19011. This research is descriptive with qualitative methodology. This research involve key informant and informant as primer source data through indeep interview and document review on audit implementation on 2012-2014.

This research result show that internal audit implementation of occupational health and safety management system in PT X is not run well and maximum yet. Key point on the weakness of internal audit are implementation, monitoring, review and improvement the audit management system that have been done. Improvement on the planning and implementation step show that it is key point to improve soon in order to increase audit management system quality and evaluate management system on that."

"Penelitian ini mebahas pengetahuan, sikap dan keterampilan pengemudi tentang System Smith di PT. X. Duri-Riau. Tujuan dibuatnya penelitian ini adalah untuk mengetahui berapa banyak pengemudi yang memenuhi kualifikasi sebagai pengemudi yang sesuai dengan System Smith . jenis penelitian yang digunakan adalah studi deskriptif analisis dengan rancangan penelitian observasional menggunakan alat ukur kuesioner dan cara ukur wawancara. Dari hasil penelitian dapat disimpulkan bahwa pengemudi yang memenuhi kualifikasi sebagai pengemudi yang sesuai dengan System Smith masih sedikit.

This study is talk about knowledge, attitude and driver’s skill on smith system at PT. X. Duri-Riau. Purpose of this study was to find out how many drivers that meet the classification of smith system. this study was using descriptive analytic with observational approach with questionnaire and interview. The result of this study can be concluded that driver who qualified with smith system still slighty."
