Abstrak :

Latar belakang: Karsinoma mukoepidermoid merupakan keganasan pada kelenjar liur yang paling sering ditemukan. Prognosis karsinoma mukoepidermoid berhubungan dengan derajat keganasannya. Cancer stem cell (CSC) diduga berperan dalam patogenesis karsinoma mukoepidermoid sehingga terjadi resisten terhadap berbagai terapi. CD44 merupakan salah satu penanda SC yang paling banyak pada kelenjar liur dan tampak meningkat pada karsinoma mukoepidermoid. Namun, peran prognostik CD44 pada keganasan masih menjadi perdebatan.

Metode: Penelitian menggunakan metode potong lintang. Sampel terdiri atas 34 kasus di Departemen Patologi Anatomik FKUI/RSCM tahun 2012 sampai 2017. Dilakukan pulasan CD44 dan perhitungan H-score dan presentasi setiap kasus. Hasil perhitungan dikelompokan menjadi ekspresi negatif/positif lemah dan positif kuat.

Hasil: Hasil penelitian menunjukkan bahwa ekspresi CD44 berhubungan secara signifikan dengan derajat keganasan (p=0,006). Ekspresi positif kuat ditemukan lebih banyak pada derajat keganasan rendah dan ekspresi negatif/positif lemah ditemukan lebih banyak pada derajat keganasan tinggi.

Kesimpulan: Ekspresi CD44 pada karsinoma mukoepidermoid berhubungan dengan derajat keganasan.

 


 

Background: Mucoepidermoid carcinoma is the most common malignancy in salivary gland. The prognosis correlates with its histological grading. Cancer stem cell (CSC) is predicted to have a role in pathogenesis of mucoepidermoid carcinoma, thus it make resistent to various therapy. CD44 is one of stem cell (SC) marker that expressed in salivary gland and seemed to be increased in mucopidermoid carcinoma. However, prognostic role of CD44 in malignancy still controversy.

Method: This is a cross sectionsl study. Samples consist of 34 cases from Anatomical Pathology Department, Faculty of Medicine Universitas Indonesia, Ciptomangunkusumo General Hospital in 2012 until 2017. CD44 staining was done and calculated wih H-score method. Then, the samples is catagorized into negative/weak expression and strong expresion.

Result: The result showed that CD44 expression associate significantly with histological grading (p=0,006). Strong expression is found more in low grade and negative/weak expresion is found more in high grade.

Conclusion: CD44 expression in mucoepidermoid carcinoma associates with histological grade.

ABSTRAK Preeklampsia merupakan sindrom sistemik yang terjadi pada 3-5 % kehamilan wanita yang disebabkan oleh gangguan faktor migrasi dan faktor seluler  yang berdampak pada gangguan diferensiasi dan invasi trofoblas yang penting dalam proses perkembangan plasenta dan mempertahankan kehamilan. Protein Cullin-1 merupakan salah satu kandidat protein yang berperan dalam proses mempertahankan kehamilan, perkembangan dan invasi trofoblas di dalam  plasenta. Hingga saaat ini belum ada penelitian yang menghubungkan ekspresi Cullin-1 pada pasien preeklampsia dengan waktu terminasi kehamilan. Oleh karena itu pada penelitian ini dilakukan analisis ekspresi Cullin-1 pasien preeklampsia dan hubungannya dengan waktu terminasi kehamilan. Sampel plasenta diambil dari pasien preeklampsia yang terdiri dari tiga kelompok usia kehamilan, kemudian dilakukan perwarnaan imunohistokimia untuk dilihat dinamika ekspresi dan distribusi Cullin-1 pada berbagai kelompok usia kehamilan dan hubungannya dengan waktu terminasi kehamilan. Cullin-1 terekspresi pada sinsitiotrofoblas dan  sitotrofoblas. Kadar Cullin-1 terendah didapatkan pada kelompok usia kehamilan very preterm, dan paling tinggi didapatkan di kelompok usia kehamilan moderate preterm. Terdapat perbedaan bermakna antara ekspresi optical density (OD) Cullin-1 dengan   waktu terminasi  kehamilan, dan terdapat perbedaan bermakna  (OD) Cullin-1 pasien preeklampsia usia kehamilan very preterm dengan usia kehamilan moderate preterm. Disimpulkan bahwa Cullin-1 terekspresi pada sinsitiotrofoblas dan sitotrofoblas dan berhubungan dengan waktu terminasi kehamilan.
ABSTRACT Preeclampsia is a systemic syndrome that occurs in 3-5% of female pregnancies caused by disorders of migration factors and cellular factors that have an impact on the disruption of trophoblast differentiation and invasion that is important in the process of developing the placenta and maintaining pregnancy. Protein Cullin-1 is one candidate protein that plays a role in the process of maintaining pregnancy, development and trophoblast invasion in the placenta. Until now there have been no studies linking the expression of Cullin-1 in preeclamptic patients with the timing of pregnancy termination. Therefore in this study an analysis of Cullin-1 expression in preeclamptic patients and their relationship to the timing of pregnancy termination was carried out. Placental samples were taken from preeclampsia patients consisting of three gestational age groups, then immunohistochemical staining was performed to see the dynamics of expression and distribution in each age group of pregnancy and to find out their relationship with  the timing of pregnancy termination. Cullin-1 was expressed in syncytiotrophoblasts and cytotrophoblasts. The lowest Cullin-1 level was obtained in the very preterm age group, and the highest was found in the moderate preterm gestational age group. There was a significant difference between Cullin-1 optical density (OD) expression and termination time of pregnancy, and there was a significant difference (OD) in Cullin-1 preeclamptic patients with very preterm gestational age with moderate preterm gestational age. It was concluded that Cullin-1 was expressed both in syncytiotrophoblasts and cytotrophoblasts and was associated with the timing of pregnancy termination.

Latar Belakang : Karsinoma nasofaring tidak berkeratin tidak berdiferensiasi masih menjadi masalah kesehatan di Indonesia berkaitan dengan tingginya angka kemoresistensi dan residif yang tinggi. Salah satu teori yang menjelaskan hal ini adalah terdapatnya sel punca kanker yaitu sel kanker yang memiliki kemampuan self-renewal dan menumbuhkan sendiri sel tumor. Jalur sel punca kanker mengakibatkan adaptasi genetik sehingga tumor menjadi resisten terhadap pengobatan. SOX2 adalah salah satu penanda gen sel punca yang berperan penting pada faktor transkripsi dalam proses self-renewal. Terdapat hubungan antara ekspresi SOX2 dengan respons terapi karsinoma nasofaring tidak berkeratin tidak berdiferensiasi.Bahan dan Metode : Penelitian ini menggunakan desain potong lintang. Sampel terdiri atas 41 kasus karsinoma nasofaring tidak berkeratin tidak berdiferensiasi di Departemen Patologi Anatomik FKUI/RSCM periode Januari 2014 hingga Desember 2016. Dilakukan pulasan imunohistokimia SOX2.Hasil : Ekspresi SOX2 positif pada 21 dari 41 51 kasus karsinoma nasofaring tidak berkeratin tidak berdiferensiasi. Sebanyak 11 dari 21 kasus diantaranya 52 memperlihatkan respons terapi pasca kemoradiasi yang baik p=0,636 . Dari 41 kasus terdapat 7 kasus residif, 2 kasus diantaranya 28,5 menunjukkan ekspresi SOX2 positif.Kesimpulan : Tidak ditemukan hubungan yang bermakna antara ekspresi SOX2 dengan respons terapi pasca kemoradiasi.
Background Nasopharyngeal carcinoma non keratinized non differentiated still become main health issue in Indonesia concerning the high rate of chemoresistance and recurrence. One of the theories was the cancer stem cell, tumor cells with self renewal and self duplicating capabilities. The cancer stem cell pathway caused genetic adaptation resulting resistance in therapy. The main function of SOX2 as transcription factor holds key in the self renewal process. SOX2 became one of the markers for cancer stem cells. The SOX2 expressions have associations with response therapy after chemoradiation in nasopharyngeal carcinoma.Materials and Methods This was a cross sectional study with 41 cases of nasopharyngeal carcinoma non keratinized non differentiated diagnosed from Anatomical Pathology Department of FKUI RSCM during January 2014 until December 2016. All of the cases stained with SOX2 antibody with immunohistochemical methods.Results SOX2 positive expression can be found in 21 from 41 cases 51 . There were 11 out of 21 cases 52 showed well response therapy. From 41 cases there were 7 recurrent cases, 2 of them 28.5 expressing SOX2 positive.Conclusions There were no statistically significant associations between SOX2 expression with response therapy after chemoradiation.

Latar belakang: Frekuensi tumor ovarium serosum ganas menempati urutan tertinggi dari seluruh keganasan ovarium di dunia barat 80-85 , sesuai dengan arsip Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia FKUI /Rumah Sakit Cipto Mangunkusumo RSCM selama 10 tahun 2004-2013 , sebanyak 200 kasus 21,4 dari seluruh keganasan ovarium. GLUT-1 dapat digunakan sebagai penanda perangai biologik tumor ovarium serosum. Tujuan penelitian ini membandingkan ekspresi GLUT-1 pada tumor ovarium serosum borderline dan ganas serta faktor risiko.Metode: Penelitian ini menggunakan metode potong lintang. Sampel terdiri atas 17 kasus untuk masing masing kelompok tumor ovarium serosum borderline dan ganas. Dilakukan pulasan GLUT-1 dengan penilaian berdasarkan intensitas dan jumlah sitoplasma dan/atau membran sel yang terpulas. Dilakukan penghitungan histoscore dan persentase setiap kasus dan dinilai ekspresi GLUT-1 berdasarkan titik potong kemudian dikelompokkan menjadi ekspresi rendah dan tinggi.Hasil: Pulasan GLUT-1 ekspresi rendah sama banyak dengan ekspresi tinggi. Background : The frequency of serous malignant tumors of ovary occupies the highest order of all ovarian malignancies in the western world 80-85 , in accordance with Department of Anatomical Pathology, Faculty of Medicine University of Indonesia / Cipto Mangunkusumo hospital datas, for 10 years 2004-2013 , as many as 200 cases 21.4 of all ovarian malignancies. GLUT-1 can be used as a marker in differentiating biological behaviour of serous ovarian tumor. The aim of the study was to compare expression of GLUT-1 in serous borderline and malignant tumours of the ovary. Methods : This was cross-sectional study. Sample consists of 17 cases for each group, serous borderline and malignant tumor of ovary, stained with GLUT-1 antibody. Quantification was based on the intensity and distribution of cytoplasm and/or cell membrane. The appraisal was done with estimating histoscore and percentage of each case. Calculation result was assessed by GLUT-1 expression, based on the point of intersection and then grouped into low and high expression.Result : The GLUT-1 low expression results are equal with high expression. Low grade expression found in majority cases of serous borderline ovarian tumors group. Groups of serous malignant ovarian tumors largely exhibit high expression. Latar belakang: Rabdomiosarkoma (RMS) adalah sarkoma jaringan lunak derajat tinggi dengan asal diferensiasi otot seran lintang. Tipe embrional RMSE dan alveolar RMSA lebih banyak ditemukan dibanding tipe pleomorfik dan tipe spindel dan umumnya terjadi pada usia anak dan remaja. Faktor prognosis RMS meliputi tipe histologik, usia, lokasi favourable/unfavourable, ukuran tumor dan adanya metastasis. Terapi kombinasi dengan reseksi komplit, kemoterapi dan radioterapi meningkatkan kesintasan tumor namun masih terdapat efek tidak diinginkan. Wilms' tumor 1 WT1 dapat berperan sebagai gen supresor tumor dan sebagai onkogen. WT1 merupakan antigen tumor pada beberapa tumor solid anak dan diekspresikan tinggi pada RMS. Penelitian ini dilakukan untuk membandingkan ekspresi WT1 pada RMSE dan RMSA dan hubungannya dengan faktor prognosis. Metode: Desain penelitian adalah potong lintang dan konsekutif. Penegakan diagnosis RMSE dan RMSA secara histopatologik dan imunohistokimia dengan marker MyoD dan atau desmin yang positif. Didapatkan 30 sampel RMSE dan 16 sampel RMSA yang memenuhi kriteria inklusi di Departemen Patologi Anatomik FKUI/RSCM selama tahun 2011-2017. Pulasan imunohistokimia WT1 pada RMSE dan RMSA dinilai secara semikuantitatif menggunakan H-score dan dibandingkan. Hubungan ekspresi WT1 dengan faktor prognosis dianalisis dengan menggunakan uji Pearson. Hasil: Terdapat perbedaan bermakna antara ekspresi WT1 pada RMSE dan RMSA (p=0,014). Rerata skor WT1 pada RMSE lebih tinggi dibandingkan RMSA (179,9 vs 149,5). Tidak terdapat hubungan antara ekspresi WT1 dengan faktor prognosis usia, ukuran tumor dan lokasi tumor (favourable dan unfavourable). Kesimpulan: Ekspresi WT1 pada RMSE lebih tinggi dibandingkan RMSA. Ekspresi WT1 tidak berhubungan secara bermakna dengan faktor prognosis.
Background: Rhabdomyosarcoma (RMS) is a high grade soft tissue sarcoma with differentiation toward skeletal muscle. The embryonal type (ERMS) and the alveolar type ARMS are more common than the pleomorphic type and spindle type (RMS). The pediatric population mostly affected. Prognostic factors in RMS including histologic type, age, favourable/unfavourable location, tumor size and metastasis. Complete resection, chemotherapy and radiation increase the survival rate, but undesired effects exist. Wilms' tumor 1 (WT1) can performs as tumor supresor gene or oncogene. WT1 is potential tumor antigen that highly expressed in RMS. The aim of this study is to demonstrate the expression of WT1 in ERMS and ARMS and analyze its correlation with prognostic factors. Method: This was retrospective, cross sectional study with the consecutive sampling method. All samples was diagnosed by histopathology and immunohistochemistry using the skeletal muscle marker MyoD1 and or desmin.30 cases of ERMS and 16 cases of RMSA are collected in Departement of Anatomical Pathology FKUI/RSCM from 2011 to 2017. The immunohistochemistry staining of WT1 in ERMS and ARMS was counted semiquantitatively using H-score. We use Pearson test to analyse the correlation of WT1expression and prognostic factors. Result: Our study show significant difference between WT1 expression in ERMS and ARMS (p=0.014). ERMS had higher mean score than ARMS (179,9 vs 149,5). There was no significant correlation between WT1 expression with prognostic factor : age, tumor size and location. Conclusion: The WT1 expression was higher in ERMS compared to ARMS. There was no significant correlation between expression of WT1 with prognostic factors.

Latar Belakang: Karsinoma sel hati (KSH) adalah lesi neoplastik ganas pada hati tersering. Transformasi keganasan sel hati normal menjadi KSH melibatkan berbagai faktor seperti inflamasi dan perubahan genetik yang menyebabkan KSH menjadi sangat heterogen pada tingkat histologik dan molekular. Perbedaan fenotipe yang dipengaruhi berbagai perubahan molekular menghasilkan berbagai derajat diferensiasi, subtipe histologik dan gambaran klinik yang berbeda dan sebagian berhubungan dengan prognosis pada KSH. Mutasi pada gen TP53 yang berfungsi menontrol proliferasi sel melalui perbaikan DNA, apoptosis, dan penuaan sel terbukti sebagai salah satu perubahan molekular tersering pada KSH dan sering dikaitkan dengan beberapa faktor risiko, derajat diferensiasi, subtipe histologik tertentu dan prognosis. Penelitian ini bertujuan menginvestigasi ekspresi p53 pada derajat diferensiasi, subtipe histologik dan stadium patologi tumor KSH. Bahan dan cara: Penelitian dilakukan di Departemen Patologi Anatomik FKUI/RSCM, Jakarta terhadap 41 kasus KSH yang diperoleh seara reseksi. Sampel kasus diklasifikasikan berdasarkan kelompok derajat diferensiasi (WHO), subtipe histologik dan stadium patologi tumor. Selanjutnya dilakukan pulasan imunohistokimia (IHK) protein 53 (p53) pada seluruh kasus dan dilakukan analisis untuk mengetahui ekspresi p53 pada variabel penelitian. Hasil: Ekspresi p53 ditemukan pada 35 kasus (85%). Berdasarkan derajat diferensiasi, ekspresi p53 ditemukan paling banyak pada derajat diferensiasi sedang dan buruk, yaitu 21 dan 14 kasus (91% dan 93%). Ekspresi p53 berdasarkan stadium patologi tumor ditemukan paling banyak pada pT1b dan pT2, yaitu 8 dan 14 kasus ( 88% dan 93%). Berdasarkan subtipe histologik, seluruh kasus macrotrabecular massive (MTM) menunjukkan ekspresi p53 (4 kasus, 100%), subtipe clear cell (CC) terpulas pada 15 kasus (93%), klasik (CL) ditemukan 16 kasus (88%) dan tidak ditemukan ekspresi p53 pada seluruh kasus steatohepatitic (SH). Terdapat perbedaan rerata bermakna ekspresi p53 pada kelompok baik dan sedang (p=0,011), baik dan buruk (p=0,015) dan tidak terdapat perbedaan rerata bermakna antara kelompok sedang dan buruk (p=0,339). Tidak ditemukan perbedaan rerata bermakna ekspresi p53 pada seluruh kelompok stadium patologi tumor (p=0,948) dan subtipe histologik (p=0,076). Kesimpulan: Terdapat perbedaan bermakna ekspresi p53 pada KSH kelompok diferensiasi baik dan sedang serta baik dan buruk. ......Background: Hepatocellular cell carcinoma (HCC) is the most common malignant neoplastic lesion of the liver. Malignant transformation of hepatocytes involves various factors such as inflammation and genetic causing HCC to be very heterogeneous at the histological and molecular level. Differences in phenotypes affected by various molecular changes produce different differentiation grade, histological subtype, clinical features and prognosis. TP53 as one of the most common molecular changes in HCC play an important role in cycle cell by controlling cell proliferation through DNA repair, apoptosis and cellular senescence, associates with several risk factors such as certain differentiation grade, histologic subtypes, and prognosis. This current study aimed to investigate p53 expression at HCC’s differentiation grade, tumor pathology stage and histologic subtype. Materials and methods: The study was conducted at the Department of Anatomical Pathology FKUI / RSCM, Jakarta on 41 cases of resected HCC. Case samples are classified based on groups of differentiation grade (WHO), histologic subtypes and tumour pathology stage. Furthermore immunohistochemical (IHC) staining of protein 53 (p53) carry out in all cases and an analysis statistic was performed to evaluated the expression of p53. Results: p53 expression was found in 35 cases (85%). Based on the differentiation grade, the expression of p53 was found mostly in the moderate and poor differentiation (91%, 21 cases and 93%, 14 cases). Based on tumour pathology stage, p53 expression was found mostly in pT1b and pT2, which were 8 and 14 cases (88% and 93%). Based on histologic subtypes, all macrotrabecullar massive (MTM) cases showed p53 expression (4 cases, 100%), clear cell (CC) subtypes were in 15 cases (93%), classic (CL) 16 cases (88%) and negative expression was found in all cases of steatohepatitic (SH). There were significant differences in mean expression of p53 in the well and moderate groups (p = 0.011), well and poor (p = 0.015) and there were no significant mean differences between the moderate and poor groups (p = 0.339). There were no significant mean differences in p53 expression in all groups of tumour pathology stages (p = 0.948) and histologic subtypes (p = 0.076). Conclusion: There is significant difference mean of p53 expression in well and moderate as well as well and poor differentiation.

Latar belakang: Kanker esofagus dilaporkan sebagai penyebab kematian keenam dari seluruh jenis kanker yang ada di seluruh dunia. Salah satu faktor risiko terjadinya keganasan esofagus, terutama adenokarsinoma esofagus adalah gastroesophageal reflux disease (GERD). Diagnosis dini GERD sangat penting karena esofagitis refluks kronis merupakan faktor risiko utama terjadinya Barret esofagus, yang merupakan lesi prekursor terjadinya adenokarsinoma esofagus.Tujuan penelitian ini adalah untuk mengetahui ekspresi p53 dan Ki67 pada esofagitis refluks derajat ringan, esofagitis refluks derajat berat dengan kriteria Esohisto dan Barret esofagus. Bahan dan cara: Penelitian ini merupakan penelitian deskriptif dengan desain penelitian potong lintang, dengan melakukan pulasan imunohistokimia p53 dan Ki67 pada 76 kasus sampel yang terbagi menjadi 30 kasus esofagitis refluks derajat ringan, 14 kasus esofagitis refluks derajat berat, dan 32 kasus Barret esofagus di Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia/Rumah Sakit Cipto mangunkusumo (FKUI/RSCM) tahun 2016-2018. Hasil: Ekspresi p53 positif pada 54 kasus sampel (71,1%), terbanyak pada Barret esofagus sebanyak 28 kasus (51,9%). Ekspresi Ki67 tinggi pada 46 kasus (60,5%), terbanyak pada esofagitis refluks derajat berat sebanyak 12 kasus (85,7%) Kesimpulan: Ekspresi p53 dan Ki67 pada esofagitis refluks derajat berrat dan Barret esofagus lebih tinggi dibanding dengan esofagitis refluks derajat ringan. ......Background: Esophageal cancer is reported as the sixth leading cause of death from all types of cancer worldwide. One of the risk factors for esophageal malignancy, especially esophageal adenocarcinoma is gastroesophageal reflux disease (GERD). Early diagnosis of GERD is very important because chronic reflux esophagitis is a major risk factor for Barrett esophagus, which is a precursor lesion to esophageal adenocarcinoma. The aim of this study was to determine p53 and Ki67 expression in mild reflux esophagitis, severe reflux esophagitis with the criteria of Esohisto and Barrett esophagus. Materials and methods: This study is a descriptive study with a cross-sectional design, by performing immunohistochemical results of p53 and Ki67 in 76 sample cases which were divided into 30 cases of mild reflux esophagitis, 14 cases of severe reflux esophagitis, and 32 cases of Barret esophagus in the Department Anatomical Pathology, Faculty of Medicine, University of Indonesia / Cipto Mangunkusumo Hospital (FKUI / RSCM) 2016-2018. Results: P53 positive expression in 54 sample cases (71.1%), most in Barret esophagus as many as 28 cases (51.9%). Ki67 expression was high in 46 cases (60.5%), most in severe reflux esophagitis as many as 12 cases (85.7%) Conclusion: The expression of p53 and Ki67 in severe reflux esophagitis and Barrett esophagus was higher than in mild reflux esophagitis.

Latar belakang: Frekuensi tumor ovarium serosum ganas menempati urutan tertinggi dari seluruh keganasan ovarium di dunia barat 80-85 , sesuai dengan arsip Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia FKUI /Rumah Sakit Cipto Mangunkusumo RSCM selama 10 tahun 2004-2013 , sebanyak 200 kasus 21,4 dari seluruh keganasan ovarium. GLUT-1 dapat digunakan sebagai penanda perangai biologik tumor ovarium serosum. Tujuan penelitian ini membandingkan ekspresi GLUT-1 pada tumor ovarium serosum borderline dan ganas serta faktor risiko.Metode: Penelitian ini menggunakan metode potong lintang. Sampel terdiri atas 17 kasus untuk masing masing kelompok tumor ovarium serosum borderline dan ganas. Dilakukan pulasan GLUT-1 dengan penilaian berdasarkan intensitas dan jumlah sitoplasma dan/atau membran sel yang terpulas. Dilakukan penghitungan histoscore dan persentase setiap kasus dan dinilai ekspresi GLUT-1 berdasarkan titik potong kemudian dikelompokkan menjadi ekspresi rendah dan tinggi.Hasil: Pulasan GLUT-1 ekspresi rendah sama banyak dengan ekspresi tinggi. Background The frequency of serous malignant tumors of ovary occupies the highest order of all ovarian malignancies in the western world 80 85 , in accordance with Department of Anatomical Pathology, Faculty of Medicine University of Indonesia Cipto Mangunkusumo hospital datas, for 10 years 2004 2013 , as many as 200 cases 21.4 of all ovarian malignancies. GLUT 1 can be used as a marker in differentiating biological behaviour of serous ovarian tumor. The aim of the study was to compare expression of GLUT 1 in serous borderline and malignant tumours of the ovary. Methods This was cross sectional study. Sample consists of 17 cases for each group, serous borderline and malignant tumor of ovary, stained with GLUT 1 antibody. Quantification was based on the intensity and distribution of cytoplasm and or cell membrane. The appraisal was done with estimating histoscore and percentage of each case. Calculation result was assessed by GLUT 1 expression, based on the point of intersection and then grouped into low and high expression. Latar belakang: Karsinoma urotelial merupakan keganasan kandung kemih tersering pada laki-laki. Faktor risikonya adalah merokok, pajanan bahan kimia, radiasi, infeksi Schistosoma hematobium. Mutasi p53 merupakan mutasi tersering pada karsinoma urotelial kandung kemih yang menyebabkan akumulasi protein p53 di inti dan terlihat dengan imunohistokimia. Tujuan penelitian adalah untuk melihat perbedaan ekspresi p53 pada karsinoma urotelial kandung kemih derajat rendah dan derajat tinggi serta hubungan ekspresi p53 dengan: "stadium tumor. Bahan dan cara: Penelitian menggunakan desain potong lintang. Sampel terdiri atas 47 kasus yang terbagi menjadi 22 kasus karsinoma urotelial derajat rendah dan 25 kasus karsinoma urotelial derajat tinggi di Departemen Patologi Anatomik Fakultas Kedokteran Universitas Indonesia/Rumah Sakit Cipto Mangunkusumo (FKUI/RSCM) tahun 2009-2017. Dilakukan pulasan imunohistokimia p53 dengan menggunakan cut off positif ≥ 20% berdasarkan penelitian Thakur et al, Ong et al, dan Saint et al. Hasil: Ekspresi p53 positif pada 33 sampel (70,21%), terbanyak pada karsinoma urotelial derajat tinggi 20 kasus (80%), sedangkan pada karsinoma urotelial derajat rendah terdapat 13 kasus (59,1%). Sebanyak 22 kasus (68,8%) Nonmuscle invasive bladder cancer dan 11 kasus (73,3%) Muscle invasive bladder cancer menunjukkan ekspresi positif. Ekspresi p53 cenderung lebih banyak ditemukan pada karsinoma urotelial derajat tinggi dan stadium tinggi. Kesimpulan: Tidak ada perbedaan ekspresi p53 pada karsinoma urotelial kandung kemih derajat rendah dan derajat tinggi. Tidak ada hubungan antara ekspresi p53 dengan stadium tumor. Risk factors for bladder cancer include smoking, exposure to chemicals, radiation and schistosoma hematobium infection. P53 is a tumor suppressor gene that is involved in the cell cycle and plays a role in the occurrence of apoptosis in response to DNA damage. P53 gene mutation is one of the most common genetic changes in urothelial bladder carcinoma. The p53 gene mutation will cause accumulation of p53 protein in the nuclei which can be detected through immunohistochemical examination. The aim of this study is to see differences of p53 expression in low grade and high grade urothelial carcinoma and to see the association of p53 expression with tumor stage. Material and method : This study uses a cross sectional study design. The sample consisted of 47 cases of urothelial bladder carcinoma divided into 22 cases of low grade urotelial carcinoma and 25 cases of high grade urotelial carcinoma originating from the archives of the Anatomical Pathology Department Faculty Medicine of Universitas Indonesia/Cipto Mangunkusumo Hospital (FKUI/ RSCM) in 2009-2017. The study was carried out by p53 immunohistochemical examination and assessment of p53 expression using a percentage with a positive cut off value of ≥ 20%. Result : This study obtained positive p53 expression in 33 samples from 47 samples studied (70,21%). Most are found in high grade urothelial carcinoma as many as 20 cases (80%). Whereas in low grade urothelial carcinoma there are 13 cases (59,1%) with positive p53 expression. As many as 22 cases (68,8%) of Non muscle invasive bladder cancer (NMIBC) and 11 cases (73,3%) of Muscle invasive bladder cancer (MIBC) showed positive p53 expression. There was no difference between p53 expression in low grade and high grade bladder urothelial carcinoma (p=0,118). This study also showed no association between p53 expression with tumor stage (p=1,000). Conclusion : P53 expression was not significantly different with tumor grade. P53 expression was not significantly associated with the tumor stage.

Latar belakang: Seminoma testis yang gagal sembuh dengan penatalaksanaan konvensional memiliki prognosis buruk. Beberapa kejadian rekuren setelah dilakukan kemoterapi juga ditemukan. Programmed Death Ligand-1 (PD-L1) terekspresi pada berbagai keganasan dan tumor infiltrating lymphocytes(TILs) serta telah diketahui perannya sebagai faktor prognostik. Penelitian ini dilakukan untuk mengetahui peran ekspresi PD-L1 pada seminoma testisdalam menentukanoverall survival(OS) danprogression free survival(PFS).

Bahan dan cara: Penelitian ini merupakan penelitiankohort retrospektif dengan desain analisis kesintasan. Data klinis diambil dari rekam medis RSUPN Cipto Mangunkusumo sejak Januari 2011-Desember 2016 yang diobservasi selama 2 tahun. Data histopatologik diambil dari Departemen Patologi Anatomik RSUPN Cipto Mangunkusumoyang kemudian dilakukan pulasan imunohistokimia PD-L1.

Hasil: Terdapat hubungan yang bermakna antara ekspresi PD-L1 pada sel tumor dengan 2-yearOS (p=0,023) dan PFS (p=0,002) padaseminoma testis.Tidak terdapat hubungan yang bermakna antara ekspresi PD-L1 pada TILs dengan 2-year OS (p=0,235) dan PFS (p=0,111). Terdapat hubungan bermakna antara ekspresi PD-L1 pada sel tumor dan ekspresi PD-L1 pada TILs dengan PFS (p=0,019). Terdapat hubungan yang bermakna antara stadiumdengan 2-yearOS (p=0,010)dan PFS (p=0,000)serta metastasis kelenjar getah beningpada 2-yearOS (p=0,010)dan PFS (p=0,000).

Kesimpulan: Ekspresi PD-L1 pada sel tumor seminoma testis berhubungan dengan OS dan PFS, tetapi hubungan tersebut tidak ditemukan pada TILs.
ABSTRACT
Background: The prognosis of testicular seminoma who failed to be cured with conventional therapy is poor. Several recurrent events after chemotherapy were also found. PD-L1 is expressed in various types of malignancy and tumor infiltrating lymphocytes (TILs) and its role is known as a prognostic factor. This study was conducted to determine the role of PD-L1 expressionseminomain determining overall survival (OS) and progression free survival(PFS).

Materials and Methods: Thisis aretrospective cohort study with survival analysis. Clinical datawere obtained from medical record in RSUPN Cipto Mangunkusumo since January 2011-December 2016 and observed for 2 years. Histopathological datawere obtained from Anatomical Pathology Department and PD-L1 immunohistochemistry staining were performed.

Results: Asignificant correlation between PD-L1 expression in tumor cells with 2-year OS (p=0,023) and PFS(p=0,002) intesticular seminoma was found. Nosignificant correlation between PD-L1 expression in TILs with 2-year OS (p=0,235)and PFS (p=0,111).We also found significant correlations between PD-L1 expression in tumor cells and TILs with PFS (p=0,019). A significant correlation between stage with 2-year OS (p=0,010)and PFS (p=0,000) and lymph node metastases with 2-year OS (p=0,010) and PFS (p=0,000)

Conclusion: PD-L1 expression in tumor cells testicular seminoma were significantly correlated with OS and PFS. There were no statistically significant associations between PD-L1 expression in TILs with OS and PFS.
