Tujuan penelitian ini untuk mengetahui pengaruh terapi kognitif terhadap perubahan harga diri dan kondisi depresi pasien gagal ginjal kronik (GGK) di ruang Haemodialisa RSUP Fatmawati Jakarta. Metode penelitian adalah quasi experiment dengan desain pre-post test design with control group. Data diambil sebelum dan sesudah pemberian intervensi terapi kognitif pada pasien GGK yang mengalami harga diri rendah dan kondisi depresi di kelompok intervensi. Sampel penelitian diperoleh secara consequtive sampling yang berjumlah 56 responden, terdiri dari 28 responden untuk kelompok intervensi dan 28 responden untuk kelompok kontrol. Instrumen penelitian untuk mengetahui tingkat harga diri dan kondisi depresi menggunakan kuesioner modifikasi dari Test Skrining Depresi Beck (Beck Depresion Inventory/BDI) yang berjumlah 25 pertanyaan. Hasil penelitian didapatkan bahwa tingkat harga diri meningkat lebih bermakna dan kondisi depresi menurun lebih bermakna pada kelompok pasien gagal ginjal kronik yang mendapatkan terapi kognitif dibanding kelompok pasien gagal ginjal kronik yang tidak mendapatkan terapi kognitif (p value = 0,000; α = 0,005). Terapi kognitif bila dilaksanakan secara konsisten oleh pasien secara mandiri berpeluang untuk meningkatkan harga diri sebesar 18,9% dan diperkirakan mampu meningkatkan nilai harga diri sebesar 20,43 poin dan juga berpeluang untuk menurunkan depresi sebesar 31,2% dan diperkirakan mampu menurunkan nilai kondisi depresi sebesar 6,29 poin. Ruang Haemodialisa RSUP Fatmawati dapat mengupayakan peningkatan harga diri dan penurunan kondisi depresi pasien GGK yang menjalani terapi haemodialisa melalui pemberian intervensi keperawatan spesialistik agar dapat memberikan asuhan keperawatan yang komprehensif, paripurna dan bermutu serta terintegrasi dalam pelayanan kesehatannya.
The aim of this study was to investigate the influence of cognitive therapy to self esteem degree and depression condition of the patient with chronic renal failure (CRF) at Haemodialysis unit of Fatmawati Hospital, Jakarta. The research method was quasi experimental pre-post test with control group. The data was gathered at before and after giving the cognitive therapy to the patient with CRF who had low self esteem and was depressed. The amount of samples were 56 respondents which 2 were 28 respondents of intervention group and 28 respondents of control group and determined by consequtive sampling method. The research instrument was a questionnaire consisted 25 questions of Likert scale statement which were modified from Beck Depresion Inventory (BDI). The result of this study showed that the level of self esteem in the intervention group was increased higher significantly and also the depression condition was decreased lower significantly than the control group (p value = 0,000; α = 0,005). The conclusion is that the level of self esteem will be higher and depression condition will be decreased whether the patient does the cognitive therapy by himself routinely (consistently) in his activities daily life with the probability score is 18,9% for self esteem degree and 31,2% for depression condition and there will be enhanced 20,43 point for self esteem degree and 6,29 point for depression condition. Fatmawati Hospital especially Haemodialysis unit could increase the patient self esteem and decrease the patient depression condition with delivering specialistic nursing intervention to their patient so that they could achieve the comprehensive, holistic and qualified nursing care which is integrated in their health services for their patient. |
