Skripsi ini membahas mengenai program observasi keselamatan sebagai cara mengubah perilaku tidak aman sehingga secara dini perilaku tindakan tidak aman sebelum cidera terjadi. Evaluasi terhadap program observasi keselamatan perlu dilakukan untuk mengetahui seberapa efektif program berjalan dalam upaya peningkatan perilaku kerja aman di PT Trakindo Utama cabang Jakarta tahun 2009. Penelitian ini adalah studi evaluasi deskriptif analitik dengan menggunakan survey berupa penyebaran kuesioner dan data sekunder perusahaan. Populasi penelitian adalah adalah seluruh pekerja pada direct service (pekerja di area workshop) sebanyak 85 diwakili 61 orang responden. Simpulan hasil penelitian bahwa ownership yang baik masih kurang berkontribusi pada pelaksanaan program observasi keselamatan dan berdasarkan tipe dalam penerapannya, PTTU cabang Jakarta berada pada tipe participative yaitu partisipasi sebatas upaya pencapaian tujuan target pengumpulan pelaporan kartu. Kontribusi terbesar dalam pelaksanaannya adalah supervisor dan foreman sedangkan partisipasi karyawan masih kurang. Karena masih belum efektifnya pelaksanaan program observasi keselamatan, maka disarankan pihak perusahaan melakukan evaluasi kinerja sesuai dengan penetapan tujuan. Hasil evaluasi perlu dianalisis sehingga dapat mengidentifikasi masalah. Selain itu, motivasi pekerja dan manajemen harus ditingkatkan sehingga upaya perubahan perilaku bekerja aman dapat tercapai dan menurunnya angka kecelakaan kerja akibat perilaku tidak aman.
