ABSTRAK Tuberkulosis (TB) menjadi salah satu masalah kesehatan yang paling serius,masalah kesehatan di dunia dan penyebab utama kematian di negara berkembang.Indonesia merupakan negara dengan pasien TB terbanyak ke-3 di dunia setelah Indiadan Cina. Hasil Survei Kesehatan Rumah Tangga (SKRT) menunjukkan bahwapenyakit TB penyebab kematian nomor 5 setelah penyakit kardiovaskular danpenyakit saluran napas pada semua kelompok usia dan nomor 1 dari golonganpenyakit infeksi. Tuberkulosis disebabkan oleh Mycobacterium tuberculosis danmenyerang organ pernapasan walaupun dapat mengenai organ lain. Sejak meluaspenyakit human immunodeficiency virus (HIV) dan pertambahan kasus TB kebal obat(MDR-TB), masalah TB yang sebelumnya telah teratasi kembali mencuat, sehinggapengawasan dan pemberantasan penyakit ini menjadi bertambah rumit.Angka kesalahan (error rate) pemeriksaan laboratorium pada Mycobacteriumtuberculosis sangat mempengaruhi penemuan penderita dan pengobatan penyakittuberkulosis. Error rate pemeriksaan laboratorium yang tinggi berarti kemampuanmendeteksi kurang, pemeriksaan belum dapat dipercaya hasil pelaporannya, akanberdampak masalah penyakit tuberkulosis di masyarakat tidak terdeteksi dengan baikdan benar, obat anti tuberkulosis tidak berhasil guna penyembuhan. Sehinggapenularan penyakit TBC tidak dapat ditanggulangi dengan baik di masyarakat.Berdasarkan hasil laporan cross check pemeriksaan BTA triwulan I sampai IVdi Kota Tangerang tahun 2012 terjadi error rate 5,26% sampai 36,36%. Nilai errorrate yang ditoleransi dari Kementerian Kesehatan maksimal 5%.Tujuan dari penelitian ini adalah mengetahui kinerja petugas pemeriksa BTA dandeterminannya. Penelitian ini merupakan penelitian observasional dengan desaincross sectional. Sampel pada penelitian ini 26 petugas laboratorium puskesmas yangmelakukan pemeriksaan BTA. Dari 26 petugas pemeriksa BTA terdapat 15 petugasdengan error rate rendah dan 11 petugas dengan error rate tinggi. Pengumpulan datadilakukan menggunakan kuisioner dan wawancara. Dari hasil penelitian diketahuifaktor yang berhubungan dengan error rate BTA adalah sistem dan beban kerja.Hasil analisis multivariat menunjukkan variabel sistem (nilai p=0,030, OR=16,0)berhubungan bermakna dan faktor dominan berhubungan dengan kinerja petugaspemeriksa BTA. Disarankan kepada petugas pemeriksa BTA sistem yang ada (pmi,menerapkan SOP) dan Dinas Kesehatan memberi pelatihan dan bimbingan intensif,serta melengkapi kebutuhan laboratorium terutama untuk pemeriksaan BTA. Laboratory error rate is high means the abilityto detect less, yet reliable inspection result reporting, will impact the problem oftuberculosis in the community are not detected properly, anti tuberculosis failed tocure. Suggested to the examiners BTA existingsystem (pmi, implement the SOP) and the Department of Health provide intensivetraining ang guidance, as well as complete laboratory requitments, especially forsmearexamination. |
