[ABSTRAK
Kelompok swabantu merupakan salah satu bentuk intervensi keperawatan dalamupaya pengendalian hipertensi pada agregat lansia berbasis pemberdayaanmasyarakat. Integrasi teori manajemen, model community as partner, familycenter nursing, dan self care digunakan sebagai kerangka kerja praktik residenkeperawatan komunitas melalui pendekatan pengelolaan pelayanan keperawatan,asuhan keperawatan komunitas dan asuhan keperawatan keluarga. Tujuankegiatan kelompok swabantu adalah meningkatkan perawatan hipertensi padalansia secara mandiri di masyarakat. Hasil kegiatan kelompok swabantumenunjukkan terjadinya peningkatan pengetahuan anggota sebesar 29,4% danpenurunan tekanan darah rata-rata 10-20 mmHg. Kelompok swabantu disarankandibentuk sebagai upaya kesehatan berbasis masyarakat.

Self-help group is a form of nursing intervention as hypertension control in theelderly which based on community empowerment. Integration of managementtheory and three models: community as a partner, family center nursing and selfcare were applied as framework for community health nursing specialist practicewith management health service, community nursing and family nursing careapproaches. The practice aimed to improve hypertension care independently inthe community. It showed an increased knowledge of group members to 29,4%and reduced blood pressure 10-20 mmHg in average.
