[Tujuan : Untuk mengetahui perubahan ketebalan makula sebelum dan sesudah operasifakoemulsifikasi pada pasien dengan NPDR ringan-sedang dengan dan tanpa profilaksistetes mata nepafenac 0.1%.Metode : Penelitian ini adalah uji klinis acak tidak tersamar. Sebanyak 36 subyek yangmemenuhi kriteria inklusi menjalani fakoemulsifikasi. Secara acak 18 subyekmendapatkan tetes mata nepafenac 0,1 % dan sisanya mendapatkan tetes mata plaseboyang dipakai 3 hari pre fakoemulsifikasi hingga 14 hari pasca fakoemulsifikasi.Ketebalan fovea dan volume makula diukur dengan SD-OCT pre-fakoemulsifikasi danminggu ke-4 pasca fakoemulsifikasi. Dilakukan juga pengukuran tajam penglihatandengan koreksi (TPDK) dan tingkat peradangan di bilik mata depan.Hasil : Didapatkan peningkatan ketebalan fovea pada kelompok plasebo 4 minggu pascafakoemulsifikasi secara statistik berbeda bermakna (uji-t berpasangan, p=0,022). Padakelompok nepafenac tidak didapatkan perubahan ketebalan fovea yang bermakna 4minggu pasca fakoemulsifikasi (uji-t berpasangan, p = 0,538). Didapatkan 1 pasien padakelompok plasebo mengalami CME. Pada penilaian perubahan volume makula 4 minggupasca fakoemulsifikasi, terdapat peningkatan volume makula pada ke-2 kelompok yangbermakna secara statitistik (uji-t berpasangan, p<0,05) tetapi antar kedua kelompok tidakbermakna (uji-t tidak berpasangan, p= 0,621). Secara klinis presentase tingkatperadangan di bilik mata depan pada kelompok plasebo lebih besar dibandingkankelompok nepafenac pada hari-1 pasca fakoemulsifikasi (38,9% : 5,6%) walaupun secarastatistik antara kedua kelompok tidak berbeda bermakna (uji Kolmogorov-Smirnov,p=0,27). Kelompok nepafenac secara klinis mendapatkan TPDK yang lebih baikdibandingkan kelompok placebo 4 minggu pasca fakoemulsifikasi walaupun secarastatistik tidak berbeda bermakna (uji t tidak berpasangan, p=0,991).Kesimpulan : Pemberian tetes mata nepafanac 0,1% dapat mencegah penebalan foveasecara bermakna (klinis dan statistik) 4 minggu pasca fakoemulsifikasi pada penderitaNPDR ringan-sedang. Secara klinis pemberian tetes mata nepafenac 0,1% dapatmengurangi resiko peradangan di bilik mata depan, resiko terjadinya CME, danpenurunan tajam penglihatan meskipun tidak mencapai kemaknaan secara statistik biladibandingkan dengan plasebo., Aim : To evaluate the effect of prophylactic nepafenac eye drops on macular thicknesschanges after phacoemulsification surgery in mild to moderate NPDR patients.Method : This study is an open label randomized clinical trial. Thirty-six subjects whomet the inclusion criteria underwent phacoemulsification. One group (18 subjects) weregiven nepafenac 0,1% eye drops and the rest were given placebo; both products wereused 3 days before phacoemulsification until 14 days after phacoemulsification. Fovealthickness and total macular volume were measured by SD-OCT before surgery and thefourth week after phacoemulsification. Best corrected visual acuity (BCVA) and degreeof inflammation in the anterior chamber were also being assessed.Result : There was a statistically significant increase foveal thickness in the placebogroup 4 weeks after phacoemulsification (paired t-test, p=0,022). In the nepafenac groupthere was no significant changes in the foveal thickness 4 weeks afterphacoemulsification (paired t-test, p = 0,538). One patient in the placebo group hadCME. There was an increased in total macular volume, which were statisticallysignificant in both groups (paired t-test, p<0,05) although not significantly differentbetween the two groups (unpaired t-test, p= 0,621). Clinically, percentage degree ofinflammation in anterior chamber in placebo group was higher than nepafenac group(38,9% : 5,6%) but not significantly different between 2 groups (Kolmogorov-Smirnovtest, p=0,27). Nepafenac group achieved clinically better BCVA than the placebo group 4weeks after phacoemulsification, although statistically there was no significant differencebetween two groups (unpaired t-test, p=0,991).Conclusion : Nepafenac 0,1% eye drops could prevent foveal thickening 4 weeks afterphacoemulsification in mild to moderate NPDR patients. Clinically, nepafenac 0,1% eyedrops could decrease the risk of inflammation in the anterior chamber, risk of CME, andvision deterioration although did not reach statistically significant.] |
