Pengaruh polimorfisme NRAMP1 dengan tuberkulosis paru di Kab Timor Tengah Selatan Nusa Tenggara Timur Indonesia = Association of NRAM1 polymorphism with lung tuberculosis in Timor Tengah Selatan East Nusa Tenggara Indonesia

Sebagai pengatur level kation sitoplasmik, terutama besi, di dalam makrofag, natural resistance associated-macrophage protein 1 (NRAMP1) diduga memiliki hubungan erat dengan kerentanan terhadap infeksi Mycobacterium tuberculosis (MTB). Zat besi sangat penting dan dibutuhkan untuk menghasilkan oksigen dan nitrogen reaktif, sementara MTB juga memerlukan zat besi, sehingga terjadilah kompetisi. Penelitian ini bertujuan untuk mendapatkan gambaran polimorfisme NRAMP1 pada pasien tuberkulosis di Timor Tengah Selatan, Nusa Tenggara Timur. Desain studi ini adalah kasus kontrol. Kasus merupakan pasien yang sudah terbukti pernah memiliki penyakit tuberkulosis. Sedangkan kontrol adalah keluarga yang tinggal bersama dengan kasus dan tidak terdiagnosis/tidak memiliki keluhan tuberkulosis. Sampel darah diambil untuk pemeriksaan polimorfisme D543N NRAMP1 dan asosiasinya dengan tuberkulosis. Sebanyak 99 pasien dengan 86 kontrol berpartisipasi dalam penelitian ini. Terdapat perbedaan yang signifikan pada pemeriksaan genotipe NRAMP1 pasien tuberkulosis dan kontrol sehat (p = 0,002), namun belum memenuhi persamaan Hardy-Weinberg. Penelitian ini memperlihatkan adanya hubungan antara polimorfisme NRAMP1 dengan kerentanan terhadap tuberkulosis. Berbeda dengan beberapa studi terdahulu yang dilakukan di Jawa dan Sulawesi, yang tidak menunjukkan adanya asosiasi ini. Penambahan jumlah subjek akan meningkatkan kekuatan penelitian dan meningkatkan kemungkinan terpenuhinya persamaan Hardy-Weinberg. Dari penelitian ini disimpulkan terdapat perbedaan proporsi polimorfisme NRAMP1 yang signifikan, tetapi perbedaan ini belum memenuhi persamaan Hardy-Weinberg. As the regulator of cationic level in the cytoplasm of macrophage, especially iron, natural resistance associated-macrophage protein 1 (NRAMP1) is suspected to have close relation with the susceptibility to Mycobacterium tuberculosis (MTB) infection. Iron is very important for producing reactive oxygen and nitrogen, but MTB also needs it for its metabolism. The aim of the research is mapping the distribution of NRAMP1 polymorphism in tuberculosis patients from East Nusa Tenggara. This is a case-control study. Cases were patients who have been diagnosed with tuberculosis. Control were they who living with tuberculosis patients but did not develop any signs of tuberculosis. Blood sample were taken for D543N NRAMP1 polymorphism examination and the association with tuberculosis. The study involved 99 pulmonary tuberculosis patients and 86 healthy controls. We observed a significant difference in the distribution of NRAMP1 genotypes frequencies between tuberculosis patients and healthy controls (p = 0,002), so it showed association between NRAMP1 polymorphism and the susceptibility to tuberculosis, but it didn?t meet the Hardy-Weinberg Equilibrium. Increasing the number of subjects will raise the possibility to meet Hardy-Weinberg equilibrium. We conclude there was a significant difference in the proportion of NRAMP1 polymorphism, however this has not yet fulfilled the Hardy-Weinberg equilibrium. |
