[ABSTRAK Praktek Kerja Profesi Apoteker di Rumah Sakit Umum Pusat Fatmawati bertujuanuntuk mengetahui gambaran umum RSUP Fatmawati, mengetahui struktur danpembagian kerja di Instalasi Farmasi RSUP Fatmawati, dan mengetahui peran dantanggung jawab apoteker dalam Peran Lintas Farmasi. Tujuan khusus berjudulPemantauan Terapi Obat Pasien Ulkus DM dengan Gagal Ginjal Kronis di LantaiV Utara Teratai RSUP Fatmawati agar mahasiswa calon Apoteker dapatmengidentifikasi masalah yang berkaitan dengan penggunaan obat pada pasienulkus DM dengan gagal ginjal kronik Lantai V Utara Teratai dan memberikanrekomendasi intervensi untuk masalah yang berkaitan dengan penggunaan obatyang dapat terjadi.
