Abstract
Praktek Kerja Profesi Apoteker di Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto Ditkesad bertujuan agar calon Apoteker memahami manajemen perbekalan farmasi dan pelayanan farmasi klinik di RSPAD Gatot Soebroto Ditkesad. Tugas khusus yang diberikan dengan judul Laporan Kasus Pasien CKD Stage 5 di Unit Perawatan Umum Lantai 4 Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto Ditkesad Tanggal 12 Mei - 16 Mei 2014 bertujuan untuk mengetahui DRP (Drug Related Problem) yang terjadi berdasarkan terapi yang diberikan kepada pasien dan mengetahui peran Apoteker dalam mencegah keparahan CKD lebih lanjut.

A special assignment that is given by title case report Patients CKD Stage 5 in General Care Unit 4th floor Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad date May 12th - May 16th 2014 have a purpose to know the DRP (Drug Related Problem) that occur based on the therapy given to patients and to know the role of the pharmacist in preventing further CKD severity.
