| [Latar Belakang: Limfoma non-Hodgkin sel B jenis sel besar difus (DLBCL; Diffuse Large B-Cell Lymphoma) adalah jenis limfoma non-Hodgkin yang paling banyak ditemukan dan merupakan suatu entitas yang sangat heterogen secara klinik maupun morfologik. Para ahli mengelompokkan DLBCL ke dalam dua subtipe yaitu subtipe Germinal Center B Cell-Like (GCB) dan non-Germinal Center B-Cell Like (non-GCB), dimana GCB memiliki prognosis yang lebih baik. Penelitian ini dilakukan dengan tujuan untuk mengetahui perbedaan ekspresi Bcl-2 pada kedua subtipe DLBCL.Bahan dan Metode: Studi potong lintang dilakukan pada 41 kasus DLBCL, terdiri atas 18 kasus subtipe GCB dan 23 kasus subtipe non-GCB. Dilakukan review diagnosis histopatologik dari sediaan H&E serta review imunofenotip dari sediaan imunohistokimia. Dilakukan pemeriksaan ekspresi Bcl-2 secara imunohistokimia dan penilaian positivitas dengan nilai cut off 31%. Selanjutnya dilakukan analisis statistik menggunakan uji Mann-Whitney.Hasil: Ekspresi Bcl-2 pada DLBCL subtipe non-GCB (nilai rerata 78,70%) lebih tinggi secara bermakna dibandingkan ekspresi Bcl-2 subtipe GCB (nilai rerata 66,61%) (p <0,05). Kesimpulan: Ekspresi Bcl-2 pada DLBCL subtipe non-GCB lebih tinggi dibandingkan ekspresi Bcl-2 subtipe GCB., Background: Diffuse Large B-Cell Lymphoma is the most common type of lymphoma non-Hodgkin found in the world, and a very heterogenous entity clinically and morphologically. The experts categorized DLBCL into two subtypes, Germinal Center B Cell-Like (GCB) and non-Germinal Center B-Cell Like (non-GCB), in which GCB has a better prognosis. This research was conducted to analyze the differences of Bcl-2 expression in these two DLBCL subtypes.Patients and methods: A retrospective, cross-sectional study was conducted in 41 DLBCL cases consisted of 18 cases of GCB subtype and 23 cases of non-GCB subtype. Review of histopathological diagnosis from H&E and immunophenotypes from immunohistochemical stained was performed. Immunohistochemistry examination of Bcl-2 expression was done and the positivity was evaluated with 31% cut off. Furthermore, statistical analysis was performed by using Mann-Whitney test.Result: Bcl-2 expression in DLBCL, non-GCB subtype (median value of 78.70%) showed a statistically higher expression compared to the expression in GCB subtype (with median value of 66.61 %) (p<0.05).Conclusion: BCL-2 expression in non-GCB subtype is higher than in GCB subtype of DLBCL.] |
