[ABSTRAK Discharge planning dapat menurunkan angka rawatan ulang. Penelitian inibertujuan mengidentifikasi hubungan implementasi sistem jenjang karir danfungsi manajemen dengan pelaksanaan discharge planning. Desain penelitiandeskriptif korelasi dengan pendekatan cross sectional. Sampel penelitian adalahperawat dan dokumen discharge planning masing-masing sebanyak 121. Carapengambilan data menggunakan simple random sampling dan proportionalsampling. Analisis data menggunakan korelasi Spearman. Hasil penelitianmenunjukkan ada hubungan implementasi sistem jenjang karir (p=0,001),penilaian kompetensi (p=0,001), kewenangan klinik (p=0,001), pengembanganprofesional berkelanjutan (p=0,001), dan fungsi manajemen (0,001) denganpelaksanaan discharge planning. Hasil penelitian ini merekomendasikan perlunyapelatihan discharge planning bagi perawat untuk meningkatkan kompetensiperawat dalam melaksanakan discharge planning.
