ABSTRAK
Latar Belakang: Ameloblastoma merupakan tumor odontogenik yang paling sering ditemukan pada tulang rahang. Berdasarkan histopatologisnya, ameloblastoma dapat dikelompokkan menjadi 6 tipe, yaitu tipe folikular, pleksiformis, akantomatosa, desmoplastik, granular, dan basal. Ameloblastoma dapat terjadi pada usia berapa saja, tetapi paling sering pada dekade3-6. MMP-9 (92-kD gelatinase/type IV collagenase = gelatinase B) mempunyai peranan dalam resorpsi tulang melalui degradasi matriks organik tulang dan perekrutan osteoklas. Beberapa penelitian menyatakan bahwa MMP-9 diekspresikanoleh ameloblastoma dan mempunyai peranan dalam sifat invasi ameloblastoma. Tujuan: Untuk menganalisa ekspresi MMP-9 di antara berbagai tipe histopatologis ameloblastoma dan kelompok usia. Metode penelitian: 40 blok parafin ameloblastoma didapatkan secara consecutive sampling. Seluruh sampel dipulas imunohistokimia menggunakan antibodi MMP-9. Hasil: Semua sampel mempunyai sel-sel yang imunopositif dengan intensitas yang beragam. 82,4% tipe pleksiformis dan 83,3% tipe campuran mempunyai ekpresi MMP-9 positif kuat, hal ini berbeda bermakna dengan tipe folikuler yang hanya 36,4% (p < 0,05). Pada usia 30 tahun ke bawah, 93,3% sampel mempunyai sel-sel imunopositif >50%, berbeda bermakna dengan usia di atas 30 tahun yang hanya 60% (p < 0,05). Kesimpulan: Ameloblastoma tipe pleksiformis dan campuran memiliki ekpresi MMP-9 yang lebih besar dibanding ameloblastoma tipe folikuler. Ameloblastoma pada usia 30 tahun ke bawah mempunyai ekspresi MMP-9 yang lebih besar dibanding ameloblastoma pada usia 30 tahun ke atas.
ABSTRACT
Background: Ameloblastoma is a common benign odontogenic tumor of the jaw. Ameloblastoma can be divided into six histopathological types, follicular, plexiform, acanthomatous, desmoplastic, granular, and basal cell. Ameloblastoma can develop in any age, with peak prevalence in 3rd - 6th decade. MMP-9 {92-kD gelatinase type IV collagenase = gelatinase B) involves in bone resorption by degradation of extracellular matrix and osteoclasts recruitment. Recent studies have found that MMP-9 is expressed by ameloblastoma and has a role in ameloblastoma local invasiveness. Objective: To analyze MMP-9 expression between different histopathological types of ameloblastoma and in different age groups. Material and method: 40 paraffin blocks were collected through consecutive sampling and the MMP-9 expression were detected using immunohistochemistry. Result: All samples showed immunopositive cells with vary intensity. 82,4% plexiform type and 83,3% mixed type have strong immunoexpression, significantly different with follicular type with only 36,4% (p < 0,05). 93,3% samples with age <30 years have >50% immunopositive cells, significantly different with those that age >30 years which only 60% (p < 0,05). Conclusion: Ameloblastoma plexiform and mixed type have higher MMP-9 expression than ameloblastoma follicular type. Ameloblastoma in age <30 years have higher MMP-9 expression than in age >30 years. |
