ABSTRAK Konsep pelayanan kesehatan primer pada era Jaminan Kesehatan Nasional (JKN)dikembangkan dengan penguatan pelayanan primer, salah satunya denganoptimalisasi peran pelayanan primer sebagai gatekeeper dengan konsep managedcare. Pada konsep managed care, suksesnya sistem gatekeeper salah satunyadinilai dari angka kunjungan dan angka rujukan ke Fasilitas Kesehatan TingkatLanjutan (FKTL). Penelitian ini bertujuan untuk mengetahui karakteristik danpersepsi pengguna pelayanan terhadap pemanfaatan Puskesmas sebagaigatekeeper di dua Puskesmas Kota Bekasi. Penelitian menggunakan desain crosssectional dan pengumpulan data melalui pengisian 208 kuesioner pada pasienpeserta JKN di Dua Puskesmas Kota Bekasi dengan metode pengambilan sampelsecara stratified purposive sampling dimana sampel diambil dari Puskesmasdengan nilai rujukan tertinggi dan Puskesmas dengan nilai rujukan terendah diKota Bekasi dengan jumlah sampel dibagi sama besar. Hasil penelitianmenunjukkan rata-rata skor pemanfaatan Puskesmas sebagai gatekeeper adalah52.25 (SD 4.87, 95% CI 51.58-52.92), namun angka rujukan masih tinggi di atas15%. Pekerjaan, persepsi terhadap sikap petugas kesehatan, dan lama berobatberhubungan dengan pemanfaatan Puskesmas sebagai gatekeeper. Persepsiterhadap sikap petugas kesehatan merupakan faktor dominan yang mempengaruhipemanfaatan Puskesmas sebagai gatekeeper. Sikap petugas kesehatanmempengaruhi pemanfaatan kembali layanan di Puskesmas dan mempengaruhipemanfaatan Puskesmas sebagai gatekeeper khususnya terkait continuity care.Penerapan konsep gatekeeper dengan baik dapat meningkatkan pemanfaatanPuskesmas dan menekan angka rujukan ke FKTL. ABSTRACT The concept of primary health care in the era of National Health Insurance (JKN)is developed by strengthening primary health care which one of them is byoptimizing the role of primary health care as a gatekeeper with the concept ofmanaged care. In this concept of managed care, the success of the gatekeepersystem one of them is judged by the visits rates and referral rates to the AdvancedLevel of Health Facilities (ALHF/Fasilitas Kesehatan Tingkat Lanjut). This studyaims to determine the characteristics and perception of service users towardsutilization of public health centers (PHC/Puskesmas) as gatekeeper in two PHC inCity of Bekasi. This study used cross-sectional design and data collection byfilling up 208 questionnaires on patients of JKN participant in two health centersin Bekasi with stratified purposive sampling methods where samples were takenfrom the health center with the highest reference score and health center with thelowest reference score in Bekasi with equally distributed samples. The resultsshowed an average score of utilization of Public Health Center (PHC) as agatekeeper: 52.25 (SD 4.87, 95% CI 51.58-52.92), but the referral rate is still highabove 15%. Jobs, perceptions of the health workers? attitudes, and treatmentduration are related to the utilization of PHC as a gatekeeper. Perceptions of thehealth workers? attitude is the dominant factor affecting the utilization of PHC asa gatekeeper. The health workers? attitude affects the repeated utilization servicesin PHC and affects the utilization of PHC as a gatekeeper, especially for that isrelated to care continuity. The proper application of gatekeeper concept canincrease the utilization of PHC and reduce the number of referrals to theAdvanced Level of Health Facilities (ALHF). |
