Infeksi Plasmodium falciparum stadium aseksual mengakibatkan terjadinya perubahan morfologi, fungsional danantigenik pada membran eritrosit hospes. Konsekuensi dari perubahan-perubahan tersebut antara lain berupapeningkatan kemampuan untuk melekat (sitoaderen) pada sel endotel di kapiler dan venula serta melekatnya eritrosittidak terinifeksi (normal) di sekeliling eritrosit terinfeksi yang disebut dengan pembentukan formasi roset. Pembentukanformasi roset tersebut merupakan suatu fenomena yang akhir-akhir ini banyak menarik perhatian karena didugamemiliki kepentin gan dalam patogenesis malaria berat terutama malaria serebral yaitu melalui terjadinya sekuestersiparasit di mikrovaskular. Pembentukan formasi roset merupakan mekanisme penempelan (sitoaderen) antara eritrositterinfeksi P. falciparum dengan eritrosit normal disekelilingnya yang melibatkan peran ligan protein parasit yaitu antaralain PfEMP1 yang diekspresikan pada eritrosit terinfeksi sebagai antigen permukaan melalui domain DBL1 alfa (DuffyBinding-Like) yang berikatan dengan reseptor CR1 dan/atau antigen A pada eritrosit normal sebagai sel target. Denganmempertimbangkan bahwa semua strain P. falciparum mampu menyebabkan terbentuknya formasi roset eritrosit, makamasih diperlukan informasi mengenai formasi roset ini baik mengenai situs sekuestersi, identritas pasti reseptor,lingkungan hemodinamik vaskuler in vivo yang mendukung dan juga pemetaan domain yang terlibat dalampembentukan formasi roset untu memastikan apakah roset berperan dalam patogenesis malaria berat.Rosette formation phenomena on patophysiology of malaria falciparum. Rossete formation is a binding mechanism betweenPlasmodium falciparum infected erythrocyte to the normal ones, which involve the role of parasite protein ligands suchas PfEMP1 expressed on infected eythrocyte as surface antigen through DBL1-alfa domain that bind to CR-1 receptorand/or A-antigen on the uninfected erythrocyte as target cell. Taking into accounts that all strains of P. falciparumparasite could cause rosette formation, therefore information of rosette formation still needed such as site ofsequesteration, identity receptors, haemodinamic enviroment of microvascular and domain mapping on rosetteformation to elucidate the role of rosette formation in phatogenesis of severe malaria. |
