Praktik kerja profesi apoteker di Apotek Kimia Farma bertujuan untuk mampu memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di apotek sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku. Fokus pembelajaran tugas dan fungsi apoteker ini terdapat pada bagian pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis dan pelayanan farmasi klinik. Salah satu tugas dan fungsinya adalah kegiatan pengkajian resep yang harus dilakukan oleh apoteker menurut Peraturan Menteri Kesehatan nomor 35 tahun 2014 tentang Standar Pelayanan Kefarmasian di Apotek. Kegiatan ini dilakukan untuk menganalisa adanya masalah terkait obat, bila ditemukan masalah terkait obat harus dikonsultasikan kepada dokter penulis resep. Kegiatan pengkajian resep meliputi kajian administratif, kajian kesesuaian farmasetik dan pertimbangan klinis. Pada bulan September 2016 di Apotek Kimia Farma No.352 mendapatkan resep yang prevalensinya paling tinggi adalah resep peptic ulcer didapatkan hasil analisa pada salah satu sampling resep resep pada bulan September 2016 yang dilakukan di apotek Kimia Farma No. 352 Depok untuk belum memenuhi kajian administratif resep karena dokter tidak menuliskan SIP, nomor telepon dokter, berat badan, alamat pasien. Pengkajian farmasetik resep belum memenuhi karena dokter tidak menuliskan bentuk dan kekuatan obat. Pertimbangan klinis resep belum memenuhi karena adanya duplikasi obat dan interaksi obat dalam resep. Work practices pharmacist profession in Kimia Farma aims to able to understand the role, duties and responsibilities of a pharmacist in the practice of pharmacy services at a pharmacy in accordance with the provisions of law and ethics applicable pharmacy. The focus of learning tasks and functions of pharmacists is contained in the management of pharmaceutical, medical devices, and medical materials consumables and clinical pharmacy services. One of the duties and functions are recipes assessment activities to be performed by pharmacists according to the Ministry of Health Regulations No. 35 of 2014 on Standards of Pharmaceutical Services in Pharmacy. This activity is conducted to analyze the existence of drug related problems, if found to be drug related problems, consulted with the prescribing doctor. Prescription assessment activities include administrative studies, study the suitability of the pharmaceutical and clinical considerations. In September 2016 in Kimia Farma 352 to get a prescription highest prevalence is a recipe peptic ulcer analysis results obtained at one sampling prescribe prescription in September 2016 conducted in pharmacy Kimia Farma No. 352 Depok to not meet administrative review of prescription because the doctor did not write SIP, physician phone numbers, weight, address patient. Assessment of pharmaceutical prescription has not met since the doctor did not write the shape and strength of the drug. Clinical judgment has not met for their prescription drug duplication and drug interactions in the recipe. |
