ABSTRAK Praktek Kerja Profesi Apoteker di Apotek Kimia Farma No. 366 Depok dilakukan selama satu bulan yaitu Bulan Februari 2017. Praktek Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker di apotek sesuai dengan standar pelayanan farmasi; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek; dan memiliki gambaran tentang permasalahan praktik kefarmasian di apotek serta mempelajari strategi dan kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di apotek. Sedangkan tugas khusus yang dilakukan selama Praktek Kerja Profesi Apoteker berjudul ldquo;Pemberian Informasi Obat dan Edukasi Pada Pasien Diabetes Melitus Tipe 2 dan Hipertensi Primer Melalui Home Care rdquo; bertujuan untuk melakukan praktek pelayanan farmasi klinik di apotek berupa pemberian informasi obat dan edukasi melalui home care untuk memonitoring penggunaan obat dan terapi pasien diabetes mellitus tipe 2 dan hipertensi sehingga dapat memberikan dampak positif bagi terapi pasien. ABSTRACT Profession Internship at Apotek Kimia Farma No. 366 Depok did in one month, February 2017. The goal of Profession Internship was the pharmacist able to understand the role, duties and responsibilities of a pharmacist in a pharmacy in accordance with the standards of pharmacy services and the applicable pharmaceutical ethics had the insight, knowledge, skills, and practical experience to carry out the practice of pharmacy in pharmacy and had an overview of the practice of pharmacy in pharmacy issues and learn the strategies and activities that can be done in order to develop the practice of pharmacy in pharmacy. While the specific tasks in this profession internship ldquo Drug and Education Information Provision In Patients with Diabetes Melitus Type 2 and Primary Hypertension Through Home Care rdquo aims to erform clinical pharmacy service practices in pharmacies in the form of providing drug information and education through home care to monitor drug use and therapy of patients with diabetes mellitus type 2 and prymary hypertension so as to have a positive impact on patient therapy. |
