Abstrak
ABSTRAK
Praktek Kerja Profesi Apoteker di Apotek Kimia Farma No. 352 dilakukan selama satu bulan yaitu Bulan Februari 2017. Praktek Kerja Profesi Apoteker ini bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker di apotek sesuai dengan standar pelayanan farmasi; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek; dan memiliki gambaran tentang permasalahan praktik kefarmasian di apotek serta mempelajari strategi dan kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian di apotek. Sedangkan tugas khusus yang dilakukan selama Praktek Kerja Profesi Apoteker berjudul ldquo;Pemberian Informasi Obat pada Pasien Diabetes Mellitus Melalui Pelayanan Kefarmasian di Rumah rdquo; bertujuan untuk melakukan memonitoring terhadap terapi pasien diabetes mellitus demi tercapainya keberhasilan terapi dan meningkatkan kualitas hidup pasien.
ABSTRACT
Pharmacists Professional Practice at Kimia Farma No. 352 Pharmacy did in one month, February 2017. The goal of Pharmacists Professional Practice was the pharmacist able to understand the role, duties and responsibilities of a pharmacist in a pharmacy in accordance with the standards of pharmacy services had the insight, knowledge, skills, and practical experience to carry out the practice of pharmacy in pharmacy and had an overview of the practice of pharmacy in pharmacy issues and learn the strategies and activities that can be done in order to develop the practice of pharmacy in pharmacy. While the specific tasks in the Pharmacists Professional Practice titled Drug Information Service in Diabetes Mellitus patient with Home Pharmacy Care aims to monitoring diabetes mellitus patient to achieve the goals therapy and increasing the quality of life.
