ABSTRAK Hidung tersumbat merupakan salah satu keluhan terbanyak pada pasien yang datang berobat ke ahli THT. Hipertrofi konka inferior merupakan salah satu penyebab sumbatan hidung dan telah banyak dipelajari dari berbagai penelitian dan terbukti berperan dalam regulasi aliran udara hidung. Namun dari beberapa penelitian terakhir ditemukan suatu struktur yang disebut nasal septal swell body NSB yang mungkin berperan dalam regulasi tahanan aliran udara hidung karena lokasinya yang berdekatan dengan katup hidung internal serta dapat mengembang mengempis sesuai siklus hidung.Tesis ini bertujuan untuk mengetahui pengaruh terapi radiofrekuensi NSB pada pasien sumbatan hidung dengan hipertrofi NSB dengan membandingkan respon klinis sebelum dan sesudah terapi berdasarkan nilai NOSE, nilai PNIF, dan nilai tahanan hidung.Penelitian ini merupakan studi eksperimental dua kelompok paralel acak yang membagi pasien sumbatan hidung dengan hipertrofi konka inferior dan NSB menjadi dua kelompok yang dilakukan radiofrekuensi konka inferior saja dan kelompok radiofrekuensi konka inferior dan NSB.Analisis data dilakukan dengan pendekatan Bootstrap. Hasil dari penelitian ini didapatkan perbedaan secara bermakna dari perubahan nilai NOSE p=0,001 dan nilai tahanan hidung 75 Pa p=0,018 antara kedua kelompok. Hasil ini berarti terapi radiofrekuensi NSB dapat menjadi terapi tambahan radiofrekuensi konka inferior terhadap pasien sumbatan hidung kronis refrakter dengan hipertrofi konka inferior dan NSB untuk mengurangi gejala sumbatan hidung.Kata kunci: radiofrekuensi, nasal septal swell body, konka inferior, sumbatan hidung kronis refrakter ABSTRACT Nasal obstruction is one of the most symptom in daily practice of ENT Surgeon. Inferior turbinate hypertrophy is one of the causes of nasal obstruction and has been widely studied from various research and has been shown having a role in the regulation of nasal airflow. But from several recent studies found a structure called a nasal septal swell body NSB that may play a role in the regulation of nasal airflow resistance because of its location adjacent to the internal nasal valve and alternating congestion and decongestion according to nasal cycle.This study aims to evaluate the effect of radiofrequency therapy of NSB on patients with nasal obstruction with NSB hypertrophy by comparing clinical response before and after therapy based on NOSE value, PNIF value, and nasal resistance value.This study is an experimental study of two random parallel groups that divide the nasal obstruction patients with hypertrophy of inferior turbinate and NSB into two groups which underwent radiofrequency of inferior turbinate only and underwent radiofrequency inferior turbinate and NSB.Data analyzed with bootstraps method. The results of this study show significant differences from changes in the value of NOSE p 0.001 and the nasal resistance value 75 Pa p 0.018 between the two groups. These results suggest that radiofrequency ablation of NSB may be an additional therapy to radiofrequency of inferior turbinate in patients with nasal obstruction and hypertrophy of inferior turbinate and NSB.Keywords radiofrequency, nasal septal swell body, inferior turbinate, refractory chronic nasal obstruction |
