Abstract
Rumah Sakit dituntut untuk memberikan pelayanan yang bermutu sesuai
dengan standar yang ditetapkan dan dapat menjangkau seluruh lapisan masyarakat.
Standar pelayanan minimal di rumah sakit pada pelayanan farmasi salah satunya
adalah penulisan resep sesuai formularium harus 100% dengan tujuan yaitu
tergambarnya efisiensi pelayanan obat kepada pasien. Formularium Rumah Sakit
bermanfaat dalam kendali mutu dan kendali biaya obat yang akan memudahkan
pemilihan obat yang rasional, mengurangi biaya pengobatan, dan mengoptimalkan
pelayanan kepada pasien. Pemantauan dan evaluasi Formularium Rumah Sakit
dilakukan terhadap kepatuhan penggunaan Fornas dan kepatuhan penggunaan
Formularium Rumah Sakit.
Penelitian ini dilakukan di RSUI pada tanggal 6 September 2021 ? 31
Oktober 2021 dan merupakan penelitian deksriptif menggunakan metode
pengambilan sampel secara retrospektif dengan mengamati dan mengevaluasi
kesesuaian peresepan obat pasien depo farmasi rawat jalan RSUI periode Januari ?September 2021 dengan Formularium Rumah Sakit RSUI berdasarkan kriteria
inklusi yaitu resep pasien rawat RSUI dengan tanggal resep pada periode Januari-September 2021 dan kriteria eksklusi yaitu resep alat kesehatan dan bahan medis habis pakai, juga order location yang bukan berasal dari poliklinik rawat jalan RSUI. Selain itu juga dilakukan evaluasi kesesuaian obat lost sales di depo farmasi rawat jalan RSUI periode Januari ? September 2021 dengan Formularium Rumah Sakit RSUI dengan kriteria inklusi seluruh resep pasien dari depo rawat jalan yang terdapat salah satu jenis item obat atau semua item yang obatnya disalin resep (copy resep) keseluruhan atas permintaan pasien atau tidak ditanggung oleh guarantor atau stok obat habis dan kriteria ekslusi yaitu Resep pasien dari depo rawat jalan yang obatnya tidak disalin resep dan disalin sebagian saja. Data diolah dan dihitung menggunakan Microsoft Excel 2013 kemudian data yang dihasilkan ditampilkan dalam bentuk persentase. Hasil penelitian di dapat bahwa peresepan obat pada depo farmasi rawat jalan RSUI periode Januari ? September 2021 secara keseluruhan didapatkan 59,52% yang sesuai dengan Formularium Rumah Sakit dan yang tidak sesuai yaitu 40,47%. Dengan peresepan obat pada depo farmasi rawat jalan non COVID-19 RSUI periode Januari ? September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 61,08% dan yang tidak sesuai yaitu 38,92%. ix September 2021 yang sesuai dengan Formularium Rumah Sakit yaitu 59,38% dan
yang tidak sesuai yaitu 40,63%. Untuk obat lost sales yang sesuai dengan
Formularium Rumah Sakit yaitu 61,12% dan yang tidak sesuai yaitu 38,88%.

......Hospitals are required to provide quality services in accordance with
established standards and can reach all levels of society. Minimum service
standards in hospitals specifically in pharmaceutical services, one of which is
writing prescriptions according to the formulary must be 100% with the aim of
depicting the efficiency of drug services to patients. Hospital Formularies are useful
in quality control and drug cost control which will facilitate rational drug selection,
reduce treatment costs, and optimize services to patients. Monitoring and evaluation
of the Hospital Formulary is carried out on compliance with the use of National
Drug Formulary and compliance with the use of Hospital Formulary.
This study was conducted at RSUI on September 6, 2021 ? October 31, 2021
and is a descriptive study using a retrospective sampling method by observing and
evaluating the suitability of drug prescriptions for outpatient pharmacy depots at
RSUI for the period January ? September 2021 with the RSUI Hospital Formulary
based on inclusion criteria are prescriptions for RSUI inpatients with prescription
dates in the January ? September 2021 period and exclusion criteria are
prescriptions for medical devices and consumable medical materials, as well as
order locations that are not from the RSUI outpatient polyclinic. In addition, an
evaluation of the suitability of lost sales drugs at the outpatient pharmacy depot of
RSUI for the period January ? September 2021 with the RSUI Hospital Formulary
was also carried out with the inclusion criteria of all patient prescriptions from the
outpatient depot which contained one type of drug item or all items whose
medicines were copied by prescription (copy of prescription) in its entirety at the
request of the patient or not covered by the guarantor or the drug stock runs out and
the exclusion criteria are patient prescriptions from outpatient depots whose
medicines are not copied and only partially copied. The data is processed and
calculated using Microsoft Excel 2013 then the resulting data is displayed in
percentage form.
The results of the study showed that drug prescriptions at the outpatient
pharmacy depot of RSUI for the period January ? September 2021 overall obtained
59.52% which were in accordance with the Hospital Formulary and those that were
not appropriate, 40.47%. By prescribing drugs at non-COVID-19 outpatient
pharmacy depots at RSUI for the period January ? September 2021, which are in
accordance with the Hospital Formulary, 61.08% and those that are not appropriate,

38.92%. Meanwhile, drug prescriptions at the outpatient pharmacy depot
specifically for COVID-19 RSUI for the period January ? September 2021 which
are in accordance with the Hospital Formulary are 59.38% and those that are not
appropriate are 40.63%. For lost sales drugs that are in accordance with the Hospital
Formulary are 61.12% and those that are not suitable are 38.88%.
