Tingginya kunjungan pasien bayar karcis pada Balai Pengobatan Anak yang mencapai 8.744 kunjungan pada tahun 2001 merupakan peluang bagi Puskesmas Selabatu untuk meningkatkan pendapatan fungsionalnya melalui penyesuaian tarif, dengan tujuan untuk meningkatkan kualitas pelayanan kepada masyarakat khususnya pelayanan pada BP Anak. Akan tetapi karena program kesehatan anak merupakan program yang mempunyai kontribusi terhadap tinggi rendahnya derajat kesehatan masyarakat, maka besarnya tarif yang akan diberlakukan harus berdasarkan pada besarnya biaya satuan pelayanan serta mempertimbangkan tingkat kemampuan dan kemauan membayar masyarakat terhadap pelayanan kesehatan. Sedangkan permasalahan yang dihadapi untuk penyesuaian tarif tersebut adalah belum adanya informasi yang tepat tentang besarnya biaya satuan pelayanan khususnya pada Balai Pengobatan Anak, serta tingkat kemampuan dan kemauan membayar dari masyarakat terhadap pelayanan kesehatan. Maka tujuan umum dari penelitian ini adalah untuk melakukan analisis tarif pelayanan pada Balai Pengobatan Anak serta mengukur tingkat kemampuan dan kemauan membayar dari masyarakat sehingga di peroleh informasi yang tepat utuk penyesuaian tarif yang rasional.Penelitian ini merupakan Penelitian deskriptif dengan rancangan cross sectional. Pengumpulan data biaya BP Anak dengan menggunakan data sekunder, sedangkan untuk data ATP/WTP masyarakat menggunakan data primer yang didapat dari wawancara langsung dengan rumah tangga terpilih. Perhitungan biaya satuan pelayanan didapatkan dari analisis biaya dengan metode double distribution. Kemudian untuk analisis tarif dikembangkan melalui simulasi tarif dengan menggunakan kurva ATP untuk memperkirakan besarnya penurunan utilisasi bila tarif dinaikkan.Hasil penelitian menunjnkkan bahwa biaya satuan aktual dengan investasi sebesar Rp. 4.442, biaya satuan tanpa investasi Rp.4.019, serta biaya satuan tanpa investasi dan gaji Rp. 2.559, dengan Cost Recovery Rate sebesar 24,68 %. Sedangkan biaya satuan normative sebesar Rp. 4.459 .Kenaikan tarif dengan mempertimbangkan tingkat kemampuan dan kemauan membayar masyarakat, yang direkomendasikan pada BP Anak adalah untuk pemeriksaan oleh Dokter umum sebesar Rp 4.500,- dengan konsekuensi masyarakat tersingkir dari pelayanan sebesar 3 %. Sedangkan tarif untuk pemeriksaan oleh Dokter Spesialis Anak adalah Rp 4.500,- ke atas dengan catatan untuk jasa medisnya dilakukan perhitungan kembali .Dengan hasil tersebut disarankan bagi Puskesmas untuk mengusulkan kenaikan tarif kepada pengambil keputusan secara bertahap, dan untuk pemeriksaan oleh Dokter Spesialis Anak dibuka pada hari - hari tertentu, sedangkan untuk masyarakat yang tidak mampu diupayakan dengan pemberian kartu sehat.

While a problem which is stood before for the cost adaptation that is : it's not available the information which is exact about the big one of multi service, specially for the Children Medicine Hall, also the ability level and willingness to pay for community to the healthy service. So the public aim from research to analysis the cost of maintenance of the Children Medicine Hall so the ability level and willingness to pay from community so far as it's gained the information to get the exact to the rational cost adoption.This research showed the descriptive by the cross sectional design. The data collection only the Children Medicine Hall by using the secondary data, while for ATP/WTP community data uses the primer data that was gained from the direct interview with the selected house hold. The multi cost amount of service was obtained from the cost analysis by double distribution method, and then the cost analysis was branched up through the cost simulation by using curve ATP to think the big run down utilization if the cost was gotten an rise.The result research showed that the multi actual cost with the big infestation as much as Rp 4.442,- , the multi maintenance without infestation as much as Rp 4.019; also the multi cost out of infestation and salary as much as Rp 2.559,- by the cost recovery rate as much as 24,68 % , while the multi normative cost as much as Rp 4.459, The cost rise with consideration the ability level and willingness to pay the community that was recommended for the Children Medicine Hall was for checking up by the public doctor as much as Rp 4.500 ,- with consequence the community to get isolated from service as much as 3 % . While the cost for checking up by the specialist doctor as much as over of Rp 4.500,- with notice for the doctor incentive by doing the recounting.By the result was suggested for Public Health Centre to propose the cost rise up taking a decision in every phase, and to check up by the specialist doctor of children in the certain days was opened in exact days, while the community was not able to be effort by giving the healthy card. |
