Penelitian Litbangkes tahun 1996 biaya penyelenggaran pelayanan kesehatan dalam kurun waktu 10 tahun meningkat, menunjukan lebih tinggi dari anggaran biaya. Pada tahun 1984/1985 biaya meningkat dari Rp. 1,89 trilliun menjadi Rp. 7.03 trilliun pada tahun 1994/1995. Peningkatan biaya penyelenggaraan pelayanan kesehatan dipengaruhi oleh tingginya inflasi umum, belum ada patokan biaya standar terhadap setiap jenis pelayanan kesehatan, dan "Consumer Ignorance" yang menimbulkan moral hazard yang tinggi. Untuk mengatasi hal tersebut diperlukan pengendalian biaya antara lain dengan penetapan DRG's.Tujuan umum penelitian ini adalah untuk mendapatkan gambaran variasi biaya yang timbul pada kasus cedera kepala dengan craniotomy.Penelitian ini adalah suatau Study Eksplorasi terhadap Variasi Biaya Kasus Cedera Kepala Dengan Craniotomy Dalam Rangka Penetapkan DRG's yang dilaksanakan di RSU Tangerang. Kasus cedera kepala adalah merupakan salah satu kasus yang masuk peringkat sepuluh penyakit terbesar. Jumlah kasus yang diteliti adalah 29 kasus yang kemudian dikelompokan menjadi DRG's 2 untuk Craniotomy dengan kelompok umur diatas 17 tahun dan DRG's 3 untuk Craniotomy dengan kelompok umur dibawah 18 tahun.Hasil penelitian menunjukan bahwa kasus terbanyak adalah kelompok umur diatas 17 tahun yaitu 79,3% dengan rata-rata umur adalah 33 tahun, kelompok umur dibawah 18 tahun yaitu 20,7% dengan rata-rata 13 tahun. Pada kelompok umur dibawah 18 tahun lama hari rawatnya lebih rendah yaitu 6,67 hari dibandingkan dengan kelompok umur diatas 17 tahun yaitu selama 9,7 hari.Ditemukan diagnosa penyerta dan penyulit, diagnosa penyerta hanya terdapat pada kelompok umur diatas 17 tahun, sedangkan untuk diagnosa penyakit sangat bervariasi. Terhadap kasus yang disertai dengan diagnosa penyerta rata-rata biaya Rp. 10.555.862, yang disertai diagnosis penyakit rata-rata biayanya yaitu Rp. 12.993.007.Dalam rangka penyusunan DRG's agar angka rata-rata biaya lebih stabil maka perlu dilakukan penelitian lebih lanjut terhadap diagnosa yang sejenis pada beberapa rumah sakit yang typenya sama dengan jumlah sampel yang cukup besar.Daftar Pustaka : 28 (1976 - 2002)
Explorative Study of Cost Variability in the Determination of Diagnostic Related Group's (DRG's) of the Cases of Cranium Injury using Craniotomy at the General Hospital of TangerangThe research of Centre for Research and Development of Department of Health in 1996 had shown that cost of health service provision had been increased for the past 10 years; it had shown higher than the allocation fund. In the fiscal year 1984/1985 the cost for health service provision was Rp 1.89 trillions and risen to Rp 7.03 trillions by the fiscal year 1994/1995. The increases of cost for health services provision is influences by high inflation rate, the absence of cost standard for every type of health service, and "consumer ignorance" that led to high morale hazards. To overcome those problems, it is a need to manage the cost, and one the ways is through determination of DRG's.This research aims at getting description of variability of cost due to head injury using craniotomy.This research is an Explorative Study of Cost Variability in the Determination of Diagnostic Related Group's (DRG's) of the Cases of Cranium Injury using Craniotomy at the General Hospital of Tangerang. The case of cranium injury is one among the top ten biggest cases in the hospital. Number of cases being examined in this research was 29 cases and it were grouped into DRG's 2 for the craniotomy of age group above 17 years old and DRG's 3 for craniotomy for the age group before 18 years old.The results of research shows the biggest cases was in the age group above 17 years old such as 79.3% with the average age of craniotomy was 33 years old, the age group before 18 years old was 29.7% with the average age was 13 years old. In the group before 18 years old, the length of stay was 6.67 days; this lower if compared to the age group above 17 years old such as 9.7 days.It was found a contributory and confounding diagnostic; wherein contributory diagnostic only for the age group above 17 years old, while confounding diagnostic was very varied. The average cost for the case with contributory diagnostic was Rp 10,555,862, while for the confounding diagnostic was Rp 12,993,007.In order to develop DRG's so that the average cost would be more stable, it is need to make a further study for the similar diagnostic in the several hospitals who have similar type of diagnostic and using a bigger number of research sample.References: 28 (1976--2002) |
