Masalah pembiayaan kesehatan di Indonesia semakin hari semakin mendapat tantangan yang berat, baik dalam segi kualitatif maupun kuantitatif, sementara biaya kesehatannya sendiri relatif kecil.Piutang merupakan salah satu masalah yang menjadi perhatian utama pengelola keuangan rumah sakit, walaupun demikian sebuah rumah sakit tidak dapat menghindari kenyataan bahwa piutang pasien merupakan bagian terbesar dari aktiva lancarnya.Pasien jaminan perorangan memiliki resiko tinggi untuk menyebabkan piutang tidak tertagih, dan bila piutang tidak tertagih pada suatu rumah sakit jumlahnya cukup besar maka akan mengganggu kelancaran operasional rumah sakit.Laporan tahunan direktorat administrasi RS Pluit menunjukkan bahwa pasien rawat inap jaminan perorangan yang menimbulkan piutang tidak tertagih pada tahun 2002 dan 2003 mencapai lebih 2,00%o dari jumlah total pasien rawat inap pada periode yang sama. Keadaan ini meresahkan manajemen rumah sakit.Penelitian ini bertujuan mencari gambaran tentang karakteristik pasien rawat inap jaminan perorangan yang berpotensi menimbulkan piutang dan piutang tidak tertagih di RS Pluit pada periode tahun 2002 dan 2003, serta efektifitas kebijakan/peraturan yang berlaku.Karakteristik pasien yang digunakan dalam penelitian ini meliputi: asal masuk pasien, pemilihan kelas perawatan, lama hari rawat, jenis tindakan, cara lepas rawat, biaya perawatan, dan pemberi rekomendasi.Dari hasil penelitian disimpulkan bahwa pasien yang berasal dari IGD, memilih kelas perawatan CCU dan VIP, dengan lama hari rawat lebih dari 6 hari, tanpa atau dengan tindakan bedah, lepas rawat dengan seijin dokter, dengan biaya perawatan lebih dan Rp.20 juta, dan direkomendasi oleh direksi atau tanpa rekomendasi mempunyai distribusi besar terhadap timbulnya piutang tidak tertagih.Kebijakan/peraturan yang berlaku ternyata tidak cukup efektif untuk meminimumkan piutang tidak tertagih pada semua kriteria pasien, kecuali untuk pasien yang lepas rawat karena meninggal dunia dan pasien yang memilih kelas perawatan di CCU.Pada hasil observasi kebijakan/peraturan yang ada sudah dilaksanakan oleh petugas yang terkait, hanya belum optimal dan masih banyak kendala yang tidak bisa dihindari.Saran-saran yang diajukan antara lain meninjau kembali kebijakan/peraturan, memperketat permintaan uang jaminan, mengintensifkan penagihan selama perawatan, membatasi otorisasi pemberi rekomendasi, meninjau kembali manfaat kartu VIP RS Pluit dan kerjasama dengan Dinas Kesehatan sebagai penyelenggara pelayanan kesehatan bagi keluarga miskin/orang miskin.Kepustakaan : 30 (1971 -- 2001)
Analysis of Uncollectable Self Paid Inpatient's Account in Pluit HospitalThe health care financial problems in Indonesia nowadays are facing even more challenging situations, both in qualitative and quantitative aspects, meanwhile the health care budgets are relatively small.The hospital management focused its main interest in the account receivable problems, even though it cannot escape from the reality that patient 's account receivable occupied the biggest part of its current account.Self paid inpatient 's possesses high risk in generating bad debts, which can contribute a bad impact to the hospital operational.Administration Department 's annual report indicate that the bed debt emerged from the self paid inpatient 's in Pluit Hospital in 2002 -- 2003 has reached to 2%o from the total inpatient in the same period This situation is certainly disturbing the hospital management.The objectives of this research are to describe the self paid inpatient 's characteristics which are potential in generating account receivable and bad debts in Pluit Hospital in the period of 2002 and 2003, and the effectiveness of the prevailing regulations /policies.The criteria of patient characteristic that are applicable in this research cover from the origin of the patient, the room grade selection, the treatment period, the care action taken, the way of patient 's dischargement, the health care cost and the person that recommend / on who 's recommendation.From the survey results can be concluded that patients originated from Emergency Room (ER), choose the CCU and VIP room, with or without undergoing surgery, discharged under doctor?s recommendation, with the health care costs more 20 millions rupiahs and with or without recommendation from board of directions have brought out a large contribution in the emergence of bad debts.The prevailing regulations or policies turned out to be effective in minimizing the bad debts from all of the patient 's criteria, except for the patient discharged for the caused of death or the patient that choose the CCU room.Based on the observation results, the prevailing regulations on policies have actually been carried out thoroughly by the officer in charge, even though many unavoidable obstacles occurred and still un-optimized.The propositions which will be promoted such as to review the regulation / policies, to tighten up the procedure of guarantee money collection, to intensify the billing collection upon treatment, to restrict the recommended authorization to review the benefits of Plait Hospital 's VIP card and cooperation with health official (Din Kes) as the organizer of health care services for the people who live under poverty line.Bibliography : 30 (1971 - 2001) |
