Makin besar rumah sakit, berarti makin banyak jumlah dokter dengan berbaagai macam keahlian, yang akan menyebabkan keanekaragaman resep yang dituliskan sehingga menyulitkan pihak rumah sakit dalam menyediakan obat. Dalam hal ini pengadaan obat menjadi semakin serba tidak pasti, membingungkan, obat dalam "hutan belantara".Formularium rumah sakit merupakan buku yang berisi nama obat-obatan yang disediakan di rumah sakit untuk pasien rawat inap dan rawat jalan. Dengan formularium diharapkan dapat memudahkan dokter dalam menulis resep. Namun dalam pelaksanaannya dapat menimbulkan konflik bagi dokter karena dirasakan sebagai pembatasan dalam memilih obat yang tepat.Penelitian ini merupakan suatu survei analitik cross sectional. Pengumpulan data primer dilakukan dengan wawancara mendalam terhadap 10 orang dokter penulis resep terbanyak yang masuk di Instalasi Farmasi Rurnah Sakit (IFRS) dan data sekunder di ambit dari resep yang masuk di IFRS selama 3 bulan terakhir yaitu bulan Maret, April, ,dan Mei 2000 yang berjumlah 1.116 lembar resep. Penelitian ini dilakukan di poli rawat jalan dan IFRS PGI Cikini. Hasil penelitian ini dianalisis secara statistik univariat dan bivariat, T test dan Chi Square menggunakan SPSS 10. Penelitian ini menyimpulkan bahwa rata-rata penulisan resep di RS PGI Cikini di luar formularium berjumlah 50,8%. Kebanyakan dokter tidak memahami isi buku formularium (70%), sedangkan pendidikan dokter dan status dokter ada hubungan dengan penulisan resep. Dokter umum dengan status honorer dan tetap lebih banyak menulis resep di luar formularium daripada dokter spesialis atau subspesialis sebagai dokter tamu. Sebagai saran untuk tindak lanjut, maka direksi rumah sakit perlu mengintervensi terhadap dokter sebagai penulis resep supaya menu makan formularium rumah sakit dan memberikan tugas kepada panitia farmasi dan terapi untuk mensosialisasikan buku formularium secara terus menerus. The bigger the hospital means that there are doctors with various specialties who prone to prescribe drugs without taking into consideration the hospital formula. This will cause difficulties to the hospital in trying to be able to provide all the drugs needed. The hospital as the drugs provider will face uncertainties confusion and without direction like somebody in the wilderness. Hospital formula is a book contains name of drugs provided by hospital for both inpatients and outpatients. With the formulary, we expect it can help and guide doctors to write a prescription. But, as a matter of fact we find that formulary can become an abstract for doctors to choose what they think is the best for their patients.This research is using a cross sectional analytic survey. The process of collecting primary data was undertaken by interviewing intensively 10 doctors as the ten biggest prescribes. Secondary data was taken from the prescription received by Hospital Pharmacy Installation in this recent 3 months, March, April, and May 2000. They are 1.116 prescriptions. This research was done in outpatient Department and Cikini Hospital Pharmacy Installation. This research is using univariat and bivariat statistic methods; T test and chi square in SPSS 10. The results show that prescription of Drugs unlisted in the formula book (50.8%). Most of doctors do not understand content of formula book (70%), whereas the basic medical education and status of the doctors have a correlation with prescription writing. General Physicians with honorary status and full time status are more often prescribing drugs unlisted in the formula book compared to specialist doctors or super specialist doctors who are the visiting doctors of the hospital.As the conclusion I suggest the board of Director should insist to the doctors as the drugs prescribes to use the Hospital Formula Book properly and do not prescribe unlisted drugs. Furthermore, The Pharmacy and Therapy Committee has to socialize the Hospital Formula Book continuously and evaluate the implementation periodically. |
