Investigasi Kontak (IK) merupakan kegiatan pelacakan pada orang-orang yang kontak dengan pasien tuberkulosis (TBC). Klinik Jakarta Respiratory Center milik Perkumpulan Pemberantasan Tuberkulosis Indonesia (JRC-PPTI) juga turut melaksanakan IK dengan meminta seluruh kontak serumah pasien TBC BTA (+) melakukan pemeriksaan dahak secara gratis di klinik. Adanya hambatan kontak serumah untuk datang langsung, menjadikan adanya modifikasi pengambilan sampel secara tidak langsung melalui pasien untuk membawakan sampel kontak serumahnya pada jadwal kontrol. Namun, kemudahan yang diberikan masih belum bisa menjangkau seluruh kontak serumah untuk melaksanakan IK, sehingga diperlukannya evaluasi untuk mengetahui efektifitas pelaksanaan program dari sisi pelaksana dengan menggunakan kerangka kerja Reach, Effectiveness, Adoption, Implementation, Maintenance (RE-AIM) dan mengetahui penerimaan program melalui persepsi kontak serumah menggunakan teori Health Belief Model (HBM). Penelitian ini adalah penelitian kualitatif dengan pendekatan studi kasus yang dilakukan dengan wawancara mendalam kepada pasien TBC, keluarga pasien, dokter, perawat dan pimpinan klinik. Tujuan penelitian ini untuk mengetahui efektifitas pelaksanaan investigasi kontak serumah difasilitas kesehatan swasta dari persepsi klinik dan kontak serumah. Hasil penelitian, pelaksanaan IK di klinik utama JRC-PPTI masih belum efektif ditinjau dari jangkauan, efektifitas, adopsi, implementasi dan pemeliharaan. Sementara penerimaan pelaksanaan IK di klinik utama JRC-PPTI dipengaruhi oleh persepsi kerentanan, keparahan, manfaat dan isyarat berperilaku kontak serumah.
Contact Investigation (CI) is a tracing activity on people who have been in contact with tuberculosis (TB) patients. The Jakarta Respiratory Center Clinic owned by the Indonesian Association Against Tuberculosis (JRC-IAAT) also participates in carrying out CI by requesting all household contacts of TB patients (+) to undergo a free sputum examination at the clinic. There are barriers for household contacts to come directly, resulting in a modification of indirect sampling through patients to bring samples of household contacts during scheduled appointments. However, some provided facilities still cannot reach all household contacts to implement IK, so an evaluation is needed to determine the effectiveness of program implementation from the implementer's side using the Reach, Effectiveness, Adoption, Implementation, Maintenance (RE-AIM) framework and to seek out program acceptance through household contact perceptions using the theory of Health Belief Model (HBM). This is qualitative research with a case study approach which was conducted through in-depth interviews with TB patients, patient’s family members, doctors, nurses, and clinic leaders. The purpose of this study was to determine the effectiveness of carrying out investigations of household contacts in private health facilities from the perceptions of clinics and household contacts. The study results indicate that the implementation of IK in the JRC-PPTI main clinic is still not effective in terms of reach, effectiveness, adoption, implementation, and maintenance, while acceptance of the implementation of CI in the JRC-PPTI main clinic is influenced by perceptions of vulnerability, severity, benefits and behavioral cues of household contact. |
