EVALUASI OPERASI TANDUR TULANG SEKUNDER PADA PASIEN CELAH ALVEOLAR DI RUMAH SAKIT ANAK BUNDA HARAPAN KITA = Evaluation Secondary Alveolar Bone Graft in Alveolar Cleft Patient in Harapan Kita Hospital

Latar belakang: Celah alveolar dapat menimbulkan masalah estetika dan fungsional yang dapat mempengaruhi kualitas hidup pasien. Terdapat beberapa cara untuk menutup celah alveolar, diantaranya dengan perawatan orthodonti, pemasangan protesa, hingga dilakukannya tandur tulang. Tandur tulang merupakan baku standar untuk dilakukannya prerawatan celah alveolar. Upaya untuk mendapatkan hasil operasi tandur tulang yang optimal perlu memperhatikan beberapa faktor diantaranya waktu operasi, teknik operasi, lebar celah, dan kondisi gigi kaninus. Penilaian terhadap hasil operasi tandur tulang digunakan untuk mengevaluasi keberhasilan operasi dengan cara mengukur sudut gigi kaninus, tinggi vertikal gigi kaninus, dan tinggi tulang alveolar. Tujuan: Untuk mengevaluasi hasil operasi tandur tulang sekunder melalui tinggi tulang alveolar dengan radiograf dan dianalisa dengan Bergland indeks serta perubahan sudut gigi kaninus, tinggi vertikal gigi kaninus. Metode: 5 radiograf OPG dan 12 radiograf oklusal didapatkan dari pasien operasi tandur tulang sekunder dengan cara restrospektif analitik dari poli CLP RSAB Harapan Kita. Hasil: Didapatkan nilai sebesar 91% keberhasilan operasi tandur tulang dinilai dari tinggi tulang alveolar. Adanya perubahan sudut gigi kaninus dan tinggi vertikal gigi kaninus sebelum dan sesudah operasi. Kesimpulan: Keberhasilan operasi tandur tulang yang ditandai dengan terbentuknya bone brigde akibat adanya gaya mekanikal pada celah alveolar setelah operasi tandur tulang yang dapat dinilai dengan adanya perubahan sudut gigi kaninus dan tinggi vertikal gigi kaninus. Background: Alveolar cleft can effect the quality of life the patient due to esthetic and functional problems. There are several techniques to close alveolar cleft such as orthodonti treatment, dental prothesis, and bone grafting. Secondary alveolar bone graft is the gold standar in alveolar cleft treatment. In order to get the optimum result of secondary alveolar bone graft, there are some factors need to be considered timing of operation, operation technique, width of the cleft, and condition of canine teeth. Some parameters were used to evaluate the secondary alveolar bone graft procedure, there are canine angle, vertical height of canine, and alveolar height. Objective: to evaluate secondary alveolar bone graft procedure using alveolar height by Bergland radiographic scale and canine angle and vertical height. Method: 5 OPG and 12 oklusal radiograph were collected from patient alveolar cleft post secondary alveolar bone grat using retrospective analytic sampling from Harapan Kita Hospital. Result: Satisfactory results were obtained in 91% of cases. There are significant changes in canine angle and vertical height post secondary alveolar bone graft procedure. Conclusion:Formation of bone bridge due to mechanical force in alveolar cleft post secondary alveolar bone graft can be identified by the change of canine angle and vertical height. |
