"Prototipe sistem informasi manajemen pemeliharan peralatan medis berbasis web adalah sebuah aplikasi sistem informasi yang dapat digunakan sebagai sarana untuk membantu teknisi elektromedis dalam melakukan kegiatan pemeliharaan dan kalibrasi peralatan medis. Riset ini bertujuan untuk menghasilkan sebuah prototipe sistem informasi berbasis Computerized Maintenance Management Systems CMMS yang dapat diimplementasikan pada unit/instalasi pemeliharan sarana dan prasana rumah sakit IPSRS . Penelitian dilakukan dengan metode kuasi eksperimental posttest only. Analisis pengembangan dan perancangan sistem yang didasarkan pada siklus hidup pengembangan sistem Systems Development Life Cycle model waterfall. Pengembangan sistem menggunakan PHP sebagai bahasa pemrograman dan MySQL sebagai database dan web server apache, sedangkan untuk design nya menggunakan HTML 5, CSS, JavaScipt dan Framework CSS Bootstrap.
Hasil pengujian yang dilakukan oleh tiga programer dan sepuluh pengguna menunjukkan bahwa aspek usabilitas sistem yang dihitung berdasarkan kategori skala Likert diperoleh persentase masing-masing sebesar 88 dan 83,6 sehingga dapat disimpulkan bahwa usabilitas prototipe sistem informasi manajemen pemeliharaan peralatan medis berbasi web sangat mudah dipergunakan dan dapat digunakan untuk memenuhi dokumen penilaian akreditasi maupuen re-akreditasi rumah sakit berdasarkan MFK.8. dan MFK. 8.1. Untuk pengembangan lebih lanjut, sistem informasi manajemen pemeliharaan peralatan medis berbasis web ini diharapkan dapat diintegrasikan dengan sistem informasi manajemen aset dan sistem informasi manajemen rumah sakit dengan menambah beberapa modul lainnya seperti modul data spare-part, capaian kinerja teknisi, library dan lain lain.

The web based prototype of medical equipment maintenance management information system is an information system application used as a means to assist electromedical technicians in performing maintenance activities and calibration of medical equipment. This research aims to produce a prototype of information system based on computerized maintenance management systems CMMS that can be applied to the hospital facility maintenance units IPSRS . The research employed the posttest only quasi experimental method. The analysis of the systems planning and development was based on systems development life cycle waterfall model . The prototype used PHP programming language for its system development, MySQL for its Database and Apache Web Server, and HTML 5, CSS, JavaScipt and Bootstrap CSS Framework for its design.
The results of the test administered to three programmers and ten users indicate that the usability of the application based on the Likert scale is at 88 and 83.6 respectively. Thus, it can be concluded that the usability of the web based prototype of medical equipment maintenance management information system is easy to use and applicable to meet the accreditation and re accreditation assessment documents of hospitals based on MFK.8. and MFK. 8.1. For further development, it is suggested that the web based medical equipment maintenance management information system integrate with asset management and hospital management information system by adding some other modules such as data modules of spare part, technician performance, library and others.

"Tesis ini merupakan penelitian yang bersifat deskriptif analitik, untuk mengetahui tingkat bahaya dan risiko yang ada di divisi ERS (equipment repair and service), departemen MOSC (Maintenance Operation Support and Certification), pada sebuah perusahaan yang bergerak dibidang minyak dan gas (Oil and Gas Company). Hasil penelitian ini memberikan gambaran tingkat risiko untuk masing-masing shop yang ada di divisi ERS, dimana tingkat risiko tertinggi ada pada PSV shop, dan hot tapping shop yang bekerja di lokasi pipa minyak produksi. Pengolahan data dilakukan dengan menggunakan JSA. Semua potensi bahaya yang diperoleh akan dilakukan penilaian consequences, probability, dan exposure. Metode analisis risiko yang digunakan adalah kualitatif dan semikuantitatif yang mengacu pada AS/NZS 4360 : 2004. Selain itu dihasilkan rekomendasi-rekomendasi untuk mengendalikan risiko-risiko yang ada sehingga sampai risiko dikategorikan menjadi risiko yang dapat diterima.

This thesis is a descriptive analytic study, to determine the level of hazards and risks that exist in ERS (equipment repair and service) division MOSC (Maintenance Operation Support and Certification) departement, in Oil and Gas Company. The study provides an overview of the level of risk for each shop in the ERS division, where there is the highest risk level at PSV shop, and hot tapping shop working on production of oil pipeline location. Data processing is done by using the JSA. All potential hazard assessment earned will be consequences, probability, and exposure. Risk analysis methods used are qualitative and semiquantitative which refers to the AS / NZS 4360: 2004. Moreover resulting recommendations to control the risks that exist so until the risk is categorized into acceptable risk."

"RSCM adalah rumah sakit rujukan nasional, tersier, pelayanan sub spesialis dan sub spesialis luas. Unit Rawat Inap Terpadu Gedung A menjadi sentral pelayanan rawat inap dengan 660 tempat tidur dari total 1033 kapasitas RSCM. Pasien yang dirawat di Gedung A 90 menggunakan alat kesehatan medis. Sistem manajemen Pemeliharaan alat medis penting untuk penyediaan alat medis yang siap pakai, laik pakai, dan aman. Tujuannya untuk mengetahui faktor-faktor yang mempengaruhi dan bagaimana perancanaan, pelaksanaan, monitoring dan evaluasi pemeliharaan. Menggunakan metode penelitian kualitatif dengan wawancara mendalan, Forum Grup Diskusi FGD, telaah dokumen dan observasi. Hasil dari penelitian ini bahwa pendanaan, pemenuhan suku cadang, sarana dan prasarana, kebijakan dan system informasi berpengaruh dalam pelaksanaan system manajemenpemeliharaan alat medis.

RSCM is a national, tertiary referral hospital, sub specialist and sub specialist services. The integrated inpatient unit building a becomes the center of inpatient services with 660 beds out of a total of 1033 rscm capacity. 90 Patients treated in integrated inpatient unit Building A RSCM use medical equipment. Health Management system maintenance of medical devices is important to provide ready made, wearable, and safe medical equipment. The purpose is to know the factors that influence and how the planning, implementation, monitoring and evaluation of maintenance. Using qualitative research methods with in depth interviews, focus grup disscusion, document the review and observation. The results of this study that funding, fulfillment of spare parts, facilities and infrastructure, policies and information systems have an effect on the implementation of medical equipment maintenance management system.
