"Nyeri pasca bedah merupakan salah satu penyebab nyeri akut yang paling umum. Penanganan nyeri yang efektif merupakan komponen fundamental dari pelayanan pasien yang berkualitas. Di RS Prima Medika belum ada protokol standar penanganan nyeri pasca bedah. Tujuan penelitian ini untuk mengukur intensitas nyeri pasien pasca bedah di RS Prima Medika, membuat protokol standar penanganan nyeri pasca bedah di RS Prima Medika dalam mewujudkan pelayanan yang berkualitas dan aman bagi pasien.
Penelitian ini merupakan penelitian deskriptif kualitatif dengan teknik pengambilan data melalui observasi intensitas nyeri pada pasien pasca laparotomi dan wawancara mendalam dengan informan serta telaah dokumen.
Hasil penelitian menunjukkan intensitas nyeri pada periode 24 jam pasca bedah adalah nyeri ringan, pada periode 48 jam pasca bedah intensitas nyeri bervariasi antara nyeri ringan sampai sedang dengan prosedur penanganan nyeri yang tidak terarah dengan baik. Diperlukan suatu SOP untuk memberikan penanganan nyeri pasca bedah yang optimal.

Fundamental component to serve a quality service to the patient. There is no Standard operating procedure in pain management in Prima Medika Hospital. The aim of this study is to measure the intensity of pain in postoperative patient in this hospital, to arrange a standard operating procedure for postoperative pain management to serve a quality and safe service to the patient.
This is a qualitative descriptive study where data were collect by observed pain intensity in post laparotomy patient, in-depth interviews with the informans and reviewed documents.
Result show that pain intensity in 24 hours after surgery were mild, and in 48 hours the pain intensity were mild to moderate with no proper protocol. A standard operatimg procedure is needed to give the best pain management to the patient

"Keselamatan pasien merupakan hal yang sangat mendasar dalam pelayanankepada pasien di rumah sakit. Sebagai langkah awal dalam upaya meningkatkankeselamatan pasien di rumah sakit adalah dengan mengukur budaya keselamatanpasien. Tujuan penelitian ini adalah untuk mengetahui hubungan kepemimpinantransformasional, kerjasama tim, dan kesadaran individual dengan budaya keselamatanpasien di Rumah Sakit Prima Medika RSPM Denpasar. Metode yang digunakanadalah metode campuran mix method. Pendekatan kuantitatif menggunakan jenispenelitian potong-lintang cross-sectional. Kuesioner dibagikan kepada sampelsebanyak 218 responden. Pada pendekatan kualitatif dilakukan penelusuran lebih lanjutterhadap hal-hal yang dirasa masih belum terjawab, untuk melengkapi penjelasan hasilpenelitian kuantitatif.
Analisis statistik menggunakan Structural Equation Modelling SEM, dengan program STATA-SE 12.1. Wawancara mendalam dilakukan denganDireksi RSPM dan pegawai yang terkait, untuk konfirmasi hasil penelittian. Hasilanalisis menunjukkan variabel independen yang saling berhubungan yaituKepemimpinan Transformasional, Kesadaran Individual, dan Kerjasama Tim, danketiganya berhubungan secara bemakna dengan variabel dependen BudayaKeselamatan Pasien p.

Patient safety is very basic in the service to patients in the hospital. As a firststep in improving patient safety in hospitals is by measuring the patient 39 s safety culture.The purpose of this research is to know the relationship of transformational leadership,teamwork, and individual awareness to patient safety culture at Prima Medika Hospital RSPM Denpasar. The method used is mix method. The quantitative approach usescross sectional research. Questionnaires were distributed to a sample of 218respondents. In a qualitative approach, further searches on things that remainunanswered, to complement the explanation of the results of quantitative research.
Statistical analysis using Structural Equation Modeling SEM, with STATA SE 12.1program. In depth interviews were conducted with the RSPM Board of Directors andrelevant employees, to confirm the results of the study. The results of the analysis showthat the independent variables are Transformational Leadership, Individual Awareness,and Team Cooperation, and all three are related significantly to the dependent variableof Patient Safety Culture p.

"Rumah sakit merupakan tempat pelayanan kesehatan yang komplek dan padat resiko, keselamatan pasien sangat penting untuk meningkatan mutu rumah sakit, salah satu caranya adalah dengan melaporkan Kejadian Tidak Diharapkan (KTD). Perawat memiliki kontak paling lama dengan pasien sehingga menjadi komponen terpenting dalam pelaporan KTD. Penelitian ini bertujuan untuk mengetahui faktor yang mempengaruhi persepsi melaporan KTD di Rumah Sakit Prima Medika Denpasar tahun 2020. Rumah Sakit Prima Medika Denpasar merupakan rumah sakit swasta tipe C. Penelitian ini merupakan penelitian kuantitatif menggunakan desain penelitian cross sectional dengan sampel penelitian 140 perawat yang dilaksanakan pada bulan Juni tahun 2020. Didapatkan gambaran persepsi perawat yang tidak pernah melaporkan KTD sebesar 63.6% karena tidak pernah menemukan kejadian KTD atau mungkin pernah mendapat kejadian KTD tetapi tidak berani mealpor. Variabel yang berhubungan signifikan dengan persepsi melaporkan KTD oleh perawat adalah variabe sikap (p value = 0.002), pendidikan (p value = 0,046), porsi beban kerja berat (p value = 0,003 ) dan porsi beban kerja ringan (p value = 0,026 ). Variabel yang paling berpengaruh adalah sikap perawat ( OR 4,33 ). Saran antara lain adalah rumah sakit menumbuhkan sikap positif perawat dalam hal melaporkan KTD dengan memberikan penghargaan kepada yang melapor, rumah sakit melakukan pelatihan keselamatan pasien secara rutin dan regular, shif kerja malam selama 12 jam perlu di evaluasi.

Hospital is complex and full of risk medical facility. Adverse events reporting is one part of the patient safety system that has an important roles to improve the hospital’s quality. Nurses have the longest contact’s time with patients so they become the most important component in reporting the Adverse Event reporting. This study aims to determine the factors that affects perception in adverse event reporting at Prima Medika Hospital Denpasar in 2020. Prima Medika Hospital is a type C private hospital. This study is quantitative study using a cross-sectional design with 140 nurses as samples and conducted in June 2020. The nurses’s perception who never report adverses event is 63.6%, assuming they never found any adverse event or maybe have found it but not have no courage to report it. The variable that significantly associated with perceptions on adverse event reporting by nurse are attitude (p value=0.002), education (p value=0.046), and workload ( heavy workload with p value= 0.003 and light workload with p value=0.026). The most affecting variable is the nurses attitude (OR 4.33). It is proposed for the hospital to build the nurses positive attitude to report adverse events one through giving appreciation to those who report the adverse event, hospital needs to give regular patient safety training to maintain the nurse knowledge, and to revisit the 12-hour nightshift which is regarded as unusual long hours.. "

"Rumah sakit dalam memberikan pelayanan kesehatan masyarakat harus dapat menjaga mutu dan kualitas. Salah satu upaya pemerintah untuk meningkatkan mutu pelayanan rumah melalui akreditasi. Akreditasi wajib dilakukan dan merupakan syarat ijin operasional sesuai aturan pemerintah. Rumah Sakit Prima Medika melakukan persiapan akreditasi KARS 2012 yang terdiri dari 4 sasaran pokok, namun penelitian ini pada dua sasaran yaitu sasaran keselamatan pasien dan Milenium Development Goals. Tujuan penelitian ini untuk mengetahui tahapan, masalah dan monitoring dalam proses persiapan akreditasi. Penelitian ini menggunakan metode kualitatif melibatkan partisipan sebanyak 7 orang yang terlibat dalam kelompok kerja akreditasi. Pengumpulan data dengan teknik wawancara mendalam dan penelusuran dokumen, dianalisis menggunakan content analysis. Hasil wawancara persiapan akreditasi dimulai komitmen dari seluruh jajaran rumah sakit, kelompok kerja akreditasi ditetapkan direktur, penyusunan dokumen sesuai elemen penilaian akreditasi, sosialisasi pada staf medis maupun non medis, implementasi sangat penting karena penilaian lebih terfokus pada pelaksanaan dilapangan, monitoring dan evaluasi dengan laporan monev, indikator mutu dan turun langsung ke lapangan. Kesimpulan tahapan persiapan yang penting adalah komitmen seluruh jajaran rumah sakit, melakukan pendekatan kepada dokter spesialis secara formal maupun informal, menambah pelatihan internal.

Hospitals in providing public health services should be able to maintain quality of services. One of the government's efforts to improve the quality of home services through accreditation, which is compulsory and required by government regulation. This study focus on two goals of Prima Medika Hospital in preparation of KARS 2012 accreditation: International Patient Safety Goals and Millennium Development Goals. The purpose of this study is to determine the stages, problems and monitoring in the preparation process of accreditation on the patient's safety goals and the Millennium Development Goals. This research uses qualitative method by involving 7 participants working group of accreditation. Data collected by in-depth interview technique and document tracing, then analyzed using content analysis. The results of the interviews show that the preparation phase begins with the commitment of all hospitals, the accreditation working group established by the director, the compilation of documents of the assessment elements according to the accreditation standards, program dissemination to medical and non-medical staff, then the implementation. The next stage are monitoring and evaluation, through monev report, quality indicator and field inspection. In conclusions, the very important stages in preparation process are: commitment from all level of hospital, approaches to the specialist doctors formally or informally, more internal training."

"Pelayanan pada pasien covid 19 di rumah sakit membutuhkan biaya perawatan yang tinggi, perlu adanya pengeloalaan yang baik dan pemantauan awal perawatan hingga pasien tersebut pulang. Agar kepastian klaim pasien tersebut layak untuk diterima atau tidak dispute, dibutuhkan adanya manajemen pengendalian klaim dan pengendalian biaya perawatan. Penelitian ini untuk menganalisis penerapan manajemen  pengendalian klaim dispute dan biaya perawatan Pasien Covid 19dari faktor Input, Proses dan Output .Penelitian menggunakan pendekatan kualitatif melalui telaah dokumen, observasi dan wawancara mendalam. Hasil penelitian menunjukan pada input :kurangnya SDM di Ruang Isolasi, kebijakan sering berubah dan berlaku surut, keterlambatan pencairan biaya yang telah dianggarkan, sarana dan prasarana yang kurang memadai, Faktor Proses : fungsi kontrol masih belum berjalan dengan baik, output yang dihasilkan : pengendalian biaya pelayanan baik namun masih terdapat klaim dispute dengan penyebabnya adalah hasil swab yang tidak terlampir, resume medis yang tidak lengkap, upload berkas tidak lengkap, input berkas tidak sesuai, berkas identitas pasien tidak lengkap. Saran dibutuhkan peningkatan pengetahuan pada kegiatan klaim covid-19 tentang persyaratan klaim covid-19 di rumah sakit prima, perlu adanya seorang person in contact (PIC) yang bertanggung jawab klaim covid-19 ,pemenuhan sarana dan prasarana, jumlah SDM dan waktu penagihan klaim lebih awal dari jadwal yang telah ditentukan.

In order to ensure that the patient's claim is eligible to be accepted or not to be disputed, it is necessary to have claims control management and treatment cost control. This study is to analyze the application of dispute claim control management and treatment costs for Covid 19 patients seen from the Input, Process and Output factors. The study uses a qualitative approach through document review, observation and in-depth interviews. The results showed the inputs: lack of human resources in the Isolation Room, policies / SOPs that often change and apply retroactively, delays in disbursing budgeted costs, inadequate facilities and infrastructure, Process Factors: control functions that are still not running well, outputs that are not working properly. Resulted: service cost control is good but there are still claims of dispute with the causes being swab results that are not attached, incomplete medical resume, incomplete file upload, incorrect file input, incomplete patient identity file. Suggestions are needed to increase the knowledge of human resources involved in Covid-19 claim activities regarding the requirements for claiming COVID-19 at a prime hospital, it is necessary to have a person in contact (PIC) who is responsible for Covid-19 claims, fulfillment of facilities and infrastructure, number of human resources and time. claim collection earlier than the predetermined schedule.

"Pembedahan mayor abdomen yang berhubungan dengan kanker Cholangiocarcinoma (CCA) seringkali dikaitkan dengan kejadian perawatan ICU pasca pembedahan dan mengakibatkan intensitas nyeri yang tinggi pasca operasi. Penatalaksanaan nyeri yang tidak optimal dapat berkontribusi pada komplikasi dan memiliki efek buruk pada kesembuhan pasien. Oleh karena itu, diperlukan terapi komplementer yang berfokus pada kenyamanan dan preferensi pasien sebagai bentuk optimalisasi penatalaksanaan nyeri pasien CCA pasca pembedahan abdomen di ICU. Karya Ilmiah Akhir ini bertujuan untuk memaparkan hasil praktik berupa asuhan keperawatan medikal bedah pada pasien tumor distal Common Bile Duct (CBD) post laparatomy double bypass dengan penerapan manajemen nyeri menggunakan aromaterapi lavender di ICU Rumah Sakit. Intervensi ini dilakukan pada pasien usia 25 tahun dengan ikterik obstruktif et causa tumor distal Common Bile Duct (CBD) suspek ganas kesan unresectable. Setelah dilakukan intervensi selama tiga hari, didapatkan kesimpulan bahwa aromaterapi lavender dapat digunakan sebagai intervensi manajemen nyeri non-farmakologi yang menyempurnakan terapi farmakologi dalam menurunkan nyeri pada pasien sadar pasca operasi laparatomy double bypass di ICU.

Major abdominal surgery related to Cholangiocarcinoma (CCA) cancer is often associated with post-surgical ICU admission and results in high pain intensity post-surgery. Suboptimal pain management can contribute to complications and have an adverse effect on patient recovery. Therefore, complementary therapy is needed that focuses on patient comfort and preferences as a form of optimizing pain management for CCA patients after abdominal surgery in the ICU. This Final Scientific Work aims to present practical results in the form of medical surgical nursing care for patients with distal Common Bile Duct (CBD) tumors post double bypass laparotomy with the application of pain management using lavender aromatherapy in the ICU Hospital. This intervention was carried out on a 25-year-old patient with obstructive jaundice caused by a distal Common Bile Duct (CBD) tumor, which was suspected to be malignant and unresectable. After three days of intervention, it was concluded that lavender aromatherapy could be used as a non-pharmacological pain management intervention that complements pharmacological therapy in reducing pain in conscious patients after double bypass laparotomy surgery in the ICU."

"Penelitian ini bertujuan untuk mendapatkan gambaran faktor kualitas pelayanan dengan kepuasan pasien berdasarkan karakteristik pasien (jenis kelamin, umur, wilayah tempat tinggal, suku, status marital, pendidikan, pekerjaan, pendapatan, ruang perawatan, jaminan/jenis pembayaran), mengetahui hubungan antara faktor kualitas dengan kepuasan pasien rawat inap, dan mengetahui faktor kualitas pelayanan yang paling dominan mempengaruhi kepuasan pasien rawat inap. Penelitian ini adalah penelitian kuantitatif dengan desain studi cross sectional. Responden berjumlah 100 orang dan merupakan pasien yang dinyatakan sudah boleh pulang rawat inap di Rumah Sakit Sentra Medika Cibinong. Pengumpulan data dengan menggunakan kuesioner yang dilakukan selama bulan Mei-Juni 2014 di Instalasi Rawat Inap Rumah Sakit Sentra Medika Cibinong. Data terkumpul dianalisis dengan metode analisis univariat, bivariat Uji Chi Square, dan analisis multivariat uji regresi linier berganda dengan menggunakan pemodelan Uji T-Test (Independent Sample T-Test).
Hasil penelitian menunjukkan bahwa ada hubungan yang bermakna antara faktor kualitas pelayanan hormat dan kepedulian, efektifitas dan kelanjutan, ketepatan, informasi, efisiensi, efektifitas - makanan, kesan pertama, dan keragaman staf dengan kepuasan pasien. Berdasarkan hasil analisis bivariat didapatkan bahwa karakteristik umur, status marital, pekerjaan, pendapatan, ruang perawatan dan jaminan/jenis pembayaran muncul sebagai faktor pengganggu (confounding) dari kualitas pelayanan. Selain itu juga didapatkan bahwa karakteristik responden bukan merupakan faktor confounding untuk kepuasan pasien. Dari analisis multivariat terbukti bahwa faktor kualitas pelayanan Keragaman Staf merupakan variabel yang paling dominan atau paling besar pengaruhnya terhadap kepuasan pasien.

This research aims to describe the service quality factors in relation with patient satisfaction by patient characteristic (gender, age, region of residence, ethnicity, marital status, education, occupation, income, inpatient rooms, type of payment), to discover the relationship between service quality factors with inpatient satisfaction, and to discover the most dominant factor of service quality that influence the inpatient satisfaction. This research is a quantitative study with a cross-sectional study design. Respondent amounted to 100 people and is a patient who otherwise has been allowed to go home in the inpatient of Sentra Medika Hospital at Cibinong. Data collection using questionnaires conducted during the months of May-June 2014 in the inpatient Sentra Medika Hospital at Cibinong. The data collected were analyzed with univariate, bivariate Chi-Square Test, and multivariate linear regression modelling using the Independent Sample T-Test.
The result showed that there is a significant relationship between service quality factors Respect and Caring, Effectiveness and Continuity, Appropriateness, Information, Efficiency, Effectiveness - Meals, First Impression, and Staff Diversity with the inpatient satisfaction. Based on the results of the bivariate analysis showed that the characteristic of age, marital status, occupation, income, inpatient rooms, type of payment appears as confounding factor of the service quality. It also found that the characteristic of the respondent is not a confounding factor for patient satisfaction. From the multivariate analysis proved that the service quality factor Staff Diversity is the most dominant variable or greatest effect on inpatient satisfaction.

Relaksasi Benson merupakan pengembangan metode respons relaksasi dengan
melibatkan faktor keyakinan pasien yang dapat mengurangi nyeri pasca bedah. Relaksasi
Benson berfokus pada kata atau kalimat tertentu yang diucapkan berulang kali dengan
ritme yang teratur disertai sikap pasrah kepada Tuhan sesuai dengan keyakinan pasien.
Penelitian ini bertujuan untuk mengetahui efektifitas Relaksasi Benson dalam
menurunkan nyeri pasien pasca bedah TUR Prostat. Metode penelitian ini adalah
quasi-eksperimental dengan pre test and post test design with control group.
Pengambilan sampel dilakukan dengan consecutive sampling. Jumlah sampel 14 orang,
7 orang kelompok intervensi dan 7 orang kelompok kontrol. Kelompok intervensi
diberikan kombinasi Relaksasi Benson dan terapi analgesik dan kelompok kontrol hanya
diberikan terapi analgesik. Intervensi Relaksasi Benson dilakukan setelah pemberian
analgesik dengan durasi 15 menit setiap hari selama dua hari. Sebelum dan sesudah
intervensi pada kelompok intervensi dan kelompok kontrol dilakukan pengukuran nyeri
dengan Numeric Rating Scale. Sehari sebelum operasi dan sehari sesudah operasi diukur
kecemasan menggunakan Visual Analog Scale. Hasil penelitian menunjukkan bahwa
kombinasi Relaksasi Benson dan terapi analgesik lebih efektif untuk menurunkan rasa
nyeri pasca bedah pada pasien TUR Prostat dibandingkan hanya terapi analgesik saja
(p=0,019). Karakteristik budaya dan kecemasan tidak berkontribusi terhadap nyeri pasca
bedah TUR Prostat (p >0,05). Implikasi dari penelitian ini adalah Relaksasi Benson
dapat digunakan untuk mengurangi nyeri pasca bedah TUR Prostat elektif dan perlu
direplikasikan dan dikembangkan lagi.

ABSTRACT
Benson Relaxation is the development ofe response relaxation method by involving
patient belief factor to relieve postoperative pain. It focuses on certain words or
sentences pronounced many times in a regular rhtym followed by resignation to The
God as patient belief. This research was aimed to explore effectiveness of Benson
Relaxation in relieving postoperative pain TUR prostate. The method used in this study
was quasi experimental using pre test and post test design with control group. A total of
14 consecutive samples participated in this study, devided into two groups , intervention
and control group, seven participants respectively. Those in intervention group received
Benson Relaxation combined with analgesic therapy where as those control group given
analgesic therapy alone. Benson Relaxation intervention given after analgesic was
taken, for 15 minutes every day for two days. Before and after the intervention for both
groups, pain scale was measured by using Numeric Rating Scale. A day before and
after the surgery, anxiety level was measured by using Visual Analogue Scale. The
results revealed that combination Benson Relaxation and analgesic therapy was more
analgesic therapy alone (p=0,019). Culture and anxiety factors did not contribute to
postoperative pain of TUR Prostate (p>0,05). The Implication of this research was
Benson Relaxation can be employed to relieve postoperative pain of elective TUR
Prostate, and it is needed for further replication and development."
